Roles of Hepatitis B Virus Mutations in the Viral Reactivation after Immunosuppression Therapies

Reactivation of hepatitis B virus (HBV) is a major problem in patients receiving chemotherapy for malignant diseases or immunosuppression therapies. It has been thought that a reduction in the immune responses might result in the reactivation of HBV replication from covalently closed circular DNA (cccDNA) residing in hepatocytes. However, not only the host's immune status, but also viral mutations have been reported to be associated with reactivation. Especially, several case reports about amino acid mutations in hepatitis B surface antigen (HBsAg) that escape from immune reactions have been reported, and recent reports showed that the frequencies of such mutations are higher than previously expected. In this review, we summarize the characteristics of viral mutations, including immune escape mutations in HBV-reactivated patients, and discuss their significance.

Summary: This study found that an imbalance of free amino acids can exacerbate hepatic steatosis by interfering with the oxidative stress sensor system, involving Nrf2 and VLDL-related lipid metabolism. Results suggest that modulation of the Keap1-Nrf2 system can improve liver damage caused by this imbalance.

Summary: This study aimed to clarify the risk factors associated with the development of hepatocellular carcinoma (HCC) during nucleos(t)ide analogues (NAs) therapies for chronic hepatitis B virus (HBV) infections. It was found that non-achievement of alanine aminotransferase (ALT) normalization and a high aMAP risk score at the start of treatment were significantly associated with an increased risk of HCC.

Trends in drug prescriptions for type 2 diabetes, hypertension, and dyslipidemia among adults with non-alcoholic fatty liver disease

Summary: Patients with comorbid non-alcoholic fatty liver disease (NAFLD) and/or non-alcoholic steatohepatitis (NASH) tend to be prescribed specific drugs for type 2 diabetes, hypertension, and dyslipidemia. However, the differences in prescription ratios are not considerable, and further investigation is needed to confirm the effects of these drugs on the prognosis of patients with NAFLD or NASH.

Summary: This study aimed to examine the association between liver steatosis and fibrosis and atherosclerosis and chronic kidney disease. The results showed that a higher fatty liver index was associated with chronic kidney disease, independent of other risk factors. Further research is needed to understand the causal relationship between liver fat accumulation and chronic kidney disease.

Duodenal Variceal Rupture during Atezolizumab and Bevacizumab Treatment for Hepatocellular Carcinoma

Summary: We report a rare case of duodenal varices rupture in an 87-year-old man during chemotherapy with atezolizumab and bevacizumab (ATZ/BV) for hepatocellular carcinoma. Emergency esophagogastroduodenoscopy (EGD) revealed massive bleeding of a ruptured varix in the horizontal portion of the duodenum. Successful hemostasis was achieved through endoscopic injection sclerotherapy. Although esophageal varices rupture has been reported with ATZ/BV treatment, this is the first case of duodenal varices rupture. Regular evaluation of both duodenal and esophageal varices is recommended before ATZ/BV treatment.

Summary: A novel liquid chromatography-electrospray ionization tandem mass spectrometry method was developed to accurately measure lenvatinib in human plasma, and it demonstrated good reproducibility. Therefore, MSW combined with this new detection method could serve as a useful microsampling device for lenvatinib therapeutic drug monitoring in patients with HCC.

Switching to tenofovir alafenamide versus continued therapy in chronic hepatitis B patients who were treated with entecavir: A prospective, multicenter, randomized controlled study

Summary: This study aimed to evaluate the efficacy and safety of switching from ETV to TAF. The results showed no significant differences in the mean change of HBsAg levels after 2 years between the TAF-switching group and the ETV-continuing group. However, the prior ETV duration was significantly shorter in the HBsAg-declined group. Although the eGFR levels tended to decrease in the TAF group, no significant differences were observed in patients with baseline eGFR < 60.
