The relationship between the concentration of plasma homocysteine and chronic kidney disease: a cross sectional study of a large cohort

Background High concentrations of homocysteine are considered a risk factor for developing atherosclerosis and coronary artery disease. The aim of this study was to assess the concentrations of homocysteine in subjects with chronic kidney disease (CKD). Methods Data were collected from medical records of individuals examined at a screening center in Israel between the years 2000-2014. Cross sectional analysis was carried out on 17,010 subjects; 67% were men. Results Significant differences were observed between four quartiles of homocysteine concentrations and estimated glomerular filtration rate (eGFR)-the higher the homocysteine concentration, the lower the eGFR (p < 0.0001). In subjects with CKD, homocysteine plasma levels were correlated with the stage of renal impairment. Mean (SD) homocysteine concentrations in subjects with eGFR < 60 mL/min per 1.73 m(2) compared to subjects with eGFR >= 60 mL/min per 1.73 m(2) were: 16.3 (5.9) vs. 11.5 (5.5) mu mol/L respectively. These findings remained significant after adjustment for age, smoking status, body mass index, hypertension and diabetes mellitus (p < 0.0001). Compared to subjects with homocysteine concentrations less than 15 mu mol/L, those with homocysteine concentrations equal and above 15 mu mol/L, had a significantly higher odds ratio (95% CI) of having an eGFR < 60 mL/min per 1.73 m(2); non adjusted model, 8.30 (6.17-11.16); adjusted model for age smoking status, body mass index, hypertension and diabetes mellitus, 7.43 (5.41-10.21). Conclusion Plasma homocysteine concentrations are higher in subjects with CKD. This may contribute to an increased risk for developing atherosclerosis and coronary artery disease in these patients.

The diagnosis and management of long COVID are challenging due to a lack of objective findings, despite the abundance of descriptive data published so far. This review aims to describe the common manifestations of long COVID, discuss diagnostic and management challenges, and address specific aspects in hematologic patients.

ESCMID rapid guidelines for assessment and management of long COVID

Summary: This article provides guidelines for the assessment and management of individuals with persistent symptoms after acute COVID-19 infection, including a definition for 'long COVID'. The recommendations are based on a search of literature on epidemiology, symptoms, assessment, and treatment of long COVID. However, due to insufficient evidence, only conditional guidance is provided, and further high-quality research is recommended.

Summary: This study identified six unique patterns of symptoms in long COVID patients, including cognitive, pain-syndrome, pulmonary, cardiac, anosmia-dysgeusia, and headache. The cognitive pattern was the most common and was higher in outpatients during the acute phase. Only the cognitive and cardiac patterns were associated with failure to return to pre-COVID occupational and physical activity status.

Predictors of survival in elderly patients with coronavirus disease 2019 admitted to the hospital: derivation and validation of the FLAMINCOV score

Summary: The objective of this study was to identify predictors of 30-day survival in elderly patients with COVID-19. A retrospective cohort study was conducted on elderly patients hospitalized in six European sites. A predictive score (FLAMINCOV) was developed using logistic regression and validated in a separate cohort. The FLAMINCOV score showed good discrimination in predicting survival outcomes and may be beneficial for triage and management of elderly patients with COVID-19.

A Longitudinal Assessment of the Natural Change in Haemoglobin, Haematocrit and Mean Corpuscular Volume with Age

Summary: We analyzed the natural annual trends in haemoglobin, haematocrit, and mean corpuscular volume (MCV) in a population of adults and investigated the influence of clinical parameters. The study found a decline in haemoglobin and haematocrit levels and an increase in MCV, with the changes being more prominent in men. Gender was found to be the only parameter significantly associated with the decline in haemoglobin.

Summary: This study aimed to evaluate outcomes of P. aeruginosa bacteremia among older adults (> 80 years) and found that disease severity, place of acquisition, and background conditions were associated with mortality, while antimicrobial regimen was not. Therefore, preventive efforts and early diagnosis might be beneficial for improving outcomes in older adults with P. aeruginosa bacteremia.

Summary: The immune response to two doses of SARS-CoV-2 mRNA vaccine is limited among kidney transplant recipients. This study aimed to evaluate the humoral and cellular response to a third dose of the BNT162b2 vaccine. The results showed that the third dose improved the immune response among kidney transplant recipients, although 30% of them remained seronegative. Temporary reduction of immunosuppression before vaccination was found to improve the antibody response.
