Spleen Stiffness Probability Index (SSPI): A simple and accurate method to detect esophageal varices in patients with compensated liver cirrhosis

Intruduction and objectives: Recent findings pointed out that even low-risk esophageal varices (EVs) are markers of severe prognosis. Accordingly, we analyzed spleen stiffness (SS) as a non-invasive method to predict EVs of any grade in a cohort of patients with compensated liver cirrhosis. Method: We measured SS and liver stiffness (LS) using point-Shear-Wave Elastography (pSWE) with Philips Affiniti 70 system in 210 cirrhotic patients who had undergone endoscopic screening for EVs. We compared SS and LS predictive capability for EVs of any grade. Results: SS was higher in cirrhotic patients with EVs if compared to patients without EVs (p < 0.001). The cut-off analysis detected 31 kPa (100% sensitivity and negative predictive value) as the value to rule-out EVs and 69 kPa (100% specificity and positive predictive value) to rule-in EVs. Besides, we developed the Spleen Stiffness Probability Index (SSPI), that can provide a probability of presence/absence of EVs. SSPI was the best model according to all discriminative and calibration metrics (AIC = 120, BIC = 127, AUROC = 0.95, Pseudo-R2 = 0.74). SS demonstrated higher correlation with spleen bipolar diameter and spleen surface (r = 0.52/0.55) if compared to LS (r = 0.30/0.25) - and with platelet count as well (r = 0.67 vs r = 0.4). Conclusion: SS showed significantly higher performance than other parameters, proving to be the best non-invasive test in the screening of EVs: by directly applying SS cut-off of 31 kPa, our department could have safely avoided endoscopy in 36% of patients. Despite cut-off analyses, it was possible to create a probability model that could further stratify low-risk from high-risk patients (for any grade of EVs). Correction and Prevention of Hyponatremia in Patients With Cirrhosis and Ascites: Post Hoc Analysis of the ANSWER Study Database

Summary: This study assessed the impact of long-term albumin administration on hyponatremic patients with ascites enrolled in the ANSWER trial. The normalization rate of baseline hyponatremia and the 18-month incidence rate of at least moderate hyponatremia were evaluated. The results showed that albumin treatment had a higher hyponatremia normalization rate compared to standard medical treatment (45% vs 28%, P = 0.042 at 1 month). Long-term albumin administration ensured a lower incidence of at least moderate hyponatremia than standard medical treatment (incidence rate ratio: 0.245 [CI 0.167-0.359], P < 0.001).

Summary: This study assessed long-term mortality and its association with chronic alcohol-related diseases in patients with acute alcoholic intoxication. The results showed that patients with alcohol use disorder, substance use disorder, and liver cirrhosis had higher death rates compared to the general population, indicating a clear excess of mortality.

Summary: This study investigated the tolerability and immune tolerance acquisition rate in infants with cow's milk allergy (CMA) who were treated with amino acid-based formula (AAF) and then switched to EHCF + LGG, a formula containing the probiotic Lacticaseibacillus rhamnosus GG. The results showed that after 12 months of treatment, the immune tolerance acquisition rate to cow milk proteins was higher in the EHCF + LGG group compared to the AAF group.

Omics-based methods, such as metagenomics and metabolomics, are commonly used for studying the gut microbiome due to their ability to provide high-throughput and high-resolution data. However, there are challenges in using machine learning-based approaches to analyze the relationship between microbiota and disease, such as small sample sizes and inconsistent experimental protocols. Efforts are being made to address these challenges, including the construction of data repositories and improved data transparency guidelines.

Summary: Little is known about the changes in organs and tissues that may make elder patients more vulnerable to acute stressors such as SARS-CoV-2 infection. In this study of elderly patients with SARS-CoV-2 infection, the association between vascular calcifications, bone density, muscle density, and the odds of dying was evaluated. Age was found to be the strongest predictor of body composition and death, overshadowing the relationships between these factors.

Bilirubin-Induced Transcriptomic Imprinting in Neonatal Hyperbilirubinemia

Summary: Severe neonatal hyperbilirubinemia can lead to brain damage and cause motor, cognitive, and auditory abnormalities. Bilirubin has been found to control the genetic development of the cerebellum, which is susceptible to bilirubin-induced damage. This study expands the research on the impact of bilirubin on postnatal brain development in regions that correlate with human symptoms. The findings show temporary alterations in gene expression in areas related to memory, learning, cognition, and auditory functions, but permanent changes in areas controlling movements, which coincide with clinical descriptions of bilirubin-induced neurotoxicity.

Low haemoglobin level predicts early hospital readmission in patients with cirrhosis and acute decompensation

Summary: This study aimed to characterize unscheduled readmissions and identify predictors of 30-day readmission in patients with decompensated cirrhosis. The results showed that early liver-related readmission was associated with higher 1-year mortality, and the model for end-stage liver disease-sodium score and low hemoglobin level at discharge were identified as independent risk factors for early readmissions.

Summary: A logistic regression model was developed and internally validated to predict the probability of 1-year readmission to the emergency department in patients with acute alcohol intoxication. The simplified model based on sex, age, previous admissions for trauma or mental/behavioral disease, and homelessness performed as well as the benchmark model. Further external validation is needed to assess the clinical utility of this model.

A clinical review of congenital hepatic fibrosis diagnosed in adulthood: presentation, complications, and outcomes

Summary: Congenital hepatic fibrosis (CHF) is a rare condition characterized by biliary tract changes and liver fibrosis. This study reviewed the medical records of 24 adult patients with CHF and found that renal cysts, splenomegaly, and a cirrhotic-appearing liver were the most common imaging findings. The most common liver-related complications were cholangitis, varices, and hepatic encephalopathy. Liver transplantation may be required for patients with decompensated disease.
