Human Papillomavirus Genotyping Compared With a Qualitative High-Risk Human Papillomavirus Test After Treatment of High-Grade Cervical Intraepithelial Neoplasia A Systematic Review

OBJECTIVE: To systematically examine human papillomavirus (HPV) genotyping compared with qualitative high-risk HPV result during follow-up after treatment of high-grade cervical intraepithelial neoplasia (CIN), for risk estimation of posttreatment high-grade CIN. DATA SOURCES: MEDLINE, Cochrane, and ClinicalTrials.gov were searched from January 2000 to April 2019 for prospective studies of women and retrospective studies of residual specimens from women, tested using HPV assays with genotype reporting. METHODS OF STUDY SELECTION: The primary outcome was posttreatment high-grade CIN after treatment of high-grade CIN. Risk of bias (individual study quality) was evaluated with a modified Newcastle-Ottawa Scale. Overall quality of evidence for the risk estimate outcomes was evaluated using modified GRADE methodology for observational diagnostic studies. TABULATION, INTEGRATION, AND RESULTS: Of the 233 identified abstracts, 33 full-text articles were retrieved, and seven studies were included in the synthesis. The risk of bias was deemed to be low. Either a positive qualitative HPV test result or a positive test result for the same genotype that was present pretreatment have a sensitivity for predicting posttreatment high-grade CIN that approaches 100%. However, the positive predictive value (PPV) for the same genotype result pretreatment and posttreatment (median 44.4%) is about double the PPV (median 22.2%) for qualitative HPV results. The PPV of a new HPV infection posttreatment approximates zero. Human papillomavirus genotyping discriminated risk of posttreatment high-grade CIN to a clinically significant degree for women after treatment procedures for high-grade CIN lesions, when same-genotype persistence was compared with new genotype infection. CONCLUSION: There is moderately high-quality evidence to support the improved clinical utility of HPV genotyping compared with qualitative HPV positivity to follow-up after treatment of high-grade CIN.

Summary: This study evaluated the performance of six HPV DNA tests and one mRNA test in detecting cervical intraepithelial lesions (CIN) and as a test-of-cure in follow-up after surgical conservative treatment. The results showed that these tests had good concordance with HC2 and a very good and comparable sensitivity in CIN2+ detection.

Summary: HPV genotyping and viral load may serve as potential diagnostic biomarkers for high-grade cervical lesions. This study evaluated a new HPV test and found that the distribution of HR-HPV genotypes and viral copy numbers differed between women with high- and low-grade cervical lesions. These findings suggest that HPV genotyping and normalized viral load may help identify women at increased risk of cervical lesions.

Which Risk Factors and Colposcopic Patterns Are Predictive for High-Grade VAIN? A Retrospective Analysis

Summary: The aim of this study was to investigate the correlation between clinical and colposcopic features and the development of high-grade Vaginal Intraepithelial Neoplasia (VAIN). A retrospective analysis of 255 women diagnosed with VAIN revealed that smoking and a history of hysterectomy for CIN2+ were associated with VAIN development. Colposcopic grade G2, papillary lesion, and vascularity were significantly associated with a greater risk of VAIN3. Accurate diagnosis based on risk factors, colposcopic patterns, and histologic grade is crucial for personalized management of VAIN.

Tips and Tricks for Early Diagnosis of Cervico-Vaginal Involvement from Extramammary Paget's Disease of the Vulva: A Referral Center Experience

Summary: In this retrospective analysis, the incidence, clinical and pathological features of cervico-vaginal involvement in extra-mammary Paget's disease (EMPD) of the vulva were investigated. It was found that 2.5% of patients developed cervico-vaginal involvement at 5 years, 6.5% at 10 years, and 14.0% at 15 years. Abnormal glandular cytology was the primary method of detection, and most patients had invasive EMPD and required hysterectomy with colpectomy. Liquid-based cytology with immunocytochemistry was recommended for early diagnosis and lifelong follow-up.

HPV-based Cervical Cancer Screening on Self-samples in the Netherlands: Challenges to Reach Women and Test Performance Questions

Summary: In 2017, cervical cancer screening in the Netherlands was switched to HPV testing using cobas 4800. Women were provided with a self-sampling kit for free and could collect samples at home. Initially, only 7% of tests were done through self-sampling, but with improved promotion and accessibility, the proportion increased to 16% in 2020. Self-sampling showed lower HPV positivity and detection rate of precancer compared to clinician sampling, suggesting a need for optimization in handling and testing protocols.

Assessment of the clinical and analytical performance of the Aptima SARS-CoV-2 assay using the VALCOR protocol

Summary: The study protocol VALCOR was conducted to evaluate the performance of Aptima SARS-CoV-2 assay using clinical samples. The results showed that Aptima had excellent performance in sensitivity, specificity, and overall agreement, with the ability to detect clinical samples at a low limit of detection. This highlights the importance of evaluating diagnostic assays for reliable test outcomes.

The questionnaire revealed a high acceptability of self-collection among women, although there was a preference for standard collection devices.
