Summary: Children with CKD need safe and effective medicines, but conducting trials for pediatric drug development remains a challenge. There are difficulties in recruitment and completion, as well as a significant time lag between adult approval and pediatric labeling. This article provides an overview of regulatory frameworks, current landscape, challenges, and progress in drug development for children with CKD.

Summary: This study identified modifiable, non-medical barriers reported by nephrologists when providing renal replacement therapy modalities and comprehensive conservative management. The barriers were categorized into three levels: patient, healthcare professional, and healthcare system. These barriers could be targeted through education and optimizing financing structure to improve access to appropriate treatments.

Summary: Sex and gender are often used interchangeably, but gender is a more fluid and dynamic concept that encompasses psychosocial and cultural aspects. Inequality in medicine, particularly gender inequality, has been overlooked for a long time and is now a pressing concern. This study investigates gender equality in patients with chronic kidney disease (CKD) and highlights disparities in access to different treatment modalities.

Summary: This study investigated factors associated with the selection of dialysis modalities in elderly patients compared to younger patients. The results showed that older patients were more likely to choose hemodialysis (HD) over peritoneal dialysis (PD), and factors such as dialysis planning and congestive heart failure (CHF) played a significant role in the modality selection for elderly patients. Tumor, on the other hand, influenced the modality selection for younger patients. The study also found that socioeconomic status had a lesser impact on modality selection in elderly patients compared to younger patients.

Summary: The DAPA-CKD trial demonstrated that dapagliflozin has significant benefits in patients with chronic kidney disease and albuminuria. Using a Markov model, it was predicted that dapagliflozin treatment would lead to longer time in lower CKD stages, reduced mortality, and fewer complications such as heart failure and kidney function decline.

Summary: ICI therapy has substantially improved prognosis of patients with advanced malignancy, with similar incidence of immune-related adverse events in ESKD patients compared with the general population. Severe reactions have been seen in some cases, but cancer remission was observed in close to 30% of patients. More data from prospective studies are needed to assess outcomes more accurately.

Summary: The study revealed that hemodialysis (HD) patients are less active during dialysis days compared to non-dialysis days, and sedentary behavior increases the risk of death. Additionally, findings showed that body composition, physical fitness, and disease burden in HD patients are related to objectively measured physical activity.

Summary: For older patients with kidney failure, symptom burden worsened considerably before and stabilized after dialysis initiation. Fatigue, decreased interest in sex, and difficulty becoming sexually aroused were considered most burdensome, of which only fatigue somewhat improved after dialysis initiation.

Summary: This study compared the content, validity, and reliability of PROMIS CATs and SF-12 in patients with chronic kidney disease. The results showed that PROMIS CATs had sufficient construct validity and test-retest reliability, and had a lower minimal detectable change compared to SF-12. The study suggests the need for further research on the feasibility of PROMIS CATs in nephrology care.

Summary: This study investigated the association between residual estimated glomerular filtration rate (eGFR), dialysis adequacy, or serum phosphate level and chronic kidney disease-associated pruritus (CKD-aP) in incident dialysis patients. The results showed that higher residual eGFR and lower serum phosphate level, but not the dialysis dose, were related to a lower burden of CKD-aP in dialysis patients.

Summary: Cystatin C is recommended as a confirmatory test to eGFR when more precise estimates are needed for clinical decision making. Summary: Mortality prediction is crucial in long-term kidney replacement therapy (KRT) for treatment decisions and resource planning. Existing models have mostly been internally validated, but this study externally validated the performance of two previously constructed models in the Dutch and UK KRT populations. The models showed good performance and encourage widespread implementation in European KRT populations.

Summary: This study aims to identify and modify unhelpful illness perceptions in patients with chronic kidney disease (CKD) prior to kidney failure, and explore the needs and requirements for identifying and supporting these patients in nephrology care. It was found that illness perceptions related to the seriousness and manageability of CKD were the most meaningful. Implementing tools to identify and discuss patients' illness perceptions and offering support for patients with unhelpful illness perceptions are important.

Summary: Using the PROBAST, researchers explored the trends of risk of bias in prediction research and assessed the inter-rater agreement. They found that high bias was prevalent in the Analysis domain and the overall bias trends remained relatively stable over time. Summary: The purpose of this article is to investigate the impact of symptom experience on health related quality of life (HRQOL) in kidney transplant recipients (KTRs) and whether illness perceptions mediated this impact. The findings suggest that worse symptom experiences could cause more unhelpful illness perceptions and consequently lead to lower HRQOL. The study highlights the importance of addressing symptom experiences and illness perceptions to improve HRQOL in kidney transplant recipients.
