Summary: This study investigated the epidemiology and outcomes of inflammatory bowel disease-associated colorectal cancer (IBD-CRC). It found that IBD patients have a higher risk of developing CRC, are diagnosed at a younger age, and have worse outcomes compared to sporadic CRC. Further research is needed to understand the reasons behind these differences and inform screening, surveillance, and treatment strategies for this high-risk group.

Summary: This systematic review and meta-analysis focused on the incidence and risk factors for advanced neoplasia in inflammatory bowel disease (IBD) patients with indefinite for dysplasia (IND) and low-grade dysplasia (LGD). The pooled incidences of advanced neoplasia in IND and LGD patients were found to be similar, indicating the need for rigorous surveillance. Risk factors for advanced neoplasia included primary sclerosing cholangitis, aneuploidy, and other factors. Further research is needed to validate these findings.

Summary: The detection of dysplasia in IBD patients is important and chromoendoscopy with targeted biopsy remains the preferred technique. However, other techniques such as virtual chromoendoscopy are being compared due to their similar results and fewer technical difficulties. New endoscopy techniques and improved devices have the potential to revolutionize the screening and follow-up of dysplasia in IBD patients. Further research is needed to define the optimal follow-up strategy.

Summary: Texture and color enhancement imaging (TXI) has been proposed as a method to increase lesion detection during colonoscopy. In this randomized trial, TXI was found to significantly increase adenoma detection rate and adenomas per colonoscopy compared to standard high definition white-light imaging (WLI), particularly in polyps smaller than 10mm.

Summary: This study aimed to describe the main diagnostic patterns for ulcerative colitis-associated neoplasia (UCAN), including lateral extension surrounding flat lesions. Through dye chromoendoscopy (DCE) imaging analysis, it was found that lesions with small round or mesh structures were more likely to have high-grade dysplasia or carcinoma. Therefore, when these characteristic mucosal patterns are found in DCE, the possibility of UCAN should be considered.

Summary: The study compared the dysplasia detection rate of chromoendoscopy and high-definition white light endoscopy (HDWLE) in patients with chronic IBD-colitis, finding that both methods had a similar diagnostic yield for dysplasia detection. Various patient and disease characteristics were found to be associated with an increased risk of polypoid dysplasia.

Summary: The study aimed to clarify the morphology of initial ulcerative colitis-associated neoplasia (UCAN). Results showed that rectum lesions and inflammation are difficult to identify as neoplasia even within the 2 years before a UCAN diagnosis, suggesting the need for more careful surveillance.

Summary: Currently, the staging of colorectal cancer (CRC) relies on ex vivo pathologic analysis which is invasive and limits sample collection. In this study, key biomarkers VEGFR2 and PTGER4 were selected based on their expression patterns in different stages of CRC. Molecular probes were constructed, and the feasibility of in vivo noninvasive CRC staging using confocal laser endoscopy (CLE) was demonstrated in mouse models. This strategy shows promise in providing precise pathological staging for CRC patients and guiding therapeutic strategies.

Summary: High-definition (HD) endoscopy is recommended for detecting dysplasia in patients with ulcerative colitis (UC). Dye-spray chromoendoscopy (DCE) and narrow-band imaging (NBI) are common adjunctive techniques. This study evaluated the incremental detection ability of DCE and NBI added to HD-WLE in patients with long-standing extensive UC. HD-DCE improved the detection of SSAs/Ps, but not dysplasia.

Summary: This study compared the performance of HDV and HDWL in detecting neoplasia in patients with IBD undergoing surveillance colonoscopy, finding no significant difference between the two. The majority of neoplasia were detected on targeted biopsy or resection, with quadrantic non-targeted biopsies providing minimal additional benefit.

Summary: This retrospective observational study analyzed the cost of endoscopic resection and surgical resection for submucosal colorectal cancer. The study found that endoscopic resection had a lower total cumulative cost and comparable oncologic outcomes as surgical resection. Therefore, endoscopic resection can be considered a cost-effective option for initial treatment of submucosal colorectal cancer.

Summary: This study compared the efficacy, tolerability, and safety of mini-OST and conventional OST. The results showed that there were no significant differences in preparation efficacy, satisfaction, and tolerability between the two groups. The first bowel movement occurred earlier in the mini-OST group. The occurrence of adverse events was comparable between the two groups. In conclusion, mini-OST showed similar efficacy, tolerability, and safety compared to conventional OST.

Summary: This study aimed to evaluate the disease characteristics and outcomes of transition in patient care among adolescent patients with inflammatory bowel disease (IBD). Data from patients diagnosed with IBD (Crohn's disease, ulcerative colitis, or intestinal Behcet's disease) were collected and analyzed. The study found that the transition IBD group had more severe disease, but lower medical non-compliance, and clinical outcomes improved after transition.

Summary: This study aimed to evaluate the expression of multiple peptide hormones in rectal neuroendocrine tumors (NETs) and their clinical and prognostic implications. The results showed that several peptide hormones were expressed in rectal NETs, and tumors expressing serotonin or somatostatin were associated with lymphovascular invasion, chromogranin A expression, and shorter disease-free survival. The peptide hormonal expression status can classify rectal NETs into L-cell, enterochromaffin-cell, D-cell, null-expression, or mixed-expression type.

Summary: This study investigated the emergence rate of clonal haematopoiesis of indeterminate potential (CHIP) and its association with inflammatory markers in patients with Behcet's disease (BD). The results showed that the detection rate of CHIP in BD patients was 11.1%, which was not significantly different from the control group. Among the BD patients, DNMT3A mutations were the most common. CHIP carriers with BD had higher levels of inflammatory markers and older age compared to non-CHIP carriers with BD.

Summary: This study explored the use of probiotics and factors associated with their prescription in patients with inflammatory bowel disease (IBD). The results showed that increased disease activity may be associated with fewer prescriptions of probiotics. However, physicians prescribed probiotics to control symptoms in both UC and CD patients. The use of MTX and iron, as well as a history of IBD-related surgeries, were associated with more frequent probiotic prescriptions in CD patients.
