Serum Alkaline Phosphatase as a Predictor of Cardiac and Cerebrovascular Complications after Lumbar Spinal Fusion Surgery in Elderly: A Retrospective Study

We retrospectively enrolled 1395 patients aged > 65 years undergoing posterior lumbar spinal fusion surgery and classified them into tertiles based on serum Alkaline Phosphatase (ALP) levels (<63, 63-79, >79 IU/L). The primary outcome was the incidence of 30-day major adverse cardiac and cerebrovascular events (MACCE; composite endpoint defined as the occurrence of >= 1 of the following events: new-onset myocardial infarction, stroke, or cardiovascular mortality). The incidence of the composite endpoint was the highest in the third serum ALP tertile (0.4% vs. 0.2% vs. 2.2% in the first, second, and third tertile, respectively, p = 0.003). Multivariate analysis showed that the third serum ALP tertile was an independent predictor of the composite endpoint of MACCE (odds ratio 4.507, 95% confidence interval 1.378-14.739, p = 0.013). The optimal cut-off value of preoperative serum ALP showing the best discriminatory capacity to predict postoperative MACCE (measured by receiver-operating characteristic curve analysis) was 83 IU/L (area under curve 0.694, 95% confidence interval 0.574-0.813, p = 0.016). Preoperative serum ALP levels were independently associated with the composite endpoint of postoperative 30-days MACCE. We suggest that serum ALP can be used as a biomarker to predict cardiac and cerebrovascular complications following lumbar spinal fusion surgery in elderly patients.

The association of pancreatic cancer incidence with smoking status and smoking amount in Korean men

Summary: This study aimed to compare the quality of recovery (QoR) of remimazolam-based and propofol-based total intravenous anesthesia. The results showed that the QoR provided by both drugs was similar, and they can be used interchangeably in female patients undergoing thyroid surgery. The remimazolam group had better analgesic effects and lower pain intensity compared to the propofol group during the postoperative period.

Summary: This study evaluated the analgesic efficacy of a four-quadrant transversus abdominis plane (4QTAP) block and a combination of 4QTAP block with needle electrical twitch and intramuscular electrical stimulation (NETOIMS) in patients undergoing cytoreductive surgery (CRS) followed by hyperthermic intraperitoneal chemotherapy (HIPEC). The results showed that the combination of 4QTAP block with NETOIMS provided better pain control and improved functional restoration and quality of recovery in patients.

Comparison of the Incidence of Postoperative Acute Kidney Injury Following the Administration of Remimazolam or Sevoflurane in Elderly Patients Undergoing Total Knee Arthroplasty: A Randomized Controlled Trial

Summary: This study evaluated the incidence of acute kidney injury (AKI) and complications in elderly patients undergoing total knee arthroplasty when either remimazolam (RMMZ) or sevoflurane (SEVO) were used. The results showed that the incidence of AKI was similar between the RMMZ and SEVO groups. The dose of intraoperative remifentanil, vasodilators, and additional sedatives was significantly higher in the RMMZ group compared to the SEVO group. Overall intraoperative heart rate and blood pressure tended to remain higher in the RMMZ group. The emergence time in the operating room was significantly faster in the RMMZ group, but the time required for an Aldrete score =9 was comparable between the RMMZ and SEVO groups. Postoperative complications and hospital length of stay were similar between the RMMZ and SEVO groups. Therefore, RMMZ may be recommended for patients expected to have decreased intraoperative vital signs, but its stable hemodynamics were not sufficient to prevent AKI.

The results showed that older age, history of hypertension, lower preoperative eGFR, and greater ACr2_pre were associated with the occurrence of eGFR6m <60. The combination of preoperative eGFR and ACr2_pre had the highest predictive power for eGFR6m <60.

Remimazolam Anaphylaxis during Induction of General Anesthesia Confirmed by Provocation Test-A Case Report and Literature Review

Summary: This case report presents a confirmed case of remimazolam anaphylaxis using a provocation test. Remimazolam should not be ruled out as the causative drug when anaphylaxis occurs during anesthesia induction. A provocation test should be considered to confirm the diagnosis in case of a negative skin test result for remimazolam.
