Objectives To analyse the influence of geolocation and ethnicity on the clinical presentation of primary Sjgren's syndrome (SjS) at diagnosis. Methods The Big Data Sjogren Project Consortium is an international, multicentre registry designed in 2014. By January 2016, 20 centres from five continents were participating. Multivariable logistic regression analyses were performed. Results We included 7748 women (93%) and 562 men (7%), with a mean age at diagnosis of primary SjS of 53 years. Ethnicity data were available for 7884 patients (95%): 6174 patients (78%) were white, 1066 patients (14%) were Asian, 393 patients (5%) were Hispanic, 104 patients (1%) were black/African-American and 147 patients (2%) were of other ethnicities. SjS was diagnosed a mean of 7 years earlier in black/African-American compared with white patients; the female-to-male ratio was highest in Asian patients (27: 1) and lowest in black/African-American patients (7: 1); the prevalence of sicca symptoms was lowest in Asian patients; a higher frequency of positive salivary biopsy was found in Hispanic and white patients. A north-south gradient was found with respect to a lower frequency of ocular involvement in northern countries for dry eyes and abnormal ocular tests in Europe (OR 0.46 and 0.44, respectively) and Asia (OR 0.18 and 0.49, respectively) compared with southern countries. Higher frequencies of antinuclear antibodies (ANAs) were reported in northern countries in America (OR=1.48) and Asia (OR=3.80) while, in Europe, northern countries had lowest frequencies of ANAs (OR=0.67) and Ro/La (OR=0.69). Conclusions This study provides the first evidence of a strong influence of geolocation and ethnicity on the phenotype of primary SjS at diagnosis.

Remission and Low Disease Activity in Granulomatosis With Polyangiitis and Microscopic Polyangiitis: Prevalence and Impact on Damage Accrual

Paolo Delvino, Federica Sardanelli, Sara Monti, Pascal Cohen, Xavier Puechal, Luc Mouthon, Carlomaurizio Montecucco, Loic Guillevin, Benjamin Terrier

Summary: This study assessed the prevalence and impact of different levels of disease activity on damage accrual in patients with GPA and MPA. The results showed that more than 60% of patients achieved prolonged remission, which was associated with better long-term outcomes.

Summary: This study aimed to investigate the prevalence and characteristics of late skin fibrosis in systemic sclerosis (SSc). The study found that late skin fibrosis is not uncommon in SSc, and different patterns of development exist among patients. Patients with lower baseline mRSS and lcSSc are more likely to develop late skin fibrosis. Anti-Scl-70 is associated with progression from lcSSc to dcSSc, while anticentromere antibodies have a protective effect.

Temporal Series Analysis of Population Cycle Threshold Counts as a Predictor of Surge in Cases and Hospitalizations during the SARS-CoV-2 Pandemic

Summary: Prediction tools can help guide public health decisions by forecasting COVID-19 case surges and hospitalizations. Low cycle threshold (CT) counts can indicate a higher concentration of SARS-CoV-2 in the respiratory tract, serving as a marker for increased viral transmission. This study validated CT counts as an epidemiological marker for predicting pandemic waves, showing a temporal association between CT counts and COVID-19 cases and hospital admissions.

Summary: This study compared the severity of pulmonary embolism (PE) and the long-term complications between COVID-19 patients and non-COVID-19 patients, and investigated the validity of risk stratification tools for PE in this population. The results showed that COVID-19 patients had fewer traditional risk factors for PE and a lower incidence of deep vein thrombosis. COVID-19 patients also had smaller PE size, less right ventricular dysfunction, and lower incidence of long-term complications.

Summary: This study investigated whether RMD patients receiving anti-IL6 therapy have less severe forms of COVID-19 and better outcomes. Retrospective analysis of the French RMD COVID-19 cohort data showed that the severity of COVID-19 was lower in the anti-IL6 group, but there were no significant differences in the evolution of COVID-19 between the groups.

Luis Gonzalez-de Paz, Laura Valdesoiro-Navarrete, Josep Roma, Esther Blat-Guimera, Jaume Benavent-Areu, Joan Bartra, Antoni Siso-Almirall

Summary: This study investigated the prevalence of asthma and allergies in children under 14 years old in Spain and their impact on health-related quality of life, activities, healthcare utilization, and environmental and household exposure risk factors. The results showed that asthma and allergies have a significant impact on children's daily life and healthcare utilization.

Summary: This study investigated the phenotypic characteristics of patients with ANCA-associated vasculitis (AAV) associated with aneurysms. It found that aneurysms were more frequently observed in granulomatosis with polyangiitis (GPA) patients and predominantly affected medium-sized vessels, especially the renal arteries. Aneurysmal rupture occurred in 43% of patients, with those experiencing rupture having higher baseline pulmonary infiltrates and creatinine levels compared to those without rupture.

Multiple Correspondence Analysis and HLA-Associations of Organ Involvement in a Large Cohort of African-American and European-American Patients with Sarcoidosis

Astrid Rasmussen, Bryan A. Dawkins, Chuang Li, Nathan Pezant, Albert M. Levin, Benjamin A. Rybicki, Michael C. Iannuzzi, Courtney G. Montgomery

Summary: Sarcoidosis is a systemic granulomatous disease with heterogeneous manifestations. African Americans have higher morbidity and mortality. Multiple Correspondence Analysis revealed different patterns of organ involvement between African American and European American patients, suggesting ancestry-specific risk profiles. Understanding these profiles will help achieve personalized medicine for this complex disease.

Summary: This retrospective study investigated the use of benralizumab in the treatment of eosinophilic granulomatosis with polyangiitis (EGPA), and found that benralizumab is effective for refractory asthma and ENT manifestations, and reduces the use of glucocorticoids. However, its efficacy is lower in patients who have previously failed mepolizumab treatment.

Summary: This retrospective study analyzed data from a large sample of patients with Wernicke's encephalopathy (WE) to examine the variability in thiamine dosages and routes used in the treatment, as well as the factors associated with this variability. The study found a high level of variability in WE treatment, with a low rate of high-dose intravenous thiamine administration. Factors such as delayed diagnosis, Caine criteria fulfillment, mental status alterations, folic acid deficiency, malnutrition, reduced consciousness, and lack of timely thiamine treatment were associated with poor outcomes.

Summary: This study describes the characteristics of 337 patients seen by the fracture liaison service of the Amiens University Hospital for at least two osteoporotic fractures between 2009 and 2019. Results showed that recurrent fracture occurs rapidly after the index fracture.
