The anatomy of cancer patients changes between radiation treatment planning and delivery as well as over the course of radiotherapy. Adaptive radiotherapy CARD aims to deliver radiation accurately and precisely in the presence of such changes. To that end, ART uses an imaging feedback loop to quantify these changes and modify the treatment plan accordingly. This paper provides an overview of anatomical changes occurring over the course of therapy and various adaptive strategies developed to account for those. Moreover, residual uncertainties present in adaptive radiotherapy are discussed as well as required tools, potential pitfalls and remaining challenges. (C) 2019 The Authors. Reducing 4DCBCT scan time and dose through motion compensated acquisition and reconstruction

Summary: Adaptive 4DCBCT has been developed to reduce imaging dose and scan time, and this study investigated reconstruction algorithms for optimal image quality. Real patient data from the first 10 adaptive 4DCBCT patients were used to assess imaging dose reduction and scan time improvement compared to conventional 4DCBCT. Reconstruction algorithms FDK, MKB, MCFDK, and MCMKB were evaluated based on SSIM, SNR, CNR, TIS-D, and TIS-T metrics, showing significant image quality improvements with adaptive 4DCBCT acquisitions.

Adaptive margins for online adaptive radiotherapy

Summary: This study evaluates and validates the dosimetric effect of respiratory motion and inter-fraction variations in SBRT using 4D-CT and daily 4D-CBCT dose accumulation scenarios. The results show that both 4D-Acc and 3D-Acc have an impact on the dose, especially in the tumor and organs at risk region. The effectiveness of CT-CBCT deformable image registration techniques is also validated.

Summary: This study aims to evaluate the efficacy and safety of Stereotactic Arrhythmia Radiotherapy (STAR). It found that STAR can reduce the number of treated ventricular tachycardia episodes by more than 50% in 67% of patients with therapy-refractory VT. No decrease in cardiac and pulmonary function or treatment-related serious adverse events were observed during follow-up.

A systematic literature review of definitions and classification systems for radiotherapy innovation: A first step towards building a value-based assessment tool for radiation oncology

Summary: In order to address the evaluation issue of radiotherapy innovations, the HERO project of ESTRO has initiated the development of a radiotherapy-specific value-based framework. A systematic literature search revealed no widely accepted definition or classification system for radiotherapy innovations. However, the unique properties of radiotherapy interventions can be used to categorize innovations in radiation oncology, but clear terminology denoting radiotherapy-specific characteristics is still needed.

Summary: This systematic review examines the use of proton beam therapy (PBT) in the management of recurrent central nervous system (CNS) and base of skull (BoS) tumors. The findings suggest that PBT can provide good local control with acceptable toxicity rates in the reirradiation setting. However, there are several factors to consider that may increase the risk of toxicities in CNS or BoS tumor recurrences, highlighting the importance of patient selection.
