Summary: This study investigated the clinical course of lung cancer patients who developed anti-cancer therapy-related pneumonitis, revealing that patients who received some anticancer therapy after pneumonitis had significantly longer survival times. Summary: Checkpoint blockade with Pembrolizumab has proven effective in treating advanced non-small cell lung cancer. However, some patients experience high-grade immune related adverse events (irAEs) during treatment. This study reveals that a deficient Breg checkpoint fails to control self-reactive T cell activity and auto-antibody formation in these patients, leading to severe auto-inflammatory sequelae. Pre-therapy B cell profiling may help identify patients at high risk of developing severe irAEs.

Efficacy and safety of consolidation durvalumab after chemoradiation therapy for stage III non-small-cell lung cancer: a systematic review, meta-analysis, and meta-regression of real-world studies

Summary: The current review summarized real-world evidence on the efficacy and toxicity of consolidation durvalumab for stage III unresectable non-small cell lung cancer (NSCLC) after curative chemoradiotherapy. A total of 23 studies with 4,400 patients were included, showing that the 1-year overall survival (OS) and progression-free survival rates (PFS) of durvalumab were 85% and 60% respectively. Adverse events such as pneumonitis, endocrine, cutaneous, musculoskeletal, and gastrointestinal events were also reported. The results indicate that durvalumab has consistent short-term efficacy and safety with the PACIFIC trial.

Risk Factors for Refractory Immune Checkpoint Inhibitor-related Pneumonitis in Patients With Lung Cancer

Summary: Checkpoint inhibitor-related pneumonitis (CIP) is a rare but fatal side effect of immune checkpoint inhibitors, and identifying patients at risk of refractory CIP is important for treatment control. In a retrospective analysis of 60 lung cancer patients with CIP, 26.7% had refractory CIP. Patients with refractory CIP had a shorter onset time and higher lactate dehydrogenase (LDH) levels. An LDH level >320 U/L at CIP onset was an independent risk factor for refractory CIP. Monitoring LDH levels during immune checkpoint inhibitors treatment is recommended.

This case emphasizes the importance of considering radiation recall pneumonitis in the differential diagnosis of lung consolidation during immunotherapy and suggests that it may anticipate more extensive immune-checkpoint inhibitor-induced pneumonitis.

However, the use of immune checkpoint inhibitors (ICIs) in NSCLC tumors with a driver mutation has been debated. Trials investigating the combination of these therapies with TKIs were halted due to concerns of increased toxicity.

Xenon-Enhanced Ventilation Computed Tomography for Functional Lung Avoidance Radiation Therapy in Patients With Lung Cancer

Summary: This phase 2 trial investigated the effectiveness of xenon-enhanced ventilation computed tomography (XeCT)-guided functional-lung-avoidance radiation therapy in reducing radiation pneumonitis (RP) in lung cancer patients receiving chemoradiation therapy. The study found that the functional-lung-avoidance plan (fAP) significantly reduced the rate of grade ≥2 RP compared to the standard plan (SP), and demonstrated positive outcomes in terms of local control, disseminated failure-free survival, and overall survival rates.

Racial Differences in Survival Among Advanced-stage Non-small-Cell Lung Cancer Patients Who Received Immunotherapy: An Analysis of the US National Cancer Database (NCDB)

Summary: This study aimed to evaluate the association between race and all-cause mortality among non-small-cell lung cancer (NSCLC) patients who received immunotherapy. The results showed that Non-Hispanic Black patients had a lower risk of death compared to Non-Hispanic White patients. Among those with comorbidities and living in regions with lower median income, Non-Hispanic Black patients had a lower risk of death. Overall, NH-Black patients experienced higher survival compared with NH-White patients.

Predictive factors and prognosis of immune checkpoint inhibitor-related pneumonitis in non-small cell lung cancer patients

Summary: The study aimed to investigate the influencing factors and prognosis of immune checkpoint inhibitor-related pneumonitis (CIP) in advanced non-small cell lung cancer (NSCLC) patients. The results showed that low pretreatment hemoglobin (HB) and albumin (ALB) levels were independent risk factors for CIP. A high neutrophil-to-lymphocyte ratio (NLR) level, a low ALB level, and the development of CIP were independent risk factors for the prognosis of advanced NSCLC patients treated with immune checkpoint inhibitors (ICIs).

Clinical Management and Outcome of Grade III Pneumonitis after Chemoradioimmunotherapy for Inoperable Stage III Non-Small Cell Lung Cancer-A Prospective Longitudinal Assessment

Summary: In a cohort of 38 patients receiving chemoradioimmunotherapy (CRT-IO), 16% of patients developed grade 3 pneumonitis after completion of CRT. Pneumonitis manifested as diffuse ground glass opacities and consolidations in CT imaging, leading to a decrease in diffusion capacity. Treatment with high dose corticosteroids resulted in symptom improvement and restoration of diffusion capacity in the majority of patients.
