Early-Pregnancy Circulating Antioxidant Capacity and Hemodynamic Adaptation in Recurrent Placental Syndrome: An Exploratory Study

Background/Aims: Placental syndromes (PS) refer to pregnancy complications that include gestational hypertension, (pre)eclampsia, HELLP syndrome, and/or placental insufficiency-induced fetal growth restriction. These disorders are characterized by increased oxidative stress. This study aims to test the hypothesis that the abnormal hemodynamic adaptation to pregnancy, typical for early PS pregnancy, is accompanied by abnormal maternal levels of antioxidants relative to those in normal pregnancy. Methods: Before, and at 12, 16, and 20 weeks pregnancy, we measured trolox equivalent antioxidant capacity (TEAC), uric acid (UA), and TEACC (TEAC corrected for UA) in maternal serum of former PS patients, who either developed recurrent PS (rPS; n = 16) or had a normal next pregnancy (non-rPS; n = 23). Concomitantly, we also measured various hemodynamic variables. Results: rPS differed from non-rPS by higher TEACC levels before pregnancy (178 vs. 152 mu M; p = 0.02) and at 20 weeks pregnancy (180 vs. 160 mu M; p = 0.04). Only non-rPS responded to pregnancy by significant rises in hemodynamic measures. Conclusion: These data indicate that rPS pregnancies are preceded by an increase in antioxidant capacity, presumably induced by subclinical vascular injury and low-grade chronic inflammation.

Rebubbling and graft failure in Descemet membrane endothelial keratoplasty: a prospective Dutch registry study

Summary: This Dutch registry study identified risk factors for rebubbling and early graft failure in DMEK. Rebubbling was associated with higher graft failure, while older recipient age and surgical complications were risk factors for rebubbling. Early graft failure risk factors included transplantation before 2016 and surgical complications. Rebubbling was also associated with significantly lower endothelial cell densities.

Placebo response in pharmacological trials in patients with functional dyspepsia-A systematic review and meta-analysis

Summary: This study conducted a systematic review and meta-analysis to investigate the placebo response in pharmacological trials for functional dyspepsia (FD). The study found that the pooled placebo response rate ranged from 35.5% to 39.6% depending on the responder definition used. Lower baseline symptom scores were significantly associated with higher placebo response rates. The study suggests considering entry criteria based on symptom severity and separate reporting of core FD symptoms in future trials.

However, in the overall European population, the presence of the HLA-B27 gene does not affect survival or the risk of death from cardiovascular disease.

Unraveling the Antioxidant Activity of 2R,3R-dihydroquercetin

Summary: It has been found that 2R,3R-DHQ does not convert into quercetin in an oxidative environment, but forms an unstable oxidation product. The addition of glutathione leads to the formation of a 2R,3R-DHQ-GSH adduct, indicating oxidation of the B-ring. Ascorbate can reduce the quinone back to 2R,3R-DHQ. This suggests that the redox-modulating activity of 2R,3R-DHQ quinone resides in its B-ring.
