Common features between neoplastic and preneoplastic lesions of the biliary tract and the pancreas

the bile duct system and pancreas show many similarities due to their anatomical proximity and common embryological origin. Consequently, preneoplastic and neoplastic lesions of the bile duct and pancreas share analogies in terms of molecular, histological and pathophysiological features. Intraepithelial neoplasms are reported in biliary tract, as biliary intraepithelial neoplasm (BilIN), and in pancreas, as pancreatic intraepithelial neoplasm (PanIN). Both can evolve to invasive carcinomas, respectively cholangiocarcinoma (CCA) and pancreatic ductal adenocarcinoma (PDAC). Intraductal papillary neoplasms arise in biliary tract and pancreas. Intraductal papillary neoplasm of the biliary tract (IPNB) share common histologic and phenotypic features such as pancreatobiliary, gastric, intestinal and oncocytic types, and biological behavior with the pancreatic counterpart, the intraductal papillary mucinous neoplasm of the pancreas (IPMN). All these neoplastic lesions exhibit similar immunohistochemical phenotypes, suggesting a common carcinogenic process. Indeed, CCA and PDAC display similar clinic-pathological features as growth pattern, poor response to conventional chemotherapy and radiotherapy and, as a consequence, an unfavorable prognosis. The objective of this review is to discuss similarities and differences between the neoplastic lesions of the pancreas and biliary tract with potential implications on a common origin from similar stem/progenitor cells.

Real-world costs and dynamics of surveillance in patients who underwent surgery for low-risk branch duct intraductal papillary mucinous neoplasms

Summary: This study evaluated the rate of correct indication for pancreatectomy in low-risk BD-IPMN patients who underwent surveillance. Only 23.8% had a high grade dysplasia, while 59.7% had a low grade dysplasia and 14.9% had invasive cancer among those who were operated. The mean surveillance cost was 194.9 euros per patient-year, with higher costs in the correct surgery group compared to the overtreatment and undertreatment groups. The rate of appropriate surgery in BD-IPMNs under surveillance is low.

Summary: Primary sclerosing cholangitis (PSC) is characterized by increased ductular reaction, liver fibrosis, hepatic total bile acid levels, and mast cell infiltration. Inhibition of apical sodium bile acid transporter (ASBT) reduces PSC phenotypes. This study evaluates the role of ASBT in PSC and has clinical significance.

Summary: Pancreatic cancer is a deadly disease with few known risk factors and susceptibility loci. The rarity of the disease and lack of adequate research power have hindered progress in understanding its genetic mechanisms. However, the PANDoRA consortium, the largest in Europe, has made significant contributions in discovering susceptibility loci, which will be crucial in developing preventive strategies.

Summary: This study found that established risk factors for pancreatic cancer may not have a strong age-related effect on early-onset pancreatic cancer (EOPC) patients. Furthermore, smoking and diabetes play important roles in EOPC.

Summary: This study compared primary endoscopic ultrasound (EUS)-guided biliary drainage with endoscopic retrograde cholangiopancreatography (ERCP) for treating unresectable malignant distal biliary obstruction (MDBO). The results showed no significant difference in 1-year stent patency rates between the two procedures, but EUS-guided biliary drainage had higher technical success and shorter procedural time.

Summary: Secretin and secretin receptor play key roles in modulating liver phenotypes. Long-term treatment with SCT 5-27 can decrease ductular reaction and liver fibrosis in PSC mice. The increased expression of miR-125b and FoxA2 induced by SCT 5-27 treatment can reverse the downregulation of FoxA2 and reduce biliary damage in PSC.

Summary: The study aimed to identify non-invasive circulating biomarkers in medullary thyroid carcinoma (MTC). By analyzing tissue and plasma extracellular vesicle samples from MTC patients, a set of circulating miRNAs, including miR-26b-5p and miR-451a, were identified as diagnostic biomarkers. These miRNAs showed high expression levels and their expression decreased in disease-free patients during follow-up. The results provide a novel non-invasive tool for the molecular diagnosis of MTC.

Summary: The study investigated the feasibility of detecting somatic mutations in endoscopic ultrasound-guided, fine-needle aspiration (EUS-FNA) specimens of well-differentiated pancreatic neuroendocrine tumors (PanNETs). The results showed that next-generation sequencing (NGS) can detect somatic mutations in EUS-FNA specimens and there was mutational concordance between EUS-FNA specimens and primary tumors.
