Summary: Chronic rhinosinusitis with nasal polyps (CRSwNP) has been considered a benign, chronic inflammatory, and hyperplastic disease. Recent studies have shown that autoimmune-related mechanisms play a role in nasal polyp pathology. Innate and adaptive immune pathways are mobilized during polyp formation, involving activated plasma cells, eosinophils, basophils, innate type 2 lymphocytes, mast cells, and proinflammatory cytokines in polyp tissue. The discovery of autoantibodies further supports the autoimmune nature of nasal polyps. This study elaborates on the role of autoimmunity in the pathogenesis of CRSwNP, discussing microbiome-mediated mechanisms, abnormal host immunity, and genetic changes.

Summary: Chronic Rhinosinusitis with Nasal Polyps (CRSwNP) is a multifactorial disease, and bacterial superantigens such as Staphylococcus aureus are known to play a role in its development. Specifically, Staphylococcus aureus nasal colonization and IgE response to Staphylococcus aureus enterotoxins have been found in CRSwNP patients. Among the enterotoxins, enterotoxin B (SEB) has been identified as a key player in the pathogenesis of CRSwNP by disrupting mucosal integrity and triggering inflammatory responses. SEB activates Toll Like Receptor 2, leading to the production of pro-inflammatory cytokines, and induces cellular stress and oxidative damage. It also promotes eosinophil degranulation and localized eosinophilic inflammation. Furthermore, SEB may be involved in the regulation of immune cells and maintenance of sinonasal mucosal inflammation. While in vitro and animal studies have provided valuable insights, further research is needed to fully understand the role of SEB in CRSwNP.

Microbiota community diversity was significantly lower in NECRSwNP samples compared to healthy control subjects. The abundance of pathogenic bacteria varied between ECRSwNP and NECRSwNP patients, with a significantly higher abundance of S. aureus in NECRSwNP patients.

The recurrent group showed significantly higher total nasal symptom score, eosinophil and neutrophil cell count, and expression levels of IFN-, IL-17A, IL-17E, and IL-18 compared to the non-recurrent group.

Summary: The current methods for diagnosis of infections are complex and time-consuming. This study presents a novel multi-excitation Raman spectroscopy-based method that can detect microbial pathogens using unprocessed clinical samples, providing rapid data for diagnosis. The method uses differential excitation of molecular components in bacterial cells to obtain strain-level distinction. The results demonstrate high accuracy in detecting respiratory pathogens associated with cystic fibrosis.

Summary: This article discusses the importance of developing a skin-directed scoring system for EN, including consistent terminology and morphologic traits, as well as identifying the most affected sites. Through a Delphi consensus exercise, it was determined that there is a need for a skin-directed scoring system for EN.
