Objective. Scleroderma renal crisis (SRC) is a severe life-threatening manifestation in patients with systemic sclerosis (SSc). However, the knowledge about risk factors for SRC is limited. We determined here the frequency of SRC and identified risk factors for the prediction of SRC. Methods. Based on regular followup data from the German Network for Systemic Scleroderma, we used univariate and multivariate generalized estimating equations to analyze the association between clinical variables, SSc subsets, therapy [i.e., angiotensin-converting enzyme inhibitors (ACEi), corticosteroids], and the occurrence of SRC. Results. Data of 2873 patients with 10,425 visits were available for analysis with a mean number of registry visits of 3.6 +/- 2.8 and a mean time of followup of 3.6 +/- 3.8 years. In total, 70 patients developed SRC (70/ 2873, 2.4%). Of these patients, 57.1% (40/70) were diagnosed with diffuse cutaneous SSc, 31.4% (22/70) with limited cutaneous SSc, and 11.4% (8/70) with SSc- overlap syndromes. Predictive independent factors with the highest probability for SRC were positive anti-RNA polymerase antibodies (RNAP), a history of proteinuria prior to SRC onset, diminished DLCO, and a history of hypertension. Interestingly, positive antitopoisomerase autoantibodies did not predict a higher risk for SRC. Further, patients with SRC were significantly more frequently treated with ACEi and corticosteroids without being independently associated with SRC. Conclusion. In this cohort, SRC has become a rare complication. By far the highest risk for SRC was associated with the detection of anti-RNAP and proteinuria.

Summary: Allergen immunotherapy (AIT) involves gradually increasing the dose of allergen extract until immunologic tolerance is achieved. Adherence to AIT is crucial for long-term success, but measuring adherence and persistence rates has been inconsistent. The development of standardized tools and parameters for measuring adherence in AIT is necessary. Additionally, exploring methods from adherence research in other chronic diseases could help improve outcomes in AIT.

Successful Rescue Therapy With Daratumumab in Rapidly Progressive Interstitial Lung Disease Caused by MDA5-Positive Dermatomyositis

Summary: This is a case report of a 19-year-old male patient with rapidly progressive interstitial lung disease that did not respond to intensive immunosuppression. Rescue therapy with daratumumab, an anti-CD38-antibody, resulted in significant pulmonary improvement and stable disease remission after 6 months of follow up.

Handling dataset dependence with model ensembles for skin lesion classification from dermoscopic and clinical images

Summary: Dataset dependence is a major concern in machine learning, especially in small and specific medical databases. We conducted experiments on skin lesion image classification using three medium-sized datasets and found that standard machine learning techniques do not effectively reduce dataset dependence. To tackle this issue, we propose using an ensemble of heterogeneous models and training with multi-source datasets.

Patch testing hydroperoxides of limonene and linalool in consecutive patients-Results of the IVDK 2018-2020

Summary: Positive patch test reactions to Limonene hydroperoxides (Lim-OOHs) and Linalool hydroperoxides (Lin-OOHs) were frequent in consecutive patients, but most of the reactions were weak, doubtful, or irritant in nature. Patients with irritant reactions to sodium lauryl sulphate (SLS) were more likely to react to Lim-OOHs and Lin-OOHs. It is recommended to chemically monitor test preparations to reduce irritancy and interpret patch test results carefully.

How to treat VEXAS syndrome: a systematic review on effectiveness and safety of current treatment strategies

Summary: The effectiveness and safety of current treatment strategies for VEXAS syndrome were evaluated in this study. The results showed that available treatments include the use of certain drugs and stem cell transplantation, but the data is limited and heterogeneous. Treatment decisions should be individualized and clinical trials are needed to develop treatment algorithms. Adverse events, especially the elevated risk of venous thromboembolism associated with JAKi treatment, remain a challenge.

Summary: This study aims to investigate the differential diagnostic spectrum in patients with suspected Behcet's syndrome (BS) in low prevalence regions. The results show that the straightforward application of the ICBD criteria may lead to overdiagnosis of BS in low disease prevalence settings.

Inflammatory rheumatic diseases in migrants

Summary: The immigration of people from different countries poses new challenges for rheumatologists due to the differences in disease frequencies and treatment situations. Diseases like familial Mediterranean fever and Behcet syndrome are rare in western Europe but common in North Africa and Mediterranean countries. Rheumatic fever is also more prevalent in African countries compared to Europe. Furthermore, genetic anemia and infections such as HIV, hepatitis, tuberculosis, and parasitosis are more frequent in the countries of origin of immigrants.

Clinical presentation and genetic variants in patients with autoinflammatory diseases: results from the German GARROD registry

Summary: This study investigated the clinical symptoms and genetic variants of patients with autoimmune orphan diseases who received anti-IL-1 targeted therapy. The results showed that most patients experienced inflammatory attacks before the age of 18, with an average delay of 17.8 years in starting anti-IL-1 therapy. Genetic analysis confirmed the diagnosis in a majority of patients and identified some variants of unknown significance. Additionally, some patients had secondary AA amyloidosis at the initiation of anti-IL-1 therapy. Turkish-Armenian ancestry was found to be associated with higher disease activity in familial Mediterranean fever.

Diagnostic accuracy of a large language model in rheumatology: comparison of physician and ChatGPT-4

Summary: Pre-clinical studies suggest that language models like ChatGPT could be useful in distinguishing inflammatory rheumatic diseases (IRD) from others. In this study, the diagnostic accuracy of ChatGPT-4 was compared to rheumatologists using patient data. ChatGPT-4 provided comparable correct diagnoses to rheumatologists in a significant number of cases, particularly for IRD-positive cases. However, its specificity was lower in non-IRD cases. The results highlight the potential of ChatGPT-4 as a triage tool for diagnosing IRD, with better sensitivity than rheumatologists.
