The value of indocyanine green clearance assessment to predict postoperative liver dysfunction in patients undergoing liver resection

Postoperative liver dysfunction remains a major concern following hepatic resection. In order to identify patients who are at risk of developing liver dysfunction, indocyanine green (ICG) clearance has been proposed to predict postoperative liver function. All patients who underwent liver resection at the Medical University Vienna, Austria between 2006 and 2015 with preoperative ICG clearance testing (PDR, R15) were analyzed in this study. Postoperative liver dysfunction was analyzed as defined by the International Study Group of Liver Surgery. Overall, 698 patients (male: 394 (56.4%); female: 304 (43.6%)) with a mean age of 61.3 years (SD: 12.9) were included in this study, including 313 minor liver resections (44.8%) and 385 major liver resections (55.2%). One hundred and seven patients developed postoperative liver dysfunction after liver resection (15.3%). Factors associated with liver dysfunction were: male sex (p = 0.043), major liver resection (p < 0.0001), and preoperative ICG clearance (PDR (p = 0.002) and R15 (p < 0.0001)). Notably ICG clearance was significantly associated with liver dysfunction in minor and major liver resections respectively and remained a predictor upon multivariable analysis. An optimal cut-off for preoperative ICG clearance to accurately predict liver dysfunction was PDR < 19.5%/min and R15 > 5.6%. To the best of our knowledge, this is the largest study analyzing the predictive value of preoperative ICG clearance assessment in patients undergoing liver resection. ICG clearance is useful to identify patients at risk of postoperative liver dysfunction.

Thermal effects of various drill materials during implant site preparation-Ceramic vs. stainless steel drills: A comparative in vitro study in a standardised bovine bone model

Summary: The study aimed to evaluate thermal effects of ceramic and metal implant drills during implant site preparation using a standardized bovine model. It found that drill material and geometry, as well as external irrigation, drilling depth, and drill diameter, significantly impact heat generation. Metal drills showed significantly lower temperatures compared to ceramic drills, with some ceramic drills reaching or exceeding the harmful temperature threshold at 16-mm drilling depths.

Summary: Temporal muscle thickness has been shown to be a potential parameter reflecting sarcopenia and an independent prognostic marker in primary central nervous system lymphoma (PCNSL) patients. Lower muscle thickness was associated with increased risk of death in PCNSL patients, suggesting its systematic inclusion in further translational and clinical studies to validate its role as a prognostic biomarker.

Summary: This study evaluated the predictive accuracy of published risk estimation models for the development of GDM in early pregnancy. Most models showed acceptable discrimination but lacked calibration, with simple biochemical variables like fasting glucose, HbA1c, and triglycerides improving risk prediction. A model incorporating clinical and laboratory parameters demonstrated satisfactory accuracy for further investigations.

Management of hard-to-heal leg ulcers with an acid-oxidising solution versus standard of care: the MACAN study

Summary: In this study conducted in Austria, a new therapeutic approach using an acid-oxidising solution (AOS) was found to be more effective in promoting wound healing progress and achieving faster healing time compared to standard of care (SOC) wound dressings. Patients treated with AOS showed higher amounts of granulation and re-epithelialisation tissue, faster wound size reduction, and a greater percentage of complete healing of hard-to-heal ulcers. Additionally, AOS was shown to have better tolerability and safety profiles, with fewer adverse events compared to SOC.

Weighted pseudo-values for partly unobserved group membership in paediatric stem cell transplantation studies

Summary: This study discusses the application of the generalised pseudo-values approach in evaluating the impact of allogeneic stem cell transplantation on childhood leukaemia and proposes a new weighted approach for more efficient estimation by adjusting weights in patient cohort grouping and pseudo-value calculation. Extensive computer simulations show that both approaches provide unbiased estimates with satisfactory confidence interval coverage in typical clinical scenarios, with the weighted pseudo-values approach being preferable in situations where donor identification takes longer than usual. Both approaches complement each other in addressing different aspects of the underlying medical question.
