Summary: This study examined the impact of anti-HLA antibodies against HLA-DP and -DQ on cord-blood transplantation outcomes. The presence of DSAs against HLA-DP and -DQ was associated with lower engraftment rates, longer engraftment time, increased risk of bacterial infection, and reduced survival after transplantation.

Summary: A nationwide retrospective study revealed that cytomegalovirus reactivation (CMVR) is associated with a lower relapse risk in acute myeloid leukemia (AML) and acute lymphoblastic leukemia (ALL) patients after allogeneic hematopoietic cell transplantation (HCT). This finding suggests important implications for CMV prophylaxis strategies after HCT.

Summary: Although exposure-directed busulfan (BU) dosing can improve transplantation outcomes, there is still large variability in BU exposure with test dose alone. We conducted a trial comparing a combined test dose and therapeutic drug monitoring strategy with a fixed-dose group. The combined strategy effectively controlled BU exposure and showed favorable outcomes in patients receiving FLU-containing regimen.

Summary: This study presents two pedigrees, one diagnosed with familial platelet disorder with predisposition to acute myeloid leukemia (FPD/AML) molecularly and another highly suspected. Both pedigrees showed a family history of thrombocytopenia, platelet dysfunction, and hematological malignancies, highlighting the importance of searching for gene mutations associated with germline predisposition.

Other iatrogenic immunodeficiency-associated lymphoproliferative disorders in a patient with anti-melanoma differentiation-associated gene 5-positive dermatomyositis: A case report and systematic literature review

Summary: A 58-year-old man with MDA5-DM developed EBV-associated malignant lymphoma during immunosuppressive therapy. EBV reactivation played a role in the pathogenesis of LPD in this case. Prompt chemotherapy and discontinuation of immunosuppressants resulted in complete remission. This is the first reported case of MDA5-DM complicated with OIIA-LPD. Among 19 similar cases, regression of LPD occurred with withdrawal of immunosuppressants in 7 patients, chemotherapy was required in 9 patients, and 5 patients died.

Summary: Therapy-related acute myeloid leukemia (t-AML) is a late complication of chemotherapy (CHT) and/or radiotherapy (RT) for primary malignancy. Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a curative approach for t-AML. This study evaluates the relationship between characteristics of primary malignancy and allo-HSCT outcomes to establish the optimal strategy.

Impact of Antimicrobial Drug-Drug Interactions on Acute Kidney Injury after Allogeneic Hematopoietic Cell Transplantation

Summary: Drug-drug interactions (DDIs) after allogeneic hematopoietic stem cell transplantation (allo-HSCT) are a major cause of acute kidney injury (AKI). This retrospective analysis of 465 allo-HSCTs in 416 patients found that AKI was significantly associated with reduced overall survival and increased transplantation-related mortality. The use of ciprofloxacin, cefepime, tazobactam/piperacillin, meropenem, vancomycin, liposomal amphotericin B, ganciclovir, and foscarnet significantly increased the incidence of AKI, and specific combinations of drugs synergistically increased the risk of AKI. These findings highlight the importance of avoiding certain drug combinations to preserve renal function and reduce AKI-related morbidity and mortality.

Summary: This study found that NOD2 activation reduces TLR9-induced IFN-alpha production through DUBA. Activation of NOD2 in hematopoietic cells can protect mice from TLR9-induced exacerbation of DSS-induced colitis. The expression of IFN-alpha and ISGs in colonic mucosa is associated with the active and remitted phases of inflammatory bowel disease.
