Robotic kidney transplantation in the obese patient: 10-year experience from a single center

Despite increasing obesity rates in the dialysis population, obese kidney transplant candidates are still denied transplantation by many centers. We performed a single-center retrospective analysis of a robotic-assisted kidney transplant (RAKT) cohort from January 2009 to December 2018. A total of 239 patients were included in this analysis. The median BMI was 41.4 kg/m(2), with the majority (53.1%) of patients being African American and 69.4% of organs sourced from living donors. The median surgery duration and warm ischemia times were 4.8 hours and 45 minutes respectively. Wound complications (mostly seromas and hematomas) occurred in 3.8% of patients, with 1 patient developing a surgical site infection (SSI). Seventeen (7.1%) graft failures, mostly due to acute rejection, were reported during follow-up. Patient survival was 98% and 95%, whereas graft survival was 98% and 93%, at 1 and 3 years respectively. Similar survival statistics were obtained from patients undergoing open transplant over the same time period from the UNOS database. In conclusion, RAKT can be safely performed in obese patients with minimal SSI risk, excellent graft function, and patient outcomes comparable to national data. RAKT could improve access to kidney transplantation in obese patients due to the low surgical complication rate.

Summary: This study reviewed the outcomes of laparoscopic, robotic-assisted living donor nephrectomy (RLDN) and found that it is a safe technique, providing a reasonable alternative to conventional laparoscopic surgery. It is especially suitable for donors with higher body mass index and multiple renal arteries, and also provides a platform for transplant surgeons to develop skills in robotic-assisted surgery for minimal invasive recipient operations.

Summary: This study found that kidney transplant recipients have slower VO2 kinetics compared to healthy controls, but improvements in hemoglobin and peak VO2 seem to occur during the first year after transplantation. VO2 kinetics were significantly associated with indices of cardiorespiratory fitness, suggesting a potential benefit in adding this index of muscular oxidative metabolism to functional evaluation in KTRs.

The introduction of minimally invasive surgical techniques in the 1990s significantly improved postoperative care and outcomes. The adoption of robotic surgical technology in the late 1990s opened new horizons for technically demanding surgeries such as transplantation to be completed in a minimally invasive fashion.

Summary: This retrospective cohort case-control study aimed to compare the effects of sugammadex and neostigmine in reversing neuromuscular block (NMB) in patients undergoing kidney transplantation. The results showed that the sugammadex group had better postoperative kidney function, lower incidence of severe hypoxemia, shorter post-anesthesia care unit stay, and reduced intensive care unit admissions compared to the neostigmine group.

The results showed that the transplanted kidneys generally increased in size and improved in function over time. However, interstitial fibrosis may lead to a long-term reduction in kidney volume.
