Summary: This study investigated preoperative hypokalemia in elderly patients and its effect on postoperative complications. The results showed that female gender, pre-existing hypokalemia at admission, and oral laxative twice or more were independent risk factors for preoperative hypokalemia in elderly patients. However, there was no increase in postoperative complications and 30-day mortality, which may be attributed to the monitoring of blood gas analysis and prompt correction of potassium levels during surgery.

Summary: Phosphatase is crucial in many biological functions, with alkaline phosphatase (ALP) serving as a key prognostic indicator for various diseases, though current detection methods are limited to in vitro colorimetric techniques. Fluorescence imaging provides a reliable way to monitor real-time changes in ALP levels, with research focusing mainly on ALP imaging probes grouped by their luminescence mechanisms. Challenges and future prospects for phosphatase probes were also discussed.

Summary: Intraoperative dexmedetomidine administration did not prevent postoperative delirium in elderly patients who underwent a laryngectomy. However, it reduced the incidence of postoperative delirium in patients with laryngeal cancer and higher tumor stage. Dexmedetomidine also reduced the levels of inflammatory markers CRP and IL-6 postoperatively.

Summary: Inspired by the strong binding capacity of pyrophosphate (ppi) and copper ion (Cu2+), a ratiometric fluorescence immunoassay triggered by alkaline phosphatase (ALP) was developed for the detection of zearalenone (ZEN). This method exhibited high accuracy and reliability, with a limit of detection (LOD) of 0.014 ng/mL and a linear detection range of 3.51-17.78 ng/mL. It provides a new approach for efficient and accurate detection of ZEN.

Summary: In this study, a method for synthesizing starch phosphate using the transferase properties of alkaline phosphatase was successfully explored. The crystallinity and enthalpy of the phosphorylated samples were reduced by up to 26.8% and 23.3%, respectively, and the grafting of phosphate radicals was further confirmed by X-ray photoelectron spectroscopy and time of flight secondary ion mass spectrometry.

Summary: Polymerase chain reaction (PCR) is an excellent technique for amplifying DNA and has potential in detecting various targets. In this study, a dNTP hydrolysis mediated PCR method was used to detect alkaline phosphatase (ALP) and mycotoxin zearalenone (ZEN). The combination of PCR and enzyme-linked immunosorbent assay (ELISA) further expanded the detection range. This method eliminated the need for raw material preparation and achieved satisfactory detection limits for ALP and ZEN.

The remimazolam group had better analgesic effects and lower pain intensity compared to the propofol group during the postoperative period.

High-volume PENG blocks may not be the ideal option for early rehabilitation, and persistent pain after PENG block might be due to the lack of obturator nerve articular branches blockade.

Summary: This study evaluated the analgesic efficacy of a four-quadrant transversus abdominis plane (4QTAP) block and a combination of 4QTAP block with needle electrical twitch and intramuscular electrical stimulation (NETOIMS) in patients undergoing cytoreductive surgery (CRS) followed by hyperthermic intraperitoneal chemotherapy (HIPEC). The results showed that the combination of 4QTAP block with NETOIMS provided better pain control and improved functional restoration and quality of recovery in patients.

Summary: This study evaluated the incidence of acute kidney injury (AKI) and complications in elderly patients undergoing total knee arthroplasty when either remimazolam (RMMZ) or sevoflurane (SEVO) were used. The results showed that the incidence of AKI was similar between the RMMZ and SEVO groups. The dose of intraoperative remifentanil, vasodilators, and additional sedatives was significantly higher in the RMMZ group compared to the SEVO group. Overall intraoperative heart rate and blood pressure tended to remain higher in the RMMZ group. The emergence time in the operating room was significantly faster in the RMMZ group, but the time required for an Aldrete score =9 was comparable between the RMMZ and SEVO groups. Postoperative complications and hospital length of stay were similar between the RMMZ and SEVO groups. Therefore, RMMZ may be recommended for patients expected to have decreased intraoperative vital signs, but its stable hemodynamics were not sufficient to prevent AKI.

The results showed that older age, history of hypertension, lower preoperative eGFR, and greater ACr2_pre were associated with the occurrence of eGFR6m <60. The combination of preoperative eGFR and ACr2_pre had the highest predictive power for eGFR6m <60.

Remimazolam should not be ruled out as the causative drug when anaphylaxis occurs during anesthesia induction.
