Analysis of Biomechanical Differences Between Condylar Constrained Knee and Rotating Hinged Implants: A Numerical Study

Background: Different levels of constraint for total knee arthroplasty can be considered for revision surgeries. While prior studies have assessed the clinical impact and patient outcomes of condylar constrained knee (CCK) and rotating hinged (RTH) implants, nowadays little is known about the biomechanical effects induced by different levels of constraint on bone stress and implant micromotions. Methods: CCK and RTH implant models were analyzed using a previously validated numerical model. Each system was investigated during a squat and a lunge motor task. The force in the joint, the bone and implant stresses, and micromotions in this latter were analyzed and compared among designs. Results: Different activities induced similar bone stress distributions in both implants. The RTH implant induces mostly high stress compared to the CCK implant, especially in the region close to tip of the stem. However, in the proximal tibia, the stresses achieved with the CCK implant is higher than the one calculated for the RTH design, due to the presence of the post-cam system. Accordingly, the condylar constrained design shows higher implant micromotions due to the greater torsional constraint. Conclusion: Different levels of constraint in revision arthroplasty were always associated with different biomechanical outputs. RTH implants are characterized by higher tibial stress especially in the region close to the stem tip; condylar implants, instead, increase the proximal tibial stress and therefore implant micromotions, as a result of the presence of the post-cam mechanism. Surgeons will have to consider these findings to guarantee the best outcome for the patient and the related change in the bone stress and implant fixation induced by different levels of constrain in a total knee arthroplasty. Summary: This study evaluates the performance of two biomaterials (MED625FLX and TPU95A) in cryoablation treatment and finds that TPU95A (1.0 and 2.5 mm) is suitable for cryoablation treatment, while MED625FLX (1.0 mm) loses its shape in the test and MED625FLX (2.5 mm) is also suitable for use with cryoenergy source. The positive results from the thermal tests on these materials and their thickness prompt further clinical investigation.

Summary: In orthopedic medical devices, it is important to address the differences in elasto-plastic behavior between bone and metallic materials to prevent mechanical issues at the bone-implant interface. This study examined the elastic behavior of different in-body gap prismatic geometries and their effectiveness in reducing prosthetic stiffness. Experimental tests and finite element models were used to evaluate the impact of these geometries on stiffness reduction. Additionally, the study explored the use of composite materials to achieve variable elastic behavior that could match the mechanical properties of bone.

Biomechanical Analysis of Femoral Stem Features in Hinged Revision TKA with Valgus or Varus Deformity: A Comparative Finite Elements Study

Summary: This study used finite element analysis to investigate the effects of different interventions on bone stress, tibio-femoral contact pressure, and force when treating varus and valgus deformities with hinged total knee arthroplasty. The results showed that changes in the varus or valgus deformity degrees altered the distribution of medio-lateral stress and force, as well as the contact area. The interventions and cement had no effect on the tibio-femoral contact biomechanics, but mainly affected the distal femoral region and had negligible effect in the proximal regions.

The Effects of Different Femoral Component Materials on Bone and Implant Response in Total Knee Arthroplasty: A Finite Element Analysis

Summary: This study compared the effects of two same-design cruciate-retaining femoral components made with CoCr and titanium alloy on periprosthetic bone stresses using a finite element model of the implanted knee. The results showed that the titanium component induced higher bone stresses, especially at higher knee flexion angles, indicating that the CoCr component led to more stress shielding. This study has significant clinical relevance in reducing bone resorption after total knee arthroplasty.

Biomechanical analysis of different levels of constraint in TKA during daily activities

Summary: This study compared the effects of different levels of constraint in knee biomechanics by using finite element analysis. The results showed that while the variations of the prosthetic implant did not significantly affect the stress distribution in different regions of the tibial bone, they did significantly alter the stress distribution at the interface between the prosthetic components.

Covariations between scapular shape and bone density in B-glenoids: A statistical shape and density modeling-approach

Summary: B-type glenoids are associated with increased risk of glenoid component loosening after total shoulder arthroplasty. This study quantified variabilities in scapular morphology and bone mineral density in patients with B glenoids and identified patterns of covariation between these two features. The findings suggest an asymmetrical distribution of bone density and provide insights into surgical guidelines for balancing bony shape correction and bone support quality in B glenoids.

Finite element analysis of malposition in bi-unicompartmental knee arthroplasty

Summary: Unicompartmental knee arthroplasty is a less invasive treatment with advantages for patients but can be more challenging for surgeons due to limited visibility and potential errors in cutting angles. This study investigated the effects of slight angle variations in the anterior-posterior slope of the lateral tibial tray. Results showed that a slight posterior slope during surgery can reduce the chances of implant instability.
