Summary: Pancreatic ductal adenocarcinoma (PDAC) is a relatively uncommon cancer with increasing incidence each year, resulting in a high percentage of patients being diagnosed with advanced disease. Current cytotoxic therapies for PDAC are moderately effective, emphasizing the importance of multidisciplinary management, comprehensive germline testing, and integrated supportive care for all patients.

Summary: Real-world data on the management and outcomes of pancreatic cancer showed that only half of elderly patients aged 75-84 with a surgically resectable tumor actually underwent resection, and a significant percentage of patients aged >= 85 refused chemotherapy, highlighting the need for expanded geriatric assessments.

Summary: This study investigated the clinical impact of sarcopenia and skeletal muscle density (SMD) in patients with metastatic pancreatic adenocarcinoma who underwent palliative first line gemcitabine-based chemotherapy. The results showed that low SMD and low SMI were associated with poor overall survival, and the co-presence of low SMI and low SMD had more powerful prognostic implication for overall survival. Severe chemotherapy toxicity was observed more frequently in patients with sarcopenia and low SMD, suggesting that assessing skeletal muscle parameters comprehensively may be more useful as prognostic factors.

Summary: This study investigates the impact of sex disparities on clinical outcomes and tumor immune profiles in patients with pancreatic ductal adenocarcinoma (PDAC). The study found that women who underwent resection preceded by neoadjuvant chemoradiotherapy had a higher overall survival rate compared to men, possibly due to their heightened sensitivity to gemcitabine-based treatment and lower levels of protumoral M2 macrophages in the tumor microenvironment.

Summary: This study retrospectively analyzed the clinical data of 26 patients with high grade pleomorphic xanthoastrocytomas (HGPXAs) who underwent surgical treatment. The results showed that complete surgical excision plus radiotherapy and chemotherapy is recommended as a treatment protocol. Further studies are needed to verify these findings.

Summary: This systematic review examines the association between body composition and chemotherapy-related toxicity in pancreatic cancer patients, and evaluates the different definitions of sarcopenia. The results show a wide variability in the association between body composition and chemotherapy-related toxicity in PC patients, and there is no uniform definition for sarcopenia.

Summary: This study investigated different treatment strategies for elderly patients with stage I pancreatic cancer. The results showed that surgical resection and post-operative chemotherapy are recommended for patients who can tolerate treatment, but post-operative chemoradiotherapy does not provide survival benefits compared to post-operative chemotherapy. For untreated patients, radiotherapy, chemotherapy, or the combination of radiotherapy and chemotherapy are significantly related to prognosis, with chemoradiotherapy having the most obvious benefit.

Summary: Liver transplant has evolved from a procedure with high mortality to a routine procedure with good survival outcomes, especially with the development of living donor liver transplant. The scope of indications for liver transplant has expanded to include various types of liver cancers and even multiple organ failure, with increasing use in elderly patients. Further development of noninvasive predictors is needed to improve patient management and outcomes.

Summary: The study found that HBV integrations are frequent in HCC tumors and are associated with increased TERT mRNA expression and more aggressive tumor behavior. Specifically, integration at the TERT promoter by HBV enhancer I (EnhI) led to TERT activation. Additionally, the E74 like ETS transcription factor 4 (ELF4) was identified as a key factor in driving telomerase activation in HCC cells through interaction with HBV.

Summary: The impact of three-dimensional (3D) visualization on laparoscopic hepatectomy for hepatocellular carcinoma was investigated in this retrospective study. The results showed that the availability of 3D laparoscopy has led to a significant increase in the proportion of laparoscopic major hepatectomies and difficult resections. After propensity score matching, patients who underwent 3D laparoscopic hepatectomy had comparable operative time and blood loss with those who underwent 2D laparoscopic hepatectomy, but had lower overall complications. Furthermore, patients who underwent 3D laparoscopic major hepatectomy had a shorter hospital stay compared to those who underwent open surgery.

Summary: This study aims to identify the lower limit of GRWR compatible with enhanced recovery after living donor liver transplant and provide recommendations from an international expert panel. The results suggest that GRWR of >= .8% is often compatible with enhanced recovery, but grafts < .8% can be used in selected LDLT recipients with optimal donor-recipient selection, surgical technique, and perioperative management.

Summary: The impact of tumor biology on radical treatment for oligo-recurrence of post-transplant hepatocellular carcinoma (HCC) remains uncertain. This retrospective study found that early recurrence, liver recurrence, and high levels of AFP upon recurrence were predictors of mortality. Radical treatment was associated with improved post-recurrence survival in patients with early and liver recurrence, but not in patients with high AFP levels. The survival benefits of radical therapy were reduced in early recurrence and high AFP, but remained similar among patients with and without liver recurrence. The presence of multiple biological risk factors diminished the survival benefit of radical therapy.
