Histogram-based models on non-thin section chest CT predict invasiveness of primary lung adenocarcinoma subsolid nodules

IQR 109 pathologically proven subsolid nodules (SSN) were segmented by 2 readers on non-thin section chest CT with a lung nodule analysis software followed by extraction of CT attenuation histogram and geometric features. Functional data analysis of histograms provided data driven features (FPC1,2,3) used in further model building. Nodules were classified as pre-invasive (P1, atypical adenomatous hyperplasia and adenocarcinoma in situ), minimally invasive (P2) and invasive adenocarcinomas (P3). P1 and P2 were grouped together (T1) versus P3 (T2). Various combinations of features were compared in predictive models for binary nodule classification (T1/T2), using multiple logistic regression and non-linear classifiers. Area under ROC curve (AUC) was used as diagnostic performance criteria. Inter-reader variability was assessed using Cohen's Kappa and intra-class coefficient (ICC). Three models predicting invasiveness of SSN were selected based on AUC. First model included 87.5 percentile of CT lesion attenuation (Q. 875), interquartile range (IQR), volume and maximum/minimum diameter ratio (AUC: 0.89, 95% CI:[0.75 1]). Second model included FPC1, volume and diameter ratio (AUC: 0.91, 95% CI:[0.77 1]). Third model included FPC1, FPC2 and volume (AUC: 0.89, 95% CI:[0.73 1]). Inter-reader variability was excellent (Kappa: 0.95, ICC: 0.98). Parsimonious models using histogram and geometric features differentiated invasive from minimally invasive/pre-invasive SSN with good predictive performance in non-thin section CT.

Summary: The natural history and outcome of unruptured posterior circulation dissecting fusiform aneurysms is not fully understood, and the treatment approaches for these aneurysms are challenging and controversial. This retrospective study analyzed data from 78 patients with unruptured intradural vertebrobasilar dissecting aneurysms and found that the baseline diameter, length, and symptomatic presentation of the aneurysms were significantly associated with growth over time. Coronary disease and diabetes were associated with an increase in mRS scores.

Summary: The COVID-19 pandemic had a significant impact on stroke admissions, IV thrombolysis (IVT), and mechanical thrombectomy volumes, with a decline observed during the first year. However, mechanical thrombectomy volumes remained stable in high-volume centers. These findings suggest a preservation of stroke care for higher severity of disease during the first year of the pandemic.

Summary: This study is a multicenter retrospective exploration of the treatment outcomes of the Woven EndoBridge (WEB) device for intracranial aneurysms. The findings suggest an increasing tendency to treat patients with ruptured aneurysms and aneurysms with smaller dimensions over time. There is also a trend towards more off-label use of WEB for sidewall aneurysms and increased adoption of transradial access. Furthermore, the study shows improved experience in using WEB, leading to higher rates of adequate occlusion over time.

Cerebroprotection in the endovascular era: an update

Summary: Despite advances in clinical diagnosis and increasing numbers of eligible patients for revascularisation, ischaemic stroke remains a significant public health concern. Recanalisation therapy alone is not sufficient, and successful translational attempts for cerebroprotection are lacking. Therefore, understanding the diverse cellular effects and complex pathophysiological cascade of ischaemic stroke, as well as developing new multimodal and multicellular cerebroprotective agents, is crucial for improving patient outcomes.

Summary: This study aimed to evaluate the long-term follow-up of aneurysms completely occluded at 6-month follow-up and compare the predictive value of different imaging modalities. The results showed that 90.3% of aneurysms completely occluded at 6 months demonstrated complete occlusion at final follow-up, with a positive predictive value of 88.4% at first follow-up, regardless of DSA, MRA, or CTA.

Summary: This systematic review and meta-analysis found that there was no significant difference in clinical and safety outcomes between first pass thrombolysis in cerebral infarction 2b and multiple pass thrombolysis in cerebral infarction 2c-3 for acute ischemic stroke. Future prospective studies and clinical trials should explore whether first pass thrombolysis in cerebral infarction 2b is a viable endpoint for thrombolysis in cerebral infarction 2c-3.

Summary: This study aimed to assess the relationship between the novel compensation index (CI) and already established CTP collateral markers, as well as to find a CS biomarker that demonstrates a stronger correlation with DSA-CS. The results showed that CI significantly correlated with DSA-CS, while CBV Index did not show any correlation.

Summary: The 7-day NIHSS score after EVT may serve as a reliable predictor of long-term outcomes in stroke patients. Patients who had an NIHSS score of 6 or lower at 7 days post-EVT had a higher proportion of favorable outcomes at 90 days. This finding provides a valuable tool for predicting the prognosis of stroke patients.

Summary: This review examines the current practices of informed consent in acute stroke treatment, specifically for patients treated with IVT or EVT, and identifies the variability and knowledge gaps in IC for these treatments. Challenges in assessing decision-making capacity and the lack of standardized guidance contribute to these gaps. Future initiatives should focus on simplifying information delivery to patients, developing formal tools for capacity assessment, standardizing ethical frameworks, and harmonizing IC standards across sites to enhance IC practices and uphold patient autonomy.
