Prospective Multicenter Study on the Prognostic and Predictive Impact of Tumor Budding in Stage II Colon Cancer: Results From the SACURA Trial

PURPOSEThe International Union Against Cancer highlighted tumor budding as a tumor-related prognostic factor. International assessment criteria for tumor budding were recently defined by the 2016 International Tumor Budding Consensus Conference (ITBCC2016). This study aimed to clarify the prognostic and predictive values of tumor budding in a randomized controlled trial evaluating the superiority of adjuvant chemotherapy with oral tegafur-uracil over surgery alone for stage II colon cancer (SACURA trial; ClinicalTrials.gov identifier: NCT00392899).PATIENTS AND METHODSBetween 2006 and 2010, we enrolled 991 patients from 123 institutions with stage II colon cancer. Tumor budding was diagnosed by central review on the basis of the criteria adopted in the ITBCC2016. We prospectively recorded all clinical and pathologic data, including the budding grade, and performed prognostic analyses after 5 years of completing the patients' registration.RESULTSOf 991 tumors, 376, 331, and 284 were classified as BD1, BD2, and BD3, respectively; the 5-year relapse-free survival (RFS) rate was 90.9%, 85.1%, and 74.4%, respectively (P < .001), and ranged widely in T4 tumors (86.6% to 53.3%). The budding grade significantly correlated with recurrence in the liver, lungs, lymph nodes, and peritoneum (P < .001 to .01). Multivariable analysis revealed that budding and T stage exerted an independent impact on RFS, and on the basis of the Harrell concordance index, these two factors substantially contributed to the improvement of the Cox model for predicting RFS. Both the BD2 and BD3 groups demonstrated greater improvement in the 5-year recurrence rate in the adjuvant chemotherapy group than the surgery-alone group by approximately 5%, but the difference was statistically nonsignificant.CONCLUSIONTumor budding grade on the basis of the ITBCC2016 criteria should be routinely evaluated in pathologic practice and could improve the benefit of adjuvant chemotherapy for stage II colon cancer.

Stress Echocardiography After Mitral Valve Repair Using a Semirigid Partial Band

Summary: The study evaluated hemodynamics after mitral valve repair using a partial semirigid band during exercise stress echocardiography and identified age, sex, and device size as significant determinants of achieving better exercise capacity after surgery. Patients with larger indexed device size were found to achieve greater exercise capacity, highlighting the importance of preoperative evaluation for selecting appropriate device size.

Summary: A peritoneal loose body (PLB) is a tissue completely separated from other intraperitoneal organs, typically found incidentally during surgery, examination, or autopsy. We report a case of a giant PLB located outside the peritoneal cavity, adjacent to the rectovesical excavation. Although it was fixed and preoperative diagnosis was uncertain, laparoscopic removal of the mass was feasible.

Effect of Polypharmacy on Clinical Outcomes in Elderly Patients With Non-Valvular Atrial Fibrillation

Summary: This study evaluated the impact of polypharmacy on 2-year outcomes in elderly patients with non-valvular atrial fibrillation. The results showed that patients receiving more medications were older, had poorer renal function, and had more comorbidities. Moreover, with increasing medications, the incidence rates of stroke, bleeding, and other adverse events significantly increased.

Summary: This study aims to explore whether the reasonable detection rate of endoscopically diagnosed lesions as adenomas can be calculated and used as a surrogate colonoscopy quality indicator. By analyzing a database generated from colonoscopy reports, it was found that the number of endoscopic adenomas was higher than pathological adenomas primarily due to non-resection during the examination. The overall polyp detection rate, endoscopic ADR, and pathological ADR were 56.4%, 48.0%, and 32.7% respectively.

Risk factors for gallstone disease onset in Japan: Findings from the Shizuoka Study, a population-based cohort study

Summary: A predictive model called Shizuoka Hip Fracture Prognostic Score (SHiPS) was developed to predict long-term mortality after hip fracture. The model used preoperative information such as sex, age, fracture site, nursing care certification, and comorbidities to classify mortality risk into four categories. The SHiPS showed good predictive performance for up to 5 years after fracture onset.
