High-Definition Chromoendoscopy Versus High-Definition White Light Colonoscopy for Neoplasia Surveillance in Ulcerative Colitis: A Randomized Controlled Trial

OBJECTIVES: Although chromoendoscopy is currently the recommended mode of surveillance in patients with long-standing ulcerative colitis, it is technically challenging and requires a long procedure time. The aim of this study was to compare the dysplasia detection rate of high-definition white light endoscopy with random biopsy (HDWL-R) vs high-definition chromoendoscopy with targeted biopsy (HDCE-T). METHODS: This was a multicenter, prospective randomized controlled trial involving 9 tertiary teaching hospitals in South Korea. A total of 210 patients with long-standing ulcerative colitis were randomized to undergo either the HDWL-R group (n = 102) or HDCE-T group (n = 108). The detection rates of colitis-associated dysplasia (CAD) or all colorectal neoplasia from each trial arm were compared. RESULTS: There was no significant difference in the CAD detection rate between HDCE-T and HDWL-R groups (4/102, 3.9% vs 6/108, 5.6%, P = 0.749). However, HDCE-T showed a trend toward improved colorectal neoplasia detection compared with HDWL-R (21/102, 20.6% vs 13/108, 12.0%, P = 0.093). The median (range) time for colonoscopy withdrawal between the 2 groups was similar (17.6 [7.0-43.3] minutes vs 16.5 [6.3-38.1] minutes; P=0.212; for HDWL-R and HDCE-T, respectively). The total number of biopsies was significantly larger in the HDWL-R group (34 [12-72]) compared with the HDCE-T group (9 [1-20]; P < 0.001). DISCUSSION: On the basis of our prospective randomized controlled trial, HDCE-T was not superior to HDWL-R for detecting CADs.

Comparative Cost Analysis Between Endoscopic Resection and Surgery for Submucosal Colorectal Cancer

The study found that endoscopic resection had a lower total cumulative cost and comparable oncologic outcomes as surgical resection. Therefore, endoscopic resection can be considered a cost-effective option for initial treatment of submucosal colorectal cancer.

Summary: This study compared the efficacy, tolerability, and safety of mini-OST and conventional OST. The results showed that there were no significant differences in preparation efficacy, satisfaction, and tolerability between the two groups. The first bowel movement occurred earlier in the mini-OST group. The occurrence of adverse events was comparable between the two groups. In conclusion, mini-OST showed similar efficacy, tolerability, and safety compared to conventional OST.

Characteristics and Treatment Outcomes of Transition among Patients with Inflammatory Bowel Disease

Summary: This study aimed to evaluate the disease characteristics and outcomes of transition in patient care among adolescent patients with inflammatory bowel disease (IBD). Data from patients diagnosed with IBD (Crohn's disease, ulcerative colitis, or intestinal Behcet's disease) were collected and analyzed. The study found that the transition IBD group had more severe disease, but lower medical non-compliance, and clinical outcomes improved after transition.

Effectiveness and tolerability of methotrexate monotherapy in Crohn's disease patients: a multicenter observational study

Clinicopathologic Impact of Peptide Hormonal Expression in Rectal Neuroendocrine Tumors

Summary: This study aimed to evaluate the expression of multiple peptide hormones in rectal neuroendocrine tumors (NETs) and their clinical and prognostic implications. The results showed that several peptide hormones were expressed in rectal NETs, and tumors expressing serotonin or somatostatin were associated with lymphovascular invasion, chromogranin A expression, and shorter disease-free survival. The peptide hormonal expression status can classify rectal NETs into L-cell, enterochromaffin-cell, D-cell, null-expression, or mixed-expression type.

Validation of the simplified magnetic resonance index of activity by using DWI without gadolinium enhancement to evaluate bowel inflammation in Crohn's disease

Summary: This study aimed to validate the modified simplified magnetic resonance index of activity (sMARIA) score using DWI on non-contrast magnetic resonance enterography (MRE) to evaluate active inflammation in patients with Crohn's disease (CD), compared to the original sMARIA scoring system. The study found that the modified sMARIA had higher accuracy in detecting active inflammation and showed comparable performance to sMARIA using contrast-enhanced MRE.

Interplay between chronic inflammation and clonal haematopoiesis of indeterminate potential in Behcet's disease

Summary: This study investigated the emergence rate of clonal haematopoiesis of indeterminate potential (CHIP) and its association with inflammatory markers in patients with Behcet's disease (BD). The results showed that the detection rate of CHIP in BD patients was 11.1%, which was not significantly different from the control group. Among the BD patients, DNMT3A mutations were the most common. CHIP carriers with BD had higher levels of inflammatory markers and older age compared to non-CHIP carriers with BD.

Factors associated with the prescription of probiotics in patients with inflammatory bowel disease: a cross-sectional study

Summary: This study explored the use of probiotics and factors associated with their prescription in patients with inflammatory bowel disease (IBD). The results showed that increased disease activity may be associated with fewer prescriptions of probiotics. However, physicians prescribed probiotics to control symptoms in both UC and CD patients. The use of MTX and iron, as well as a history of IBD-related surgeries, were associated with more frequent probiotic prescriptions in CD patients.
