Aerosolization Performance of Jet Nebulizers and Biopharmaceutical Aspects

In this work, 13 jet nebulizers, some of which in different configurations, were investigated in order to identify the biopharmaceutical constraints related to the quality attributes of the medicinal products, which affect their safety, efficiency, compliance, and effectiveness. The aerosolization parameters, including the aerosol output, aerosol output rate, mass median aerodynamic diameter, and fine particle fraction, were determined according to the European Standard EN 13544-1, using sodium fluoride as a reference formulation. A comparison between the aerosol output nebulization time and the fine particle fraction displayed a correlation between the aerosol quality and the nebulization rate. Indeed, the quality of the nebulization significantly increased when the rate of aerosol emission was reduced. Moreover, the performance of the nebulizers was analyzed in terms of respirable delivered dose and respirable dose delivery rate, which characterize nebulization as the rate and amount of respirable product that could be deposited into the lungs. Depending on which of these two latter parameters was used, the nebulizers showed different performances. The differences, in terms of the rate and amount of delivered aerosol, could provide relevant information for the appropriate choice of nebulizer as a function of drug product, therapy, and patient characteristics.

Summary: This study developed omega-3 fatty acid nanoemulsions loaded with CUR and QU for the treatment of brain diseases. The formulations prepared by two different methods exhibited good stability and similar incorporation of CUR and QU. High-pressure homogenization (HPH) was found to be advantageous, and the obtained CQ NE showed potential for sustained release. Toxicity studies showed that the formulations were not toxic for Caenorhabditis elegans. The developed omega-3 fatty acid nanoemulsions have the potential to enhance the nose-to-brain permeation of CUR and QU for improved treatment efficacy.

Summary: This study developed a dry powder for inhalation, which carries calcium phosphate nanoparticles in mannitol microparticles for direct delivery of loaded bioactive peptide to the heart. By discussing the quality attributes of inhalable microparticles embedding nanoparticles and studying the release of peptide drug from microparticles, the dissolution of microparticles and degradation of nanoparticles were correlated.

Summary: The objective of this study is to develop and optimize a novel nasal film of Donepezil hydrochloride (DH) for the treatment of Alzheimer's disease. Factors affecting DH permeation and film flexibility were determined through experimental design, and the optimal combination was identified. The prepared nasal films exhibited good quality and stability.

No-shaking and shake-fire delays affect respirable dose for suspension but not solution pMDIs

Summary: This study investigates the effects of unshaken, shaken, and delayed use of suspension pressurized metered-dose inhalers (pMDIs) on Fine Particle Mass (<5 μm), Extra Fine Particle Mass (<2 μm), and MMAD. Most suspension pMDIs produce variable amounts of respirable size drug when subjected to shake-fire delays, with FPM ranging from -82% for Symbicort to 363% for Ventolin compared to the control values if not shaken. In terms of MMAD measurements, Seretide and Serzyl inhalers experience an increase from around 3 μm to 4.2-5.1 μm when not shaken. On the other hand, FPM and MMAD for solution aerosols remain unchanged regardless of being shaken or the delay in actuation after shaking.

The impact of possible improper use on the performance in vitro of NEXThaler in comparison with Ellipta inhaler

Summary: The correct use of dry powder inhalers is crucial for effective treatment and management of the disease. This study evaluated the impact of inhaler misuse on dose delivery and aerodynamic parameters. Two different inhaler devices, NEXThaler and Ellipta, were investigated. NEXThaler consistently delivered the correct dose, while Ellipta had reduced dose delivery when operated facing downward. NEXThaler's dose protector prevented powder loss during incorrect manipulations. Both devices proved reliable during double activation. Exhalation conditions had a slight effect on the aerodynamic profile of the inhalers.

Nebulizers effectiveness on pulmonary delivery of alpha-1 antitrypsin

Summary: Two nebulizers, a jet and a mesh vibrating system, were compared in terms of performance and effectiveness for delivering alpha-1 antitrypsin (AAT) to the lungs. The mesh nebulizer demonstrated higher efficiency in drug delivery, with preserved protein activity and conformation. Nebulization of AAT is a suitable administration strategy for AATD patients, either as a support therapy or for preventive purposes.

The optimized powder exhibited faster dissolution, better respirability, and showed efficacy in reducing IL-6 and inhibiting SARS-CoV-2 replication, suggesting its potential as a therapeutic strategy for lung rejection prevention and COVID-19 treatment.

Inhalable Microparticles Embedding Biocompatible Magnetic Iron-Doped Hydroxyapatite Nanoparticles

Summary: There has been a growing interest in developing biocompatible inhalable nanoparticle formulations for treating and diagnosing lung disease. In this study, superparamagnetic iron-doped calcium phosphate nanoparticles (FeCaP NPs) were found to be safe for inhalation administration to human lung cells. D-mannitol spray-dried microparticles embedding FeCaP NPs were formulated to achieve optimal particle size distribution for successful inhalation and deposition. This work demonstrates a spray drying method to provide a dry powder platform for the lung delivery of safe FeCaP NPs for magnetically driven applications.
