A rapid smartphone-based lactate dehydrogenase test for neonatal diagnostics at the point of care

There is a growing recognition of the importance of point-of-care tests (POCTs) for detecting critical neonatal illnesses to reduce the mortality rate in newborns, especially in low-income countries, which account for 98 percent of reported neonatal deaths. Lactate dehydrogenase (LDH) is a marker of cellular damage as a result of hypoxia-ischemia in affected organs. Here, we describe and test a POC LDH test direct from whole blood to provide early indication of serious illness in the neonate. The sample-inresult- out POC platform is specifically designed to meet the needs at resource-limited settings. Plasma is separated from whole blood on filter paper with dried-down reagents for colorimetric reaction, combined with software for analysis using a smartphone. The method was clinically tested in newborns in two different settings. In a clinical cohort of newborns of Stockholm (n = 62) and Hanoi (n = 26), the value of R using Pearson's correlation test was 0.91 (p < 0.01) and the R-2 = 0.83 between the two methods. The mean LDH (+/- SD) for the reference method vs. the POC-LDH was 551 (+/- 280) U/L and 552 (+/- 249) U/L respectively, indicating the clinical value of LDH values measured in minutes with the POC was comparable with standardized laboratory analyses.

Right Heart Structure, Geometry and Function Assessed by Echocardiography in 6-Year-Old Children Born Extremely Preterm-A Population-Based Cohort Study

Summary: This study found that children born extremely preterm at 6.5 years old exhibited important changes in the right heart structure and function, including smaller right atria, right ventricles, and higher estimated pulmonary vascular resistance. Preterm children with PDA had higher PVR and right ventricular myocardial performance index compared to those without PDA, suggesting an association between PDA and altered cardiac outcomes in extremely preterm children.

Multi-drug resistance in Streptococcus pneumoniae among children in rural Vietnam more than doubled from 1999 to 2014

Summary: The diversity of bla(NDM-1)-encoding plasmids from neonates at a large Vietnamese hospital was characterized using optical DNA mapping (ODM) and next-generation sequencing (NGS). The results showed that there were at least 10 different plasmids encoding bla(NDM-1) among the 18 isolates, indicating wide plasmid diversity. This study highlights the importance of ODM for plasmid analysis.

Phase-Changing Glauber Salt Solution for Medical Applications in the 28-32 °C Interval

Incidence of Intrapartum-Related Events at the Largest Obstetric Hospital in Hanoi, Vietnam: A Retrospective Study

Summary: Every year, a large number of neonates die or suffer injuries during their first month of life, mainly due to intrapartum-related events, prematurity, and infection. This study focused on neonates born at the largest obstetric hospital in Hanoi, Vietnam, and found that while other indicators were consistent with previous data, the number of intrapartum-related events was surprisingly low, even compared to high-income countries. Further research on the current clinical practices at Phu San Hanoi Hospital may help identify the factors contributing to this success.

Summary: In this study, the colonization rate of carbapenemase-producing Enterobacterales (CPE) on admission to intensive care units in Vientiane, Lao PDR was determined. The prevalence of extended-spectrum beta-lactamases (ESBLs) was high, while carbapenemases were less common. However, clonal dissemination of bla(NDM-1)-producing Klebsiella pneumoniae isolates in one of the intensive care units was detected, highlighting the need for stringent infection prevention and antimicrobial stewardship strategies.

Summary: Neonatal sepsis is a major problem in low- and middle-income countries, and understanding the challenges of managing global multi-center research studies is crucial for obtaining high-quality data. This paper discusses the complexities faced by diverse research teams in different countries, and presents actions taken to overcome these challenges in a large observational study of neonatal sepsis. Flexibility in recruitment strategies, ongoing training, and meticulous design and monitoring of the database were key to success. The importance of a strong central management team and interdisciplinary collaborators was also emphasized.

Endotracheal intubation performance at a large obstetric hospital delivery room, Hanoi, Vietnam

Summary: This study conducted at Phu San Hanoi Hospital in Vietnam found that incidences of positive pressure ventilation and endotracheal intubation were low in comparison to global estimates. Three out of four intubations were successful at the first attempt, with a median first attempt duration longer than recommended. Future studies should focus on achieving and maintaining intubation skills, and consider alternative devices for airway management at birth.

Prevalence of carbapenem resistance and its potential association with antimicrobial use in humans and animals in rural communities in Vietnam

Summary: Vietnam and Southeast Asia are hotspots for antimicrobial resistance and the prevalence of carbapenem-resistant Enterobacteriaceae (CRE), particularly Escherichia coli (CREC) and Klebsiella pneumoniae (CRKP) is emerging. The study found that in the Mekong Delta of Vietnam, CRE isolates were present in human rectal swabs (0.6%), pigs (0.7%), chickens (2.1%), and ducks (0.7%). The presence of CRE was higher among individuals who had used antimicrobials recently. All CRE/CRAB strains were multidrug resistant, but susceptible to some antibiotics. The identification of carbapenemase genes raises concerns about potential transmission of resistant organisms between healthcare and community settings.
