Primary Sjogren's syndrome (pSS) is a complex autoimmune disease with distinct clinical phenotypes and variable outcomes. The systemic form of the disease is characterized by immune complex mediated manifestations and is complicated by lymphoma as a result of a polyclonal B cell hyperactivity that is evolving into B cell malignancy. In the past decades, well-established clinical and serological markers have been described in the literature to identify high-risk patients and to predict lymphoma development. However, specific biologic treatments have proven ineffective to control the disease. Significant research effort has been made to reveal the major underlying biological events in this subgroup and identify biomarkers for early diagnosis, prognosis and response to treatment. In this review, we summarize the current data for the proposed histological, molecular and genetic biomarkers. (C) 2016 Elsevier B.V. All rights reserved.

Summary: Evidence suggests that low complement levels may be an independent predictor of treatment resistance in patients with pauci-immune glomerulonephritis, particularly in cases with higher histopathological activity at diagnosis. Treatment resistance is associated with factors such as low sC3 levels, high serum creatinine, acute dialysis requirement, and high chronicity score.

Summary: This article presents the HarmonicSS cloud computing exemplar platform, which provides state-of-the-art data analytics services to address the clinical unmet needs of primary Sjogren's Syndrome (pSS). The platform successfully develops lymphoma classification models and identifies biomarkers for lymphomagenesis by interlinking, curating, and harmonizing multiple regional and international databases. The federated AI algorithms trained on the harmonized databases yield robust lymphoma classification models with high accuracy and identify 5 biomarkers for lymphoma development.

APCA and patient age were independently associated with the presence of organ-specific autoimmunity. COVID-19: Clinical features and outcomes in unvaccinated 2-dose and 3-dose vaccinated against SARS-CoV-2 patients with systemic autoimmune and autoinflammatory rheumatic diseases

Summary: This observational study reports the clinical features and outcomes of COVID-19 in patients with systemic autoimmune and autoinflammatory rheumatic diseases (SAARD) who were unvaccinated or received 2 or 3 doses of the SARS-CoV-2 vaccine. The study found that vaccination with 2 or 3 doses significantly reduced hospitalizations and the need for oxygen supplementation in COVID-19 patients with SAARD. Furthermore, the study showed that receiving 3 doses of the vaccine was associated with a significantly lower risk of severe/critical COVID-19.

Liver Fibrosis in Primary Sjogren's Syndrome

Summary: This study explored the occurrence of liver fibrosis (LF) in primary Sjogren syndrome (pSS) patients in the context of non-alcoholic fatty liver disease (NAFLD). The results showed that pSS patients had a lower prevalence of liver steatosis and significant liver fibrosis compared to the comparators. Advanced LF was significantly associated with older age, higher LS, greater BMI, and disease status, with age being identified as an independent LF risk factor.

Summary: This study aimed to explore the impact of a past medical history of autoimmune disease on the clinical presentation and outcomes of patients with antineutrophil cytoplasmic autoantibodies associated vasculitis. The results showed that patients with a history of autoimmune disease had significantly fewer relapses after achieving remission.

Summary: A fully automatic method for scoring salivary glands in SGUS images was proposed in this study, where deep learning algorithms such as MobileNetV2, ResNet, and Dense-Net showed promising results. Further work should focus on evaluating on larger and international datasets to establish SGUS as an effective noninvasive pSS diagnostic tool.

Combined seronegativity in Sjogren's syndrome

Summary: This study described the clinical spectrum of Sjogren's syndrome patients with combined seronegativity, revealing that such patients have a milder disease phenotype. Triple seronegative patients showed lower frequency of peripheral nervous involvement compared to controls, while quadruple seronegative patients presented less frequently with persistent lymphadenopathy and lymphoma.

Searching for the X factor in Sjogren's syndrome female predilection

Summary: Sjogren's syndrome exhibits a strong female predilection, with the genetic imbalance of sex chromosomes, particularly the X chromosome, playing a potentially significant role. The imbalance can affect disease presentation in a gender-discordant manner and lead to differences in innate and adaptive immunity functions. Research on specific immune X-linked loci in Sjogren's syndrome may shed light on the explanation for women's predilection in autoimmunity.
