Lung Carcinoma Predictive Biomarker Testing by Immunoperoxidase Stains in Cytology and Small Biopsy Specimens Advantages and Limitations

Context.-In the burgeoning era of molecular genomics, immunoperoxidase (IPOX) testing grows increasingly relevant as an efficient and effective molecular screening tool. Patients with lung carcinoma may especially benefit from the use of IPOX because most lung carcinomas are inoperable at diagnosis and only diagnosed by small tissue biopsy or fine-needle sampling. When such small specimens are at times inadequate for molecular testing, positive IPOX results still provide actionable information. Objective.-To describe the benefits and pitfalls of IPOX in the detection of biomarkers in lung carcinoma cytology specimens and small biopsies by summarizing the currently available commercial antibodies, preanalytic variables, and analytic considerations. Data Sources.-PubMed. Conclusions.-Commercial antibodies exist for IPOX detection of aberrant protein expression due to EGFR L858R mutation, EGFR E746_A750 deletion, ALK re-arrangement, ROS1 rearrangement, and BRAF V600E mutation, as well as PD-L1 expression in tumor cells. Automated IPOX protocols for ALK and PD-L1 detection were recently approved by the Food and Drug Administration as companion diagnostics for targeted therapies, but consistent interpretive criteria remain to be elucidated, and such protocols do not yet exist for other biomarkers. The inclusion of cytology specimens in clinical trials would expand patients' access to testing and treatment, yet there is a scarcity of clinical trial data regarding the application of IPOX to cytology, which can be attributed to trial designers' lack of familiarity with the advantages and limitations of cytology. The content of this review may be used to inform clinical trial design and advance IPOX validation studies.

Assessment of the feasibility of frozen sections for the detection of spread through air spaces (STAS) in pulmonary adenocarcinoma

Summary: Intraoperative detection of STAS on frozen sections has low sensitivity and positive predictive value, while the determination of tumor grade on frozen sections offers better sensitivity and specificity, and is associated with recurrence-free survival. Further study is needed to explore the utility of assessing tumor grade on frozen sections.

Summary: This study reports on the clinical course, pulmonary pathology, and outcomes after lung transplantation in three patients with chronic respiratory failure due to postacute sequelae of SARS-CoV-2 infection. The major pathological findings in the lung explants suggest ongoing fibrosis, prominent macrophage infiltration, neoangiogenesis, and microvascular thrombosis.

Diagnostic Challenges in the Cytology of Thymic Epithelial Neoplasms

Summary: Thymic epithelial neoplasms, including thymoma, thymic carcinoma, and thymic neuroendocrine neoplasms, are rare tumors that make up the majority of anterior mediastinal masses. Fine needle aspirations (FNA) of these masses are challenging to interpret due to the histologic diversity of thymic neoplasms and overlapping features with tumors from other sites. However, with the use of ancillary studies and radiology, FNAs can provide clinically actionable results. This review discusses the utility and limitations of thymic FNAs and highlights the diagnostic features and pitfalls of these specimens.

The Milan System for Reporting Salivary Gland Cytopathology A Cytohistologic Correlation and Interobserver Study From a Large Academic Medical Center

Cervicovaginal cytology, HPV testing and vaginal flora in transmasculine persons receiving testosterone

Summary: This study aimed to evaluate cervicovaginal cytology specimens and high-risk HPV testing in transmasculine individuals receiving testosterone. The study found that transmasculine individuals showed higher rates of abnormal Pap tests, and testosterone seemed to induce changes in squamous cells and shifts in vaginal flora. HR-HPV testing can be a useful adjunct in the workup of abnormal Pap tests from transmasculine individuals.

Amyloid deposition in an explanted bioprosthetic aortic valve: case report and review of the literature

Summary: This article presents a case study of an 80-year-old man who underwent aortic valve replacement 12 years ago and subsequently required re-replacement due to calcification. The surgically explanted valve tissue showed amyloid deposits with specific protein composition. The amyloid was classified as Mu heavy chain, which is uncommon in bioprosthetic valves. The article also provides a literature review on isolated amyloid deposition in aortic valves.

Case Report: Giant Thyroid Angiolipoma-Challenging Clinical Diagnosis and Novel Genetic Alterations

Summary: This article reports a rare case of lipomatous thyroid tumor, which was initially non-diagnostic through fine needle aspiration and eventually diagnosed as a benign pathology after surgical removal and pathological examination. Genetic testing revealed multiple genetic alterations in this case, supporting the concept of angiolipomas being true neoplasms. The case suggests that conservative surgical management can be considered for lipomatous thyroid tumors before a final pathological diagnosis is determined.

Minor Salivary Gland Biopsy in Diagnosis of Sjogren's Syndrome

Summary: This study aims to assess the adequacy and risk of complications of minor salivary gland biopsy in the diagnosis of Sjogren's syndrome.
