This comprehensive review of the 2018 dental literature is provided to inform busy dentists about progress in the profession. Developed by the Committee on Scientific Investigation of the American Academy of Restorative Dentistry, each author brings discipline-specific expertise to one of the 8 sections of the report including (1) prosthodontics; (2) periodontics, alveolar bone, and peri-implant tissues; (3) implant dentistry; (4) dental materials and therapeutics; (5) occlusion and temporomandibular disorders; (6) sleep-related breathing disorders; (7) oral medicine and oral and maxillofacial surgery; and (8) dental caries and cariology. The report targets important information that will likely influence day-to-day treatment decisions. Each review is not intended to stand alone but to update interested readers so that they may visit source materials if greater detail is desired. As the profession continues its march toward evidence-based clinical decision-making, an already voluminous library of potentially valuable dental literature continues to grow. It is the intention of this review and its authors to provide assistance in navigating the extensive dental literature published in 2018. It is our hope that readers find this work useful in the clinical management of patients moving forward.

Is the oral cavity a reservoir for prolonged SARS-CoV-2 shedding?

Matthias Troeltzsch, Ronald Berndt, Markus Troeltzsch

Summary: Limited understanding of the contagiosity and case fatality rate of COVID-19 has resulted in strict limitations on non-emergency healthcare, especially in head and neck medicine and dentistry. The oral cavity may provide a favorable environment for SARS-CoV-2 entry and persistence, but intraoral innate immune mechanisms could provide antiviral effects. More research is needed to determine the significance of intraoral tissues in SARS-CoV-2 transmission and persistence, as well as to investigate the true source of contagiosity in asymptomatic COVID-19 patients.

Fatty Acid Receptor CD36 Functions as a Surrogate Parameter for Lymph Node Metastasis in Oral Squamous Cell Carcinoma

Selgai Haidari, Matthias Troeltzsch, Thomas Knoesel, Paris Liokatis, Anastasia Kasintsova, Marian Eberl, Florian Ortner, Sven Otto, Florian Fegg, Marko Boskov, Florian A. Probst

Summary: This study found that CD36 is expressed in oral squamous cell carcinoma and correlates with tumor grading, T-status, and especially the occurrence of lymph node metastasis. CD36 may be useful for risk stratification regarding lymph node metastasis in OSCC.

Management of medication-related osteonecrosis of the jaw-a review of recent study results in comparison to established strategies

Matthias Troeltzsch, Markus Troeltzsch, Christoph Pautke, Sven Otto

Summary: Antiresorptive agents are commonly used drugs in osteology and oncology. A potentially severe side effect is medication-related osteonecrosis of the jaw (MRONJ). This review provides an overview of the latest developments in the etiology, diagnosis, and treatment of MRONJ.

Desmoglein-3 overexpression in oral squamous cell carcinoma is associated with metastasis formation and early recurrence: An immunohistochemical study

Matthias Troeltzsch, Verena Kunzel, Selgai Haidari, Markus Troeltzsch, Sven Otto, Michael Ehrenfeld, Florian Probst, Thomas Knoesel

Summary: This study aimed to investigate the expression patterns of specific desmosomal cadherins (DSG1/2/3) in oral squamous cell carcinoma (OSCC) and their associations with clinicopathological parameters and recurrence rates. The results showed that overexpression of DSG-3 was significantly associated with poor histologic differentiation, cervical node metastasis at primary diagnosis, and early recurrence. Determination of DSG-3 expression in OSCC specimens may be valuable for treatment planning and post-therapeutic risk assessment.

Association between obstructive sleep apnea and periodontitis in Chinese male adults: A cross-sectional study

Yanlong Chen, James E. Metz, He Gao, Xuemei Gao

Summary: This cross-sectional study investigated the association between obstructive sleep apnea and periodontitis in Chinese male adults. The results showed a positive correlation between obstructive sleep apnea and periodontitis, with 32.5% of patients diagnosed with periodontitis also having obstructive sleep apnea. The lowest oxygen saturation was significantly associated with the prevalence of periodontitis.

Digital planning and individual implants for secondary reconstruction of midfacial deformities: A pilot study

Paris Liokatis, Yoana Malenova, Florian-Nepomuk Fegg, Selgai Haidari, Monika Probst, Marko Boskov, Carl-Peter Cornelius, Matthias Troeltzsch, Florian-Andreas Probst

Summary: This study evaluated the feasibility and accuracy of using three-dimensional virtual surgical planning (VSP) and additive manufactured tools in the secondary reconstruction of midface deformities. The results showed that there were deviations between the virtual plans and postoperative positions, and the use of patient-specific implants (PSI) can reduce the displacement of bone segments.

Cancer testis antigen (PRAME) as an independent marker for survival in oral squamous cell carcinoma (OSCC)

Selgai Haidari, Matthias Troeltzsch, Riham Fliefel, Achim A. Jungbluth, Sven Otto, Florian Fegg, Paris Liokatis, Nima Ahmadi, Marian Eberl, Florian Andreas Probst, Thomas Knoesel

Summary: This study assessed the expression patterns of PRAME, NY-ESO1, and SSX2 in oral squamous cell carcinoma (OSSC) and found that PRAME expression was associated with poorer progression-free survival. Additionally, the expression of SSX2 and NY-ESO-1 was rare in the samples. Therefore, PRAME may serve as a suitable marker for oral squamous cell carcinoma.

Primary cold atmospheric plasma combined with low dose cisplatin as a possible adjuvant combination therapy for HNSCC cells-an in-vitro study

Teresa F. Brunner, Florian A. Probst, Matthias Troeltzsch, Sabina Schwenk-Zieger, Julia L. Zimmermann, Gregor Morfill, Sven Becker, Ulrich Harreus, Christian Welz

Summary: The aim of this study was to investigate the cytostatic effects of cold atmospheric plasma (CAP) on different HNSCC cell lines and its combination treatment with low dose cisplatin. The results showed that 120 s of CAP exposure significantly reduced cell viability, while 60 s of exposure increased DNA damage. Concurrent treatment with CAP and low dose cisplatin exhibited additive effects, and CAP could restore the sensitivity of Cal 27 cells to cisplatin.

Chronic Periodontal Infection and Not Iatrogenic Interference Is the Trigger of Medication-Related Osteonecrosis of the Jaw: Insights from a Large Animal Study (PerioBRONJ Pig Model)

Matthias Troeltzsch, Stephan Zeiter, Daniel Arens, Dirk Nehrbass, Florian A. Probst, Paris Liokatis, Michael Ehrenfeld, Sven Otto

Summary: This study used a pig model to investigate the link between chronic oral infections (periodontitis) and medication-induced osteonecrosis of the jaw (MRONJ). The results showed that oral infections can trigger MRONJ without prior dental surgical interventions, thus ruling out the decisive role of oral mucosal trauma in MRONJ pathogenesis.
