Post-translational polymodification of β1-tubulin regulates motor protein localization in platelet production and function

Abdullah O. Khan, Alexandre Slater, Annabel Maclachlan, Phillip L. R. Nicolson, Jeremy A. Pike, Jasmeet S. Reyat, Jack Yule, Rachel Stapley, Julie Rayes, Steven G. Thomas, Neil Morgan

Summary: This study investigates the role of specific tubulin isoforms and post-translational modifications in platelet production and function. The research reveals distinct patterns of polymodification in megakaryocytes and platelets, which are regulated by specific enzymes. These findings provide insights into the spatial regulation of motor proteins and cytoskeletal reorganization in platelet formation and thrombus formation.

Platelet transfusion and anticoagulation in hematological cancer-associated thrombosis and thrombocytopenia: The CAVEaT multicenter prospective cohort

Stephen Booth, Michael Desborough, Nicola Curry, Simon Stanworth

Summary: This study describes the practice and outcomes of patients with hematological cancer, venous thromboembolism (VTE), and thrombocytopenia. The results demonstrate the heterogeneity of management approaches and high rates of bleeding in this patient population. Randomized data is needed to inform optimal management strategies.

Intraoperative cell salvage using swab wash and serial thromboelastography in elective abdominal aortic aneurysm surgery involving massive blood loss

Mark D. Stoneham, Antonio Barbosa, Keith Maher, Paul Douglass, Michael J. R. Desborough, Steve Von Kier

Summary: This study investigated the usage of allogeneic blood components and coagulopathy in 53 patients who experienced >50% blood loss during elective abdominal aortic aneurysm surgery. The results showed that with a tailored cell salvage technique, most patients did not require allogeneic blood component transfusion, and there was no evidence of coagulopathy. This study suggests possible alternatives to the current massive transfusion protocols for managing elective vascular surgical patients.

Drug-induced thrombotic thrombocytopenic purpura: A systematic review and review of European and North American pharmacovigilance data

Jeremy Schofield, Sara Hosseinzadeh, Kieran Burton, Sue Pavord, Tina Dutt, Carolyn Doree, Wen Yuen Lim, Michael J. R. Desborough

Summary: Many medications have been reported to be associated with TTP, but there is a lack of synthesis of evidence regarding drug-induced TTP. This systematic review demonstrates a paucity of evidence for any drug causing DI-TTP, and highlights the need for standardized reporting of potential drugs causing TTP.

Real world reduction in red cell transfusion with restrictive transfusion threshold in haematology inpatients

Omer Pervaiz, Jay Dhanapal, Lakshmi Pillai, Sue Pavord, Heather Leary, Toby Eyre, Andrew Peniket, Julie Staves, Paolo Polzella, Michael J. R. Desborough

Summary: The aim of this study was to assess the reduction in red cell transfusions following a change in the red cell transfusion threshold for haematology inpatients from 80 to 70 g/L. The results showed a 23% reduction in the total number of red cells administered per month after the change in threshold to 70 g/L or less from 80 g/L.

Summary: This study identifies the role of platelet FXIII-A in platelet function. Platelet FXIII-A enhances fibrinogen binding to the platelet surface and improves platelet aggregation and thrombus formation. FXIII-A is also involved in platelet spreading and clot retraction.

Desmopressin for patients with spontaneous intracerebral haemorrhage taking antiplatelet drugs (DASH): a UK-based, phase 2, randomised, placebo-controlled, multicentre feasibility trial

Summary: This study assessed the feasibility of randomizing patients with spontaneous intracerebral hemorrhage who are taking antiplatelet drugs to receive desmopressin or placebo. The results showed that randomizing patients to receive desmopressin or placebo was feasible. This finding supports the need for a definitive trial to determine if desmopressin improves outcomes in patients with intracerebral hemorrhage on antiplatelet drug therapy.
