Does chlorhexidine mouthwash, with an anti-discoloration system, reduce tooth surface discoloration without losing its efficacy? A systematic review and meta-analysis

Objectives To investigate whether chlorhexidine mouthwash (CHX-MW), with an anti-discoloration system(ADS), is effective in preventing extrinsic tooth surface discoloration. Additionally, this paper seeks to evaluate whether CHX combined with an ADS maintains its efficacy with respect to reducing plaque and gingivitis scores. Material and methods MEDLINE-PubMed and Cochrane-Central were searched up to October 2018 to identify eligible studies. Papers evaluating the effect of CHX-MW+ADS compared to CHX without an ADS were included. A descriptive analysis and when feasible a meta-analysis was performed. Results Screening resulted in 13 eligible publications, presenting 16 comparisons. Six of these evaluated the MW in a non-brushing model and ten as an adjunct to toothbrushing. A descriptive analysis demonstrated that the majority showed no differences in bleeding, gingivitis and plaque scores. This was confirmed by the meta-analysis. In non-brushing experiments, the difference-of-means (DiffM) for plaque scores was 0.10 (P = 0.45, 95%CI: [-0.15; 0.34]) and for the gingival index 0.04 (P = 0.15,95%CI: [-0.02; 0.11]). The DiffM in brushing studies for plaque scores was 0.01 (P = 0.29, 95%CI: [-0.01; 0.02]) and for the gingival index 0.00 (P = 0.87,95%CI: [-0.05; 0.06]). With respect to staining scores, the meta-analysis revealed that in non-brushing studies, the standardized mean difference was 3.19 (P = 0.0005,95%CI: [-3.98; -1.41]) while in brushing studies, the DiffM was 0.12 (P = 0.95,95%CI: [-3.32; 3.55]). Conclusion There is moderate quality evidence from non-brushing studies that the addition of an ADS to CHX-MW reduces tooth surface discoloration and does not appear to affect its properties with respect to gingival inflammation and plaque scores. In brushing studies, there is also moderate quality evidence that ADS does not affect the anti-plaque and anti-gingivitis efficacy of CHX. The majority of comparisons and the meta-analysis including these indicate no significant effect of ADS on tooth staining in situations where the mouthwash is used in addition to toothbrushing.

A perspective on dental activity during COVID-19: The Italian survey

Summary: During the COVID-19 epidemic in Italy in March and April 2020, dental activities were significantly reduced and limited to urgent treatments. A survey of dental professionals found that a majority of dentists used additional personal protective equipment, although some reported difficulty in obtaining necessary equipment. Following the peak of the epidemic, it is expected that there will be significant changes in dental activities before full restart.

Periodontitis in Italian patients with established rheumatoid arthritis: A cross-sectional study

Impact of rehabilitation versus edentulism on systemic health and quality of life in patients affected by periodontitis: A systematic review and meta-analysis

Summary: The study investigates the effect of rehabilitation on oral-health-related quality of life and systemic health in edentulous patients with stage IV periodontitis. The results show that oral rehabilitation can significantly improve OHRQoL in patients with periodontal diseases, with potential effects on systemic health. However, further research is needed to determine if these findings apply to patients with severe periodontitis.

Treatment of gingivitis is associated with reduction of systemic inflammation and improvement of oral health-related quality of life: A randomized clinical trial

Summary: This study aimed to compare the effects of introducing an electric toothbrush and intensive oral hygiene maneuver adaptation (OHI) versus routine habits (no-OHI) on the level of inflammatory markers, oral health-related quality of life (OHRQoL), and gingival parameters in patients with generalized gingivitis. The results showed that after 1 month, the OHI group had a significant reduction in gingival inflammation and plaque levels, as well as improvements in systemic inflammatory markers and OHRQoL compared to the no-OHI group.

Acute-phase response following one-stage full-mouth versus quadrant non-surgical periodontal treatment in subjects with comorbid type 2 diabetes: A randomized clinical trial

Summary: This study aimed to compare the levels of inflammatory markers and endothelial function in type 2 diabetes mellitus patients after Q-SRP and FM-SRP treatments. The results showed that FM-SRP led to a significant increase in CRP and a decrease in FMD at Day 1 compared to Q-SRP. The improvement in HbA1c levels at Day 90 was significantly better in the Q-SRP group. Therefore, FM-SRP may have adverse effects on endothelial function and HbA1c levels in diabetes patients.

Leisure-time and occupational physical activity demonstrate divergent associations with periodontitis: A population-based study

Summary: This study evaluated the association between leisure-time/occupational physical activity (LTPA/OPA) and periodontitis in a nationally representative sample of the U.S. population. The results showed that high-intensity LTPA was a protective indicator for periodontitis, while high-intensity OPA was a significant risk indicator. The combination of low-intensity LTPA and high-intensity OPA had a cumulative independent association with periodontitis.

Summary: To improve evidence-based implant dentistry and quality of care, this study aimed to develop a core outcome set and measurements for implant dentistry clinical trials. Through systematic reviews, patient focus groups, and expert discussions, 13 mandatory outcome domains were identified, including pathophysiology, implant/prosthesis lifespan, life impact, and access to care. These domains captured both the benefits and harms of therapy and will contribute to improving evidence-informed implant dentistry and quality of care.

Efficacy of adjunctive measures in peri-implant mucositis. A systematic review and meta-analysis

Summary: The study found that in systemically healthy humans with peri-implant mucositis, patient-performed or administered adjunctive measures can significantly reduce bleeding on probing when combined with submarginal instrumentation. The greatest reductions in bleeding were achieved with antiseptics, followed by probiotics and systemic antibiotics. However, complete disease resolution was rarely reported.

The results identified 22 core outcomes in four domains, including pathophysiology, implant/prosthesis lifespan, life impact, and access to care. This study is of significant importance in improving implant dentistry and quality of care.

Healthy lifestyles are associated with a better response to periodontal therapy: A prospective cohort study

Summary: The aim of this study was to evaluate the association between lifestyle behaviors and clinical periodontal outcomes after Steps 1/2 of periodontal therapy. 120 subjects with untreated Stage II/III periodontitis participated in the study. The results showed that poor sleep quality, smoking, and excessive alcohol use were associated with a lower chance of achieving the endpoint of therapy. In addition, subjects with unhealthy lifestyles had higher proportions of residual deep periodontal pockets and lower odds of achieving the endpoint of therapy at re-evaluation.

Panoramic Radiography Features of Medication-Related Osteonecrosis of the Jaws (MRONJ)

Summary: This study retrospectively evaluated the radiographic signs of MRONJ on panoramic radiography to describe the characteristics of the lesions at different stages of the disease. A weak relationship between radiographic signs and disease stage was observed, with mandibular canal enhancement being associated with the advanced disease stage. The distribution of the different radiographic features was not influenced by the underlying disease treated with antiresorptive drugs. Panoramic radiography appears to be a valid support in the assessment of MRONJ lesions.
