Comparison of Coronary Atherosclerotic Plaque Burden and Composition as Assessed on Coronary Computed Tomography Angiography in East Asian and European-Origin Caucasians

Recent evidence suggests plaque morphology evaluated on coronary computed tomography angiography has prognostic implications. East Asians have a lower prevalence of myocardial infarction and cardiovascular mortality compared with European-origin Caucasians. We aimed to compare coronary atherosclerotic burden and plaque composition in a matched cohort of Caucasian and East Asians patients with stable chest pain who underwent computed tomography angiography. Two-hundred symptomatic patients (age 58.8 +/- 7.9, male 51%) were matched for age, gender, body mass index, and diabetes (100 each ethnic group). A blinded core-laboratory quantified calcified and noncalcified plaque (NCP) volume and burden. Components of NCP were differentiated by plaque hounsfield unit (HU) thresholds which defined high-risk necrotic core (-30 to 30HU), fibrofatty plaque (31 to 130HU); and low-risk fibrous plaque (131 to 350HU). Composition of NCP components was derived as (NCP component volume/total NCP volume) x 100%. Segment Involvement Score, percent diameter and area stenosis were comparable in both groups. Similarly, there was no difference in the volume and burden of total, calcified and NCP. Compared with Caucasians, East Asians demonstrated lower composition of plaque attenuation corresponding to necrotic core (3.5 vs 5.1%; p = 0.004) and fibrofatty plaque (29.6 vs 37.3%; p = 0.005), and higher fibrous plaque (65.7 vs 57.6%; p = 0.004). On multivariable analysis East Asian ethnicity was independently associated with lower composition of high-risk plaque after adjustment for risk factors and scan parameters. These findings were consistent in a propensity-matched sensitivity-analysis. In conclusion, based on this matched cohort, East Asian ethnicity is associated with significantly less composition of high-risk NCP (necrotic core and fibrofatty plaque) and a higher composition of low-risk fibrous plaque compared with Caucasians; which may confer a lower risk of cardiovascular events. Coronary artery disease in East and South Asians: differences observed on cardiac CT

Summary: Epidemiological studies have found differences in the development of coronary artery disease between East Asians and Caucasians, with East Asians exhibiting less calcification and high-risk plaque. South Asians, on the other hand, show higher rates of coronary calcification and stenosis. Future research may clarify the prognostic value of cardiac CT in South Asians and guide preventative therapy for coronary artery disease in both East and South Asians.

Summary: This study investigates the incremental value of Delta FFRCT in predicting early revascularization and improving efficiency of catheter laboratory utilization in patients with stable coronary artery disease. The results show that AFFRCT improves the discrimination of patients who underwent early revascularization compared to a standard diagnostic strategy of CCTA with FFRCT. AFFRCT has the potential to aid decision-making for ICA referral and improve efficiency of catheter laboratory utilization.

Summary: Computed tomography coronary angiography (CTCA) is an effective non-invasive diagnostic method that provides a comprehensive assessment of coronary arteries and atherosclerosis. It has been increasingly used to evaluate both stable and acute chest pain, and recent guidelines support its use as a first-line diagnostic strategy. CTCA offers advantages over functional tests in detecting coronary artery disease and guiding preventive therapies. Further studies are needed to evaluate the feasibility and cost-effectiveness of CTCA for chest pain evaluation in Australia.

However, integrating FFRCT into clinical practice remains a challenge due to various factors affecting FFRCT calculation and interpretation.

Summary: Patients with ST-elevation myocardial infarction (STEMI) without standard modifiable risk factors (SMURFS) have higher short-term mortality. The association between SMURFS and mortality in younger patients is unclear. A retrospective cohort study of young patients aged 18 to 45 years with STEMI was conducted. SMURF-less patients were more likely to have cardiac arrest, require vasopressors, mechanical support, intensive care admission, and experience left anterior descending artery infarcts. Higher mortality rates were observed in SMURF-less patients at 30 days, 1 year, and 2 years. These findings emphasize the need for improved prevention and management of SMURF-less STEMI.

The results show that using CTCA in rural centers can avert nearly half of the referrals and is a cost-effective strategy for risk stratification of patients with suspected CAD.

Can GRACE Risk Score Predict Mortality and the Need for Thrombolytic Treatment in Acute Pulmonary Embolism?

Coronary Angiography and Chronic Total Occlusion Angioplasty in a Patient With Situs Inversus from the Right Radial Approach

Pericardial Adipose Tissue Thrombospondin-1 Associates With Antiangiogenesis in Ischemic Heart Disease

Summary: This study evaluated the angiogenic capacity of adipose tissue in patients undergoing cardiac surgeries and found that patients with coronary artery disease (CAD) had reduced angiogenic capacity in their pericardial adipose tissue. Gene expression analysis revealed upregulated expression of the anti-angiogenic gene TSP-1 in CAD patients, with no significant differences in other angiogenic factors. Additionally, inhibiting TSP-1 expression significantly improved angiogenic deficiency in CAD patients.

Evaluation of MADIT-II Risk Stratification Score Among Nationwide Registry of Heart Failure Patients With Primary Prevention Implantable Cardiac Defibrillators or Resynchronization Therapy Devices

This study evaluated the feasibility of using the MADIT-II-based Risk Stratification Score (MRSS) to predict the survival benefit of prophylactic ICDs in patients with HF. The results showed that different risk subgroups had varying levels of ICD survival benefit.
