Rapid intrapartum test for maternal group B streptococcal colonisation and its effect on antibiotic use in labouring women with risk factors for early-onset neonatal infection (GBS2): cluster randomised trial with nested test accuracy study

Jane P. Daniels, Emily Dixon, Alicia Gill, Jon Bishop, Mark Wilks, Michael Millar, Jim Gray, Tracy E. Roberts, Jane Plumb, Jonathan J. Deeks, Karla Hemming, Khalid S. Khan, Shakila Thangaratinam

Summary: This study evaluated the use of intrapartum rapid test to detect maternal GBS colonization in women with clinical risk factors for early neonatal infection. The results showed that the use of rapid test did not reduce the rates of antibiotics administered to prevent neonatal early-onset GBS infection compared to usual care. The accuracy of the rapid test is within acceptable limits.

Lactic acid gel versus metronidazole for recurrent bacterial vaginosis in women aged 16 years and over: the VITA RCT

Lindsay Armstrong-Buisseret, Clare Brittain, Joe Kai, Miruna David, Jocelyn Anstey Watkins, Mara Ozolins, Louise Jackson, Zainab Abdali, Trish Hepburn, Frances Griffiths, Alan Montgomery, Jane Daniels, Alice Manley, Gillian Dean, Jonathan D. C. Ross

Summary: This study found that metronidazole was more effective than lactic acid gel in relieving symptoms, although both drugs showed similar treatment failure rates. Metronidazole had higher incidence of side effects and lower treatment cost. However, some participants preferred using lactic acid gel, despite its perceived lower effectiveness.

Effect of Regional vs General Anesthesia on Incidence of Postoperative Delirium in Older Patients Undergoing Hip Fracture Surgery The RAGA Randomized Trial

Ting Li, Jun Li, Liyong Yuan, Jinze Wu, Chenchen Jiang, Jane Daniels, Rajnikant Laxmishanker Mehta, Mingcang Wang, Joyce Yeung, Thomas Jackson, Teresa Melody, Shengwei Jin, Yinguang Yao, Jimin Wu, Junping Chen, Fang Gao Smith, Qingquan Lian

Summary: A randomized clinical trial in Southeastern China involving 950 patients aged 65 years and older undergoing hip fracture surgery found that regional anesthesia without sedation did not significantly reduce the incidence of postoperative delirium compared with general anesthesia. The study also investigated secondary outcomes such as delirium severity, duration, and subtype, as well as postoperative pain score, length of hospitalization, mortality, and complications.

Uterine artery embolization or myomectomy for women with uterine fibroids: Four-year follow-up of a randomised controlled trial

Jane Daniels, Lee J. Middleton, Versha Cheed, William Mckinnon, Fusun Sirkeci, Isaac Manyonda, Anna -Maria Belli, Mary Ann Lumsden, Jonathan Moss, Olivia Wu, FEMME Trial Collaborative Grp

Summary: This study aimed to compare the quality of life in women with symptomatic uterine fibroids treated with uterine artery embolization (UAE) or myomectomy. The results showed that myomectomy resulted in greater improvement in quality of life compared to UAE, although the difference was not statistically significant at four years. The pregnancy rate was lower in the UAE group and the repeat procedure rate was also lower. It is important to note that missing data may affect the generalizability of the results.

A rapid intrapartum test for group B Streptococcus to reduce antibiotic usage in mothers with risk factors: the GBS2 cluster RCT

Jane Daniels, Emily F. Dixon, Alicia Gill, Jon Bishop, Maria D'Amico, Khaled Ahmed, Julie Dodds, Kostas Tryposkiadis, Mark Wilks, Michael Millar, Shahid Husain, Jim Gray, Angela Whiley, Patrick Moore, Ruvimbo L. Munetsi, Karla Hemming, Tracy Roberts, Jane Plumb, Jonathan Deeks, Khalid S. Khan, Shakila Thangaratinam

Summary: This study investigated whether intrapartum antibiotic prophylaxis guided by a rapid test reduces antibiotic use in mothers and neonates. The results showed that this strategy did not reduce the rates of antibiotic prophylaxis for preventing early-onset group B Streptococcus infection, but did reduce neonatal antibiotic exposure.

Uterine artery embolisation versus myomectomy for premenopausal women with uterine fibroids wishing to avoid hysterectomy: the FEMME RCT

Jane Daniels, Lee J. Middleton, Versha Cheed, William McKinnon, Dikshyanta Rana, Fusun Sirkeci, Isaac Manyonda, Anna-Maria Belli, Mary Ann Lumsden, Jonathan Moss, Olivia Wu, Klim McPherson

Summary: This study compares the clinical effectiveness and cost-effectiveness of uterine artery embolisation and myomectomy in the treatment of symptomatic fibroids. It is a multicentre, open, randomised trial with a parallel economic evaluation conducted in 29 UK hospitals.

Effects on heavy menstrual bleeding and pregnancy of uterine artery embolization (UAE) or myomectomy for women with uterine fibroids wishing to avoid hysterectomy: The FEMME randomized controlled trial

Fusun Sirkeci, John Moss, Anna M. M. Belli, Klim McPherson, Jane Daniels, Isaac Manyonda, Lee Middleton, Versha Cheed, Olivia Wu, Mary A. A. Lumsden, FEMME Trial Collaborative Grp

Summary: This study compared myomectomy with uterine artery embolization (UAE) as treatment options for women who wish to avoid hysterectomy. The results showed that myomectomy resulted in better quality of life at 4 years compared with UAE, but both treatments had similar effects on menstrual bleeding and ovarian reserve.

Combination of gefitinib and methotrexate to treat tubal ectopic pregnancy (GEM3): a multicentre, randomised, double-blind, placebo-controlled trial

Andrew W. Horne, Stephen Tong, Catherine A. Moakes, Lee J. Middleton, W. Colin Duncan, Ben W. Mol, Lucy H. R. Whitaker, Davor Jurkovic, Arri Coomarasamy, Natalie Nunes, Tom Holland, Fiona Clarke, Ann M. Doust, Jane P. Daniels

Summary: This study evaluated whether oral gefitinib in combination with methotrexate is more effective than methotrexate alone in the treatment of tubal ectopic pregnancy. The results showed that oral gefitinib does not offer clinical benefit over methotrexate and increases minor adverse reactions.
