Ultrasonographic features for differentiating follicular thyroid carcinoma and follicular adenoma

Background: Preoperative differentiation of follicular thyroid carcinoma (FTC) from follicular adenoma (FA) remains an unsolved puzzle. Patients sometimes undergo unnecessary lobectomy for histology confirmation inevitably. Objective: In this retrospective study, we propose new gray-scale ultrasonographic (US) features that may help to differentiate FTC from FA. Method: Medical charts and US images of follicular thyroid neoplasms were collected prospectively. Gray-scale US features including conventional parameters adding tubercle-in-nodule and trabecular formation were recorded. Results: The histopathologic diagnosis was FA in 139 and FTC in 49 patients. In patients with FTC, minimally invasive follicular carcinoma (MIFC) was seen in 36 patients and widely invasive follicular carcinoma (WIFC) in 13. The incidences of calcifications (p < 0.0001), tubercle-in-nodule signs (p < 0.0001), spiculated margins (p = 0.014), and trabecular formations (p = 0.03) were significantly higher in FTC. Tubercle-in-nodule (p < 0.01) and calcification (p < 0.001) were independent factors in the differentiation of FTC in multivariate analysis (area under the curve = 0.689). Conclusions: US characteristics of tubercle-in-nodule in combination with calcification help to differentiate FTC from FA. (C) 2019 Asian Surgical Association and Taiwan Robotic Surgery Association. Multicenter Study of Benign Thyroid Nodules with Radiofrequency Ablation: Results of 762 Cases over 4 Years in Taiwan

Summary: This study evaluated the effectiveness and safety of radiofrequency ablation (RFA) treatment for benign thyroid nodules (BTNs) in five medical centers in Taiwan. The results showed that RFA had established reliability and safety for treating BTNs, and it has become an alternative to surgery in Taiwan. Different specialists may have varying outcomes in terms of volume reduction ratio (VRR) after 6 months of follow-up.

Summary: This study aimed to report the experience with the use of intraoperative transcutaneous laryngeal ultrasonography (TLUSG) in evaluating vocal cord twitch response in thyroid surgeries and compared it with laryngeal twitch palpation (LTP). TLUSG showed better reliability in assessing vocal cord twitch response compared to LTP, providing practical troubleshooting guidance for patients with no intraoperative neuromonitoring (IONM) signals.

Summary: The study investigated the effects of calcitriol on CD4(+) T-cell polarization and kidney injury during sepsis in obese mice. The results showed that calcitriol administration maintained blood CD4(+) T-cell levels, reduced the percentages of Th2 and Th17 cells, and modulated the Th17:Treg ratio. Additionally, calcitriol increased cathelicidin levels, decreased inflammatory chemokines and kidney injury markers, and promoted M1 macrophage polarization toward the M2 type.

Summary: This study investigated the effects of enteral cholecalciferol and/or intravenous calcitriol administration on immune cell subsets, the renin-angiotensin system, and acute lung injury in obese mice. The results suggest that calcitriol administration has a stronger impact on immune cell balance and anti-inflammatory pathways in the lungs, while both forms of vitamin D attenuate sepsis-induced lung injury in obese animals.

Urinary Exosomal Tissue TIMP and Angiopoietin-1 Are Preoperative Novel Biomarkers of Well-Differentiated Thyroid Cancer

Summary: Finding non-invasive and sensitive biomarkers for early screening of high-risk patients is crucial in clinical practice. This prospective study analyzed urine exosomal proteins in patients with thyroid cancer, aiming to identify new prognostic biomarkers for decision-making before surgery. The study found a significant association between urine exosomal TIMP and lymph node metastasis. However, no association was found between MACIS score and urine exosomal proteins. Further research is needed to determine the potential of urine exosomal proteins as prognostic indicators.

Synergic evacuation device helps smoke control during endoscopic thyroid surgery

Summary: This study retrospectively reviewed 327 patients who underwent endoscopic thyroidectomy and compared the use of a smoke evacuation system. The results showed that the group using the evacuation system had better field of view and shorter operating time. The conclusion is that using evacuators in low-pressure and small-space endoscopic thyroid procedures can enhance the field of view and optimize the operating time.
