Summary: Individuals with primary hyperparathyroidism have reduced cortical bone mineral density, but relatively preserved trabecular bone density. However, their fracture risk is still increased. Trabecular bone score may provide a more accurate assessment of bone quality and fracture risk. This study compared the effectiveness of bone mineral density and trabecular bone score in describing bone quality in patients with primary hyperparathyroidism, and also evaluated changes in trabecular bone score following parathyroidectomy.

Summary: In this meta-analysis, the effectiveness of anti-resorptive therapies (bisphosphonates and denosumab) in preventing/treating bone loss and hypercalcemia in patients with primary hyperparathyroidism (PHPT) was investigated. The results showed that anti-resorptives significantly increased bone mineral density (BMD) at the lumbar spine and femoral neck, increased serum parathyroid hormone (PTH), and decreased serum calcium levels. Bisphosphonates only increased BMD at the lumbar spine, while denosumab increased BMD at both the lumbar spine and femur neck. Alendronate significantly improved bone loss and hypercalcemia in PHPT patients, without altering PTH and calcium levels.

Summary: Osteoporosis is a global public health problem with high prevalence rates among postmenopausal women, premenopausal women, and men. Identifying secondary causes of osteoporosis is crucial as treatment often involves addressing the underlying condition. Screening is recommended for all osteoporosis patients, with advanced investigations necessary for certain groups. The response to conventional therapy may be insufficient if the underlying condition is not recognized and treated. Bone densitometry can underestimate or overestimate fracture risk in certain chronic diseases. Tools such as FRAX and trabecular bone score provide additional information on fracture risk in secondary osteoporosis.

The duration of disease in the MHPT group was longer, but the age at hospital visit was significantly lower. Prophylactic antibiotic treatment for Pneumocystis jirovecii pneumonia or other bacterial infections appears unnecessary, while screening and prophylactic therapy for hepatitis B and latent tuberculosis are critical, especially in prevalent areas.

Summary: This study analyzed the incidence of urothelial carcinoma (UC) in Taiwan from 2001 to 2018 using data from the Taiwanese National Health Insurance Research Database and Taiwanese National Cancer Registry. The findings showed that the overall incidence of UC decreased after the ban on Chinese herbal preparations containing aristolochic acid (AA), but the incidence of upper urinary tract UC significantly increased. It is suggested to include herbal preparations in the adverse drug reaction or poison surveillance system and raise public awareness of the potential toxicity of phytotherapy.
