Summary: This study aimed to identify factors and strategies that hindered and facilitated the implementation of new critical care practices during the COVID-19 pandemic. Through interviews with critical care leaders in the US, the study found that staff resilience, commitment, and innovation, along with collaborative feedback and decision-making mechanisms between leadership and frontline staff, were important facilitators of implementation. On the other hand, barriers included lack of access to reliable and transferable information, available resources, and uncollaborative leadership and communication styles.

Summary: In this study, the multilevel barriers to the widespread adoption of preconception reproductive genetic carrier screening (RGCS) in the United States were explored through key informant interviews. The barriers were identified at different levels, including test characteristics, patients and couples, clinicians and care teams, and the external health care and policy environments. Potential solutions were discussed by the informants, which may help to alleviate the obstacles.

Summary: This study investigated the barriers and enablers in the implementation process of the National Chest Pain Centers Program (NCPCP) in China. The barriers mainly included the complexity of the intervention, low flexibility of requirements, and a lack of recognition of chest pain in patients with acute coronary syndromes (ACS). The enablers of intervention implementation included inner motivation for change, evidence strength and quality of intervention, and involvement of hospital leaders.

Summary: This study conducted focus groups, workshops, and project steering committee meetings with HCPs and patients from 2 outpatient clinics to identify potential determinants of implementation for a digital patient-provider communication intervention. Data were analyzed into 18 CFIR constructs, leading to important intervention tailoring and key aspects for an implementation plan, including adjustments to digital features, fitting clinical workflow, and involving key stakeholders in the implementation process.

Summary: According to a study conducted at Vienna General Hospital from 2008 to 2018, Hepatitis E virus (HEV) infection can lead to severe viral hepatitis and liver failure. Despite an increase in HEV detection rates, the number of diagnosed HEV infections at Vienna General Hospital remained constant. While approximately half of the patients with severe acute HEV infection required hospitalization, admissions to the intensive care unit (ICU) and short-term mortality were low.

Summary: The objective of this study is to investigate the effects and consequences of surgical treatment of patients with musculoskeletal malignancies on everyday life. Data collection and analysis were conducted using a modified form of grounded theory. The study involved 16 interviews and the main category that emerged from the analysis emphasized the importance of implementing psychosocial care involving the social environment and the role of relatives/partners in the recovery process. The study also highlighted the need for improved communication between specialists and family physicians.

Summary: The study aimed to evaluate the systemic and hepatic renin-angiotensin system (RAS) fingerprint in advanced chronic liver disease (ACLD). It was found that patients with decompensated ACLD (dACLD) had significantly higher levels of angiotensin I, angiotensin II, and aldosterone in their plasma. Increased angiotensin 1-7 was associated with severe liver disease, portal hypertension, endothelial dysfunction, and inflammation.

Summary: Findings from two conjoint experiments conducted in Austria and Italy revealed that tailored vaccination campaigns based on previous vaccination status are more effective. For the unvaccinated, campaign messages conveying community spirit had a positive effect, while offering positive incentives was pivotal to the decision-making of those vaccinated once or twice. Among the triple vaccinated, vaccination readiness increased when adapted vaccines were offered, but costs and medical dissensus reduced their likelihood to get vaccinated. Failing to mobilize the triple vaccinated is likely to result in booster vaccination rates falling short of expectations.

Summary: In this study, the researchers evaluated the effect of polyhydroxylated bile acids (BAs) on the inflammation and fibrosis caused by Schistosoma mansoni infection. They found that mice lacking the Bsep gene showed less hepatic inflammation and fibrosis after infection, which could be attributed to the changes in bile acid composition and blood pH resulting from the loss of Bsep gene, affecting parasite fecundity and thus attenuating the development of S. mansoni-induced hepatic inflammation and fibrosis.

Summary: The GLP-2 analogue teduglutide has hepatoprotective and antifibrotic effects in a mouse model of sclerosing cholangitis. GLP-2 treatment decreases hepatic inflammation and fibrosis, and activates the intestinal FXR-FGF15/19 signaling axis, resulting in reduced expression of Cyp7a1 and increased expression of Cyp2c70 in the liver, contributing to its protective effects.

Summary: This study aimed to compare the radiological features between PSVD and cirrhosis. The results showed that compared to cirrhosis, patients with PSVD were younger and had lower HVPG, liver stiffness, and MELD. CT/MRI results revealed that PSVD patients had more intrahepatic portal tract abnormalities, FNH-like lesions, and abnormal liver morphology. A specific radiological feature of PSVD identified in gadoxetic acid-enhanced MRI was periportal hyperintensity in the HBP.
