Summary: Stroke in young adults is becoming more common in the United States due to the increasing prevalence of traditional risk factors and non-modifiable risk factors. The unique mechanisms causing stroke in this population include cervical dissection, cardioembolic phenomenon, vasculitis and vasculopathy, connective tissue disease, patent foramen ovale, and cerebral venous thrombosis. Diagnosing stroke in young adults can be challenging because of its diverse clinical presentations. This article provides an overview of the epidemiology, explores the mechanisms, and updates the literature on the management and prevention of stroke in young adults.

Summary: The incidence of ischemic stroke in young adults has been increasing, and illicit drug use, including cannabis, may be a contributing factor. However, the mechanisms and clinical presentation of cannabis-associated ischemic stroke are unclear. This study aimed to describe the characteristics of ischemic stroke in cannabis users compared to nonusers among young adults. The results showed a significant and independent association between cannabis use and atherosclerotic stroke phenotype, particularly with frequent and daily use.

Summary: This study aimed to investigate the short-, medium-, and long-term outcomes of intravenous thrombolysis in minor stroke patients. The results suggest that intravenous thrombolysis may be safe and effective in improving functional outcomes in minor stroke patients within a 4.5-h window. However, further randomized controlled trials are needed for validation. The functional recovery rate in the overall study population ranged from 12.8% to 18.85%.

Among patients without a history of ASCVD but with prevalent CV risk factors, there does not appear to be a difference in MACE risk between tofacitinib 5 mg twice daily and TNFi, although any absolute risk excess is likely low.

Summary: This study investigated the pro-inflammatory effects of air pollution PMs on pulmonary endothelial cell NO bioavailability and protein expression, and whether EPA can restore endothelial cell function. The results showed that EPA can alter protein expression related to inflammatory pathways and reduce the levels of the inflammatory molecule sICAM-1, improving the NO/ONOO- release ratio. These cellular changes may contribute to the anti-inflammatory, cytoprotective, and lipid changes associated with EPA treatment during air pollution exposure.

Summary: This study aimed to investigate the relationship between dual task cost (DTC) and white matter hyperintensities (WMH) in Lewy body disorders. The results showed that increased DTC was associated with higher frontal WMH burden, and increased age was associated with higher DTC. This indicates the importance of the association between DTC and WMH in Lewy body disorders.

Summary: This exploratory study aimed to investigate whether patients with transient ischemic attack (TIA) have measurable regional cerebral hypoperfusion unrelated to the location of ischemia, and to determine the relationship between regional cerebral blood flow (rCBF) and their cognitive profiles. The results showed that TIA patients had significantly lower cognitive scores and regional hypoperfusion in certain brain regions, which may serve as early markers of future cognitive decline.
