Summary: Early non-invasive detection and prediction of graft function after kidney transplantation is important. The study analyzed four urinary biomarkers and found that KIM-1 and NAG at early timepoints after transplantation were significant predictors for eGFR, while NGAL and NAG at day 1 after transplantation were negative predictors. Including these biomarkers improved the prediction models for graft outcome. Factors such as timing of measurement and transplantation factors need to be considered when using urinary biomarkers.

However, most studies have methodological problems, calling for further high-quality research to evaluate their impact.

The development and usage of robust, validated risk communication tools are necessary to improve shared decision-making in pediatric surgery.

Summary: Magnetic resonance-guided laser interstitial thermal therapy (MRgLITT) is a popular minimally invasive alternative for drug-resistant epilepsy (DRE). This study identified independent predictors of seizure outcomes and complications following MRgLITT for DRE. The results showed that MRgLITT is particularly effective in treating patients with well-circumscribed lesional DRE, such as cerebral cavernous malformation (CCM) and mesial temporal sclerosis/atrophy (MTS/A).

Summary: Precision medicine requires clinicians to act as both patient advocates and resource allocators, creating tensions with current allocation rules and justice-based factors. Recent advancements in eplet matching offer promise for improving solid organ transplant outcomes, but may exacerbate ethnic/racial disparities. The rule of rescue, while ethically important, can hinder optimal allocation strategies and the use of scarce transplant resources. Eplet-linked allocation schemes need careful design and clear guidelines to complement existing conventions and maintain public trust.
