Summary: This study examined acute ischemic stroke patients who received endovascular thrombectomy (EVT) in the late-window period and found that patients with both clinical-diffusion mismatch (CDM) and perfusion-diffusion mismatch (PDM) had a better response to EVT. Early neurological improvement (ENI) was independently associated with onset-to-door time, complete recanalization, initial NIHSS score, and the presence of CDM.

A SARS-CoV-2 outbreak associated with vaccine breakthrough in an acute care hospital

Summary: This study analyzed an outbreak caused by a vaccine breakthrough infection in a hospital with an active infection control program. The results showed that during the outbreak, 74.5% of the infections were vaccine breakthrough infections. Most transmissions appeared to be caused by three index cases. Preventive measures, including adequate ventilation, are crucial to minimize transmission in hospitals.

Late-Onset Granulomatous Pneumocystis jirovecii Pneumonia in A Renal Transplant Recipient: A Clinical Grand Round Conference Case in 2022

Summary: Late-onset Pneumocystis jirovecii pneumonia (PCP) can occur in solid organ transplant (SOT) patients and granulomatous P. jirovecii pneumonia (GPCP) is rare in this patient population. GPCP diagnosis is challenging due to low sensitivity of conventional tests and atypical patterns. This article presents a case of a 60-year-old male who developed nodular and patchy lung lesions after renal transplantation 24 years ago. Empirical treatment partially resolved the condition but relapsed after 4 months. GPCP was confirmed after surgical resection of the pulmonary nodule.
