Summary: This scoping review emphasizes the need for effective treatments for the high suicide rate observed among individuals with psychosis. Two neglected treatment targets for suicide in people with psychotic disorders, emotional distress and emotion dysregulation, are highlighted. The review summarizes the research on these constructs in psychosis and their relationship to suicide, and offers clinical recommendations based on the findings. Emotional distress and emotion dysregulation may hold promise as treatment targets for suicide in people with psychosis.

Summary: Cognitive function may be associated with a higher level of suicidality in patients with recent-onset schizophrenia. This study found that suicidal ideation was common among patients, and it was related to younger age and longer duration of untreated psychosis. Patients with suicidal ideation had higher scores in general psychopathology, depression, stress, and hopelessness, and lower scores in subjective well-being and resilience. They also performed better in cognitive tests.

Summary: The risk of suicide is high in individuals experiencing first-episode psychosis (FEP), but the factors associated with this risk are not well-understood. This study aimed to identify the sociodemographic and clinical factors that contribute to suicide attempts in FEP patients over a two-year period. The findings revealed that prior suicide attempts, low functionality, depression, and feelings of guilt were significantly associated with suicide attempts. These findings highlight the importance of targeted interventions, particularly during the prodromal stage, in identifying and treating FEP patients with a high suicide risk.

Summary: In this six-year prospective cohort study, researchers investigated the relationship between smoking behavior polygenic scores (PRS) and psychosis. They found that smoking initiation and age at regular smoking initiation were associated with various symptom dimensions of schizophrenia. However, these associations were only observed in unaffected siblings and healthy controls, not in patients. The findings suggest that other genetic risk factors may play a dominant role in patients.

Summary: This study aimed to establish the risk of suicide associated with incident psychotic depression (PD) compared to incident non-psychotic severe depression (NPD). The results showed that PD was associated with a twofold increased risk of suicide compared to NPD, with the highest relative risks for impact-related and suffocation-related suicides. This study revealed some characteristics of PD patients in terms of the severity of suicidal ideation and choice of self-harm methods.

Summary: This study analyzed the relationship between tobacco use and first-episode psychosis, age at onset of psychosis, and specific diagnosis of psychosis. The findings showed that patients with first-episode psychosis were more likely to use tobacco and smoke heavily compared to controls. Tobacco use was associated with an earlier age at psychosis onset and a diagnosis of schizophrenia, but these associations were no longer significant after controlling for cannabis use.

Results showed that participation in smoking interventions was associated with higher odds of abstinence in the medium-term, but not long-term follow-ups. Limited knowledge on the factors associated with staying quit in smokers experiencing severe mental illness was found.

Summary: This study used ecological momentary assessment to investigate the relationship between perceived burdensomeness (PB) and thwarted belonginess (TB) with suicidal ideation (SI) in individuals with psychotic disorders. The results showed that burdensomeness varied less within participants compared to belongingness, and participants with SI had higher burdensomeness ratings and lower belongingness ratings. Social context, psychotic symptoms, mood, and SI were all found to be related to these interpersonal suicide-related risk factors.
