Summary: Replacement of the common hepatic artery during pancreaticoduodenectomy is a challenging condition with a prevalence of 1.8% in the general population. A 6-type classification system introduced by Huang is used to categorize these cases, with the type II setting being extremely rare. Preservation of the replaced common hepatic artery is recommended whenever possible during the procedure.

Summary: The efficacy of FOLFIRINOX chemotherapy has renewed interest in surgery for locally advanced pancreatic ductal adenocarcinoma. Recent reports have shown acceptable short and long-term outcomes for patients undergoing pancreatectomy with arterial and venous resection by high volume institutions with dedicated surgical teams, including extensive bowel mobilization and specific arterial reconstruction.

Summary: Knowledge of variations in the celiac trunk is crucial for successful abdominal surgeries. This case report describes a rare absence of the celiac trunk artery and the common hepatic artery, and how the use of computerized 3D vascular reconstruction helped in preoperative planning and the successful laparoscopic radical resection of gastric cancer.

Summary: The trajectory of the splenic artery and its branches in the hilum region was studied on 330 organ complexes. The splenic artery can have four different types of trajectory: straight, sinusoidal, serpentine, and alternating. The presence of certain branches and accessory spleens can affect the length of the pancreas.

Summary: In this study, a retrospective analysis of 244 CT scans performed between June and July 2019 revealed that 72.8% of participants had the umbilical fissure vein (UFV), with most draining into the left hepatic vein. The findings suggest that pre-operative identification of UFV can assist in surgical planning and serve as a landmark during procedures such as left lateral segmentectomy and extended right hepatectomy.

Summary: This study compared the surgical volumes and exposure areas of different surgical approaches to Meckel's cave and found that endoscopic approaches can provide adequate exposure, especially for the medial portions, while microsurgical transcranial approaches still seem to be the gold standard for optimal exposure and surgical volumes in the most lateral portion of Meckel's cave.

Summary: The resectability of pancreatic ductal adenocarcinoma (PDAC) is determined by its relationship with peripancreatic vasculature. Various surgical techniques can be used to resect and reconstruct the replaced hepatic arteries, ensuring adequate blood flow to the liver. These improvements in surgical techniques provide more treatment options and hope for patients with PDAC.

Summary: Extracorporeal Membrane Oxygenation (ECMO) has a reasonable survival rate in children with primary immunodeficiencies (PID) who require respiratory support. The type of immunodeficiency and the decade of support do not affect survival rates. The presence of a pre-ECMO infection or infection during the ECMO run does not impact survival rates. Cardiac, pulmonary, and neurological complications are independently associated with increased mortality.

Summary: Epidemiological studies have shown an association between thyroid diseases and extra-thyroidal malignancies, particularly breast cancer. Patients with autoimmune thyroiditis have an increased risk of breast cancer, although the risk is lower for those who are thyroid autoantibody-positive. The molecular links between thyroid autoimmunity and breast cancer remain uncertain.

Summary: This article reviews the current literature on women in surgery and ergonomics. The study found that women face challenges in surgery due to their shorter height and smaller glove size. Additionally, women experience more musculoskeletal pain than men, possibly due to the size and design of surgical tools being biased towards male and taller individuals.

Summary: Benign and malignant thyroid diseases are linked to the occurrence of extrathyroidal malignancies, including colorectal cancers. Understanding the relationship between these diseases can provide insights into their etiology and/or progression, as well as potential shared genetic and environmental factors. Exploring the underlying molecular mechanisms can lead to improved clinical management and increased surveillance for patients with thyroid diseases at a higher risk of developing extrathyroidal malignancies.

Summary: Persistent Mullerian Duct Syndrome (PMDS) is a rare autosomal recessive disorder in individuals with XY genotype, characterized by the presence of female reproductive structures. The main complications of PMDS are infertility and the risk of malignant degeneration. We report a case of a 49-year-old man with PMDS who declined surgical treatment. Subsequent imaging studies showed no signs of malignancy, but regular follow-up is necessary.

Summary: This study systematically reviewed and meta-analyzed the efficacy of sleeve gastrectomy (SG) with concomitant hiatal hernia repair (SG + HHR) versus SG with fundoplication (SG + FP) on gastroesophageal reflux disease (GERD) remission in patients with obesity. The results showed that SG + FP had better control of GERD compared to SG + HHR, despite a higher overall complication rate. Both strategies can be considered as suitable alternatives to conventional SG in patients with obesity and concomitant hiatal hernia and/or GERD.

Summary: This study compared two groups of patients who underwent difficult colon polypectomies with and without the assistance of the Endocuff Vision device. The results showed that Endocuff Vision significantly improved endoscope stability and reduced operation time. Summary: Basosquamous cell carcinoma (BSCC) is a rare malignancy that occurs on sun-exposed areas of the skin. This article reports the case of a 70-year-old male with BSCC in the nipple-areola complex, which is the first reported case in this area. The patient presented with a fast-growing bleeding ulcerated lesion that involved the nipple-areola complex. The diagnosis was confirmed through biopsy and the patient underwent surgery without any signs of recurrence or metastases at the follow-up.
