Summary: This study aimed to assess the drug susceptibility of clinical isolates of Neisseria gonorrhoeae to spectinomycin, ceftriaxone and azithromycin. The study also examined the temporal trends in the minimum inhibitory concentration (MIC) of five antibiotics from Zhejiang, China. The results showed a high prevalence of resistance to ciprofloxacin and penicillin, while azithromycin resistance rate was above 5%. Ceftriaxone and spectinomycin are recommended for the treatment of Neisseria gonorrhoeae in Zhejiang based on this study.

Summary: This study evaluated the effect of alternative growth supplements on the antibiotic susceptibility testing of Neisseria gonorrhoeae. The results showed that sheep blood can be considered as an alternative material for antibiotic susceptibility surveillance, while hemoglobin may not be suitable as a supplement.

Summary: This study retrospectively compared the performance of gradient diffusion to agar dilution for phenotypic antimicrobial susceptibility testing of Neisseria gonorrhoeae. The results showed that gradient diffusion performed well for cefixime, ceftriaxone, and ciprofloxacin compared to agar dilution. Genome sequencing was identified as a useful tool to resolve discrepancies between the two methods.

Summary: This study evaluated the clinical performance of the GeneXpert (Xpert) CT/NG assay for the detection of Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (NG) in Chinese patients. The results showed that the Xpert test had high sensitivity and specificity for the detection of CT and NG in urine and cervical samples. The quick turnaround time of this test may enable prompt treatment for patients.

Summary: The aim of this study was to analyze genetic resistance determinants and study the resistance to macrolides (azithromycin) in the modern Russian population of N. gonorrhoeae. Although azithromycin is not used to treat gonococcal infection in Russia, 13.6% of the 162 collected isolates in 2020-2021 exhibited phenotypic resistance to azithromycin. Mutations in 23S rRNA genes were only found in two isolates, while the erm and mefA genes were absent. Azithromycin resistance was mainly associated with mutations in the mtrR and mtrD genes of the MtrCDE efflux pump, as well as mosaic alleles formed due to horizontal transfer from N. meningitidis. A total of 30 types of mtrR alleles and 10 types of mtrD alleles, including mosaic variants, were identified. The matching between mtrR and mtrD alleles indicated cooperative molecular evolution of these genes. The association between mtrR and mtrD alleles and NG-MAST types was only found in NG-MAST 228 and 807, which are typical of N. gonorrhoeae in Russia. The high level of azithromycin resistance in Russia may be related to the spread of multiple transferable resistance to antimicrobials, regardless of their use in the treatment of gonococcal infection.

Summary: This study isolated a novel Streptomyces sp. strain EMB24, showing its potential as an excellent source of antimicrobial compounds. The strain exhibited antibacterial activity against various pathogenic bacteria, including resistant strains. Whole genome sequencing and bioinformatics analysis provided insights into the genome and biosynthetic gene clusters producing antibiotics for this strain.

Summary: Research using nucleic acid amplification tests (NAATs) has shown that rectal and oropharyngeal infections with Chlamydia trachomatis and Neisseria gonorrhoeae are common and potentially harder to treat than genital infections. This study addresses the lack of FDA-cleared NAATs for anorectal or oropharyngeal samples by forming a partnership between academic institutions and diagnostic manufacturers. The study found that these assays offer better ability to detect extragenital infections than culture and provide potential solutions for appropriate sexual health care.

Summary: Fosfomycin has regained importance as an antibiotic due to the rise of bacteria resistant to traditional antibiotics. It is effective against ESBL-producing bacteria, especially in infections caused by carbapenem and colistin resistant bacteria. Different methods for fosfomycin susceptibility testing showed varying levels of agreement and error rates, with agar dilution being the most reliable method for Klebsiella pneumoniae isolates.
