Purpose: Health literacy has been attracting increasing attention because low health literacy is considered an important predictor of adverse health outcomes in many chronic conditions, including diabetes. However, it is unclear how health literacy is associated with health outcomes. The purpose of this study was to formulate a hypothetical structural equation model linking health literacy to self-efficacy, self-care activities, and health-related quality of life (HRQOL) in patients with type 2 diabetes. Methods: A cross-sectional survey design was employed, and 459 patients with type 2 diabetes were recruited from outpatient clinics in two university hospitals. The patients completed a pack of questionnaires. The hypothetical model was tested using structural equation modeling analysis. Results: The values of multiple fit indices indicated that the proposed model provided a good fit to the data. Health literacy exerted not only a direct effect on self-care activities, but also an indirect effect on self-care activities via self-efficacy. However, health literacy exerted only an indirect effect on HRQOL. This structural model was invariant across hemoglobin-A1c-controlled and hemoglobin-A1c-uncontrolled groups. Based on R-2 values, the final model accounted for 20.0% of the variance in self-efficacy, 61.0% of the variance in self-care activities, and 16.0% of the variance in HRQOL. Conclusions: This study suggests that self-care activities are crucial to the link between health literacy and HRQOL. Both health literacy and self-efficacy need to be considered in clinical practice for enhancing self-care activities in patients with type 2 diabetes. This approach may ultimately improve HRQOL in these patients. Copyright (C) 2016, Korean Society of Nursing Science. Published by Elsevier. Summary: This study evaluated the psychometric properties of the Diabetes Knowledge Test (DKT2). The results showed that the DKT2 had a poor unidimensional structure with low person reliability and separation index. Females and elderly individuals found certain items more difficult to respond to. Caution should be exercised when using the DKT2 in practice and research.

Relationship between Type D personality and adverse health outcomes in people with type 2 diabetes: The parallel mediating roles of diabetes distress and social isolation

Eun-Hyun Lee, Young Whee Lee, Duckhee Chae, Eun Hee Kang, Hyun-Jung Kang

Summary: The aim of this study was to investigate the direct and indirect relationship between Type D personality and adverse health outcomes (glycated haemoglobin and health-related quality of life) in people with type 2 diabetes, specifically through the mediating factors of diabetes distress and social isolation. A secondary analysis of data from 524 participants with type 2 diabetes was conducted using the PROCESS macro of SPSS. The results showed that 31.3% of participants had Type D personality, which had a direct effect on health-related quality of life but not on glycated haemoglobin. Type D personality indirectly affected both glycated haemoglobin and health-related quality of life through the mediators of diabetes distress and social isolation.

Internal Structure of the Health-Related Quality of Life Instrument with 8-Items in a Nationally Representative Population

Eun-Hyun Lee

Summary: This study evaluated the internal structure of the Health-Related Quality of Life Instrument with Eight Items (HINT-8) and found it to have satisfactory validity and consistency. However, the interpretation of HINT-8 scores varies across different sociodemographic groups, making comparisons difficult. On the other hand, the interpretation of HINT-8 remains consistent for individuals with or without certain medical conditions.

Summary: When assessing test-retest reliability, the stability of the measured construct over time should be considered. In order to evaluate responsiveness, a longitudinal study design with a treatment/intervention known to induce change on the construct being measured is necessary.

Summary: Before evaluating the validity of a measurement instrument, it is important to design an instrument that reflects hypothetical relationships or differences with other comparator instruments or groups. For criterion validity, a gold-standard instrument measuring the same construct should be carefully selected.

Summary: The passage discusses the psychometric properties of the internal structure of an instrument, including structural validity, internal consistency, and cross-cultural validity/measurement invariance. It highlights the methods used in Korean nursing journals to assess these properties, as well as recommending Korean nursing researchers to use a greater variety of methods to evaluate the internal structure of instruments.
