Markers of neutrophil mediated inflammation associate with disturbed continuous electroencephalogram after out of hospital cardiac arrest

Pirkka T. Pekkarinen, Federico Carbone, Silvia Minetti, Davide Ramoni, Giuseppe Ristagno, Roberto Latini, Lauri Wihersaari, Kaj Blennow, Henrik Zetterberg, Jussi Toppila, Pekka Jakkula, Matti Reinikainen, Fabrizio Montecucco, Markus B. Skrifvars

Summary: This study found that elevated levels of OPN and MPO at ICU admission were associated with disturbances in cEEG in the subsequent 48 hours after cardiac arrest. However, these inflammatory markers had little predictive value for long-term prognosis compared to 48-hour NFL.

Role of Uric Acid in Vascular Remodeling: Cytoskeleton Changes and Migration in VSMCs

Elisa Russo, Maria Bertolotto, Valentina Zanetti, Daniela Picciotto, Pasquale Esposito, Federico Carbone, Fabrizio Montecucco, Roberto Pontremoli, Giacomo Garibotto, Francesca Viazzi, Daniela Verzola

Summary: This study demonstrates that uric acid (UA) can induce changes in VSMC cytoskeleton, leading to increased cell migration and activation of specific signaling pathways. These findings suggest that UA may play a critical role in vascular remodeling, emphasizing the importance of interventions to prevent the development of cardiovascular diseases.

Relapsing pericarditis: Peripheral blood neutrophilia, lymphopenia and high neutrophil-to-lymphocyte ratio herald acute attacks, high-grade inflammation, multiserosal involvement, and predict multiple recurrences

Enrico Tombetti, Francesca Casarin, Emanuele Bizzi, Sofia Bezer, Ruggiero Mascolo, Giada Pallini, Claudia Gabiati, Aldo Bonaventura, Lucia Trotta, Massimo Pancrazi, Silvia Maestroni, Antonio Brucato

Summary: Peripheral blood neutrophilia and lymphopenia are typical of acute idiopathic pericarditis. Acute attacks of pericarditis are associated with neutrophilia and lymphopenia, as compared with disease remission.

Vitamin D in atherosclerosis and cardiovascular events

Federico Carbone, Luca Liberale, Peter Libby, Fabrizio Montecucco

Summary: Experimental and clinical findings linking vitamin D to cardiovascular risk have prompted consideration of its supplementation for overall health improvement. However, several meta-analyses do not support its clinical effectiveness, and recent research has revealed complex non-classical effects of vitamin D in cardiovascular diseases.

Endovascular aortic repair impact on myocardial contractility: A prospective study

Giovanni Spinella, Gian Antonio Boschetti, Matteo Bauckneht, Stefano Raffa, Cecilia Marini, Alice Finotello, Bianca Pane, Giovanni Pratesi, Domenico Palombo, Gianmario Sambuceti

Summary: This study aimed to assess the impact of altered sphygmic wave transmission on left ventricular (LV) contractile function in patients undergoing endovascular aortic repair (EVAR). Data from preoperative and 6-month postoperative single photon emission computed tomography (SPECT) with arterial stiffness measurement showed significant changes in pressure wave curve and myocardial perfusion parameters.

Inflammatory biomarkers of ischemic stroke

Amedeo Tirandi, Cosimo Sgura, Federico Carbone, Fabrizio Montecucco, Luca Liberale

Summary: Ischemic stroke is the second leading cause of death globally and a major cause of morbidity. Current therapeutic options are limited to early reperfusion strategies, while pharmacological neuroprotective strategies have failed to enter clinical practice despite promising results in experiments. Inflammation plays a significant role in the pathophysiology of ischemic stroke, and inflammatory mediators have been investigated as potential prognostic markers and therapeutic targets. This article summarizes the available evidence on the role of cytokines, soluble adhesion molecules, and adipokines in the pathophysiology, prognosis, and therapy of ischemic stroke.

Early sclerostin assessment in frail elderly patients with sepsis: insights on short- and long-term mortality prediction

Amedeo Tirandi, Eleonora Arboscello, Stefano Ministrini, Luca Liberale, Aldo Bonaventura, Alessandra Vecchie, Maria Bertolotto, Daniele Roberto Giacobbe, Luca Castellani, Michele Mirabella, Silvia Minetti, Matteo Bassetti, Fabrizio Montecucco, Federico Carbone

Summary: In this pilot study, SOST levels were found to be associated with age, cardiovascular comorbidities, and early death in septic patients. SOST showed a high predictive value for 30-day mortality and outperformed validated scoring scales for critically ill patients. The variation in SOST levels also predicted long-term mortality.

Early Experience of Inner Branch Retrograde Cannulation With E-nside Branch Stent Graft for Thoracoabdominal Aortic Aneurysms

Giovanni Spinella, Bianca Pane, Alice Finotello, Martina Bastianon, Jorge Miguel Mena Vera, Sara Di Gregorio, Giovanni Pratesi

Summary: The feasibility of retrograde cannulation using devices with inner branches for endovascular treatment of thoracoabdominal aortic aneurysms (TAAAs) was investigated. A retrospective analysis was conducted on TAAAs patients undergoing endovascular treatment. The results showed that retrograde cannulation with inner branches was successfully performed in 93% of cases, with a total of 26 out of 28 target vessels cannulated. Two intra-procedural complications were observed, and one case of mortality was observed at 30 days follow-up.

Obesity phenotypes and cardiovascular risk: From pathophysiology to clinical management

Alberto Preda, Federico Carbone, Amedeo Tirandi, Fabrizio Montecucco, Luca Liberale

Summary: The obesity epidemic is a global health crisis, affecting over one billion people worldwide. Obesity has a significant impact on cardiovascular health, causing various structural, functional, humoral, and hemodynamic changes. Accurate assessment of cardiovascular risk in individuals with obesity is crucial for reducing mortality rates and maintaining quality of life. Recent evidence suggests the existence of different phenotypes of obesity, each associated with varying degrees of cardiovascular risk. Diagnosis should not solely rely on anthropometric parameters, but also include a thorough evaluation of metabolic status. The World Heart Federation and World Obesity Federation have proposed an action plan for managing obesity-related cardiovascular risk and mortality, emphasizing the importance of comprehensive multidisciplinary programs. This review aims to provide an updated summary of the different obesity phenotypes, their specific effects on cardiovascular risk, and differences in clinical management.
