Coronary Flow Evaluation in Heart Transplant Patients Compared to Healthy Controls Documents the Superiority of Coronary Flow Velocity Reserve Companion as Diagnostic and Prognostic Tool

Annagrazia Cecere, Peter L. M. Kerkhof, Giovanni Civieri, Annalisa Angelini, Antonio Gambino, Angela Fraiese, Tomaso Bottio, Elena Osto, Giulia Famoso, Marny Fedrigo, Enrico Giacomin, Giuseppe Toscano, Roberta Montisci, Sabino Iliceto, Gino Gerosa, Francesco Tona

Summary: Coronary microvascular dysfunction after heart transplantation can be classified into four endotypes, with one type showing higher mortality rate over a 28-year follow-up period. Independent predictors of mortality include CMD, DPVh < 75 cm/s and CCFVR < 80 cm/s.

Cohort profile: the ESC EURObservational Research Programme Non-ST-segment elevation myocardial infraction (NSTEMI) Registry

Ramesh Nadarajah, Peter Ludman, Yolande Appelman, Salvatore Brugaletta, Andrzej Budaj, Hector Bueno, Kurt Huber, Vijay Kunadian, Sergio Leonardi, Maddalena Lettino, Dejan Milasinovic, Chris P. Gale

Summary: The aim of this study is to identify international patterns in the management of NSTEMI and compare them with the 2015 ESC Guidelines. By collecting data from 3620 NSTEMI patients in 287 centers across 59 countries, this study will provide unique insights into the contemporary management of NSTEMI patients.

Machine learning approach in diagnosing Takotsubo cardiomyopathy: The role of the combined evaluation of atrial and ventricular strain, and parametric mapping

Riccardo Cau, Francesco Pisu, Michele Porcu, Filippo Cademartiri, Roberta Montisci, Pierpaolo Bassareo, Giuseppe Muscogiuri, Antonio Amadu, Sandro Sironi, Antonio Esposito, Jasjit S. Suri, Luca Saba

Summary: This study developed a machine learning model incorporating non-contrast CMR parameters to identify patients with Takotsubo cardiomyopathy. Left atrial strain and strain rate were found to be important imaging markers for the identification of these patients.

Coronary Flow Velocity Reserve by Echocardiography: Beyond Atherosclerotic Disease

Giovanni Civieri, Roberta Montisci, Peter L. M. Kerkhof, Sabino Iliceto, Francesco Tona

Summary: Coronary flow velocity reserve (CFVR) is a reliable tool to assess hemodynamic relevance and investigate microvascular dysfunction in various conditions. It has been proven to be an effective prognostic factor for risk stratification. This review focuses on the applications of CFVR by echocardiography, particularly in the coronary involvement of systemic diseases.

Does Patient Compliance Influence Wearable Cardioverter Defibrillator Effectiveness? A Single-Center Experience

Luca Fazzini, Maria Francesca Marchetti, Ferdinando Perra, Mattia Biddau, Nicola Massazza, Vincenzo Nissardi, Elena Agus, Roberta Demelas, Roberta Montisci

Summary: The study aimed to assess patient adherence to wearable cardioverter defibrillator as an indicator of device effectiveness. Patient training in this area is not standardized widely. 25 patients with a wearable cardioverter defibrillator were enrolled, and they were followed-up until the decision to upgrade to an implantable device was made. Results showed that standardized patient training is important for achieving high adherence to instructions and improving device effectiveness.

Catecholamine-induced Takotsubo syndrome: a case series

Nicola Campana, Alessandra Gioi, Maria Francesca Marchetti, Martina Giusti, Simone Angius, Ludovica Caggiari, Mattia Biddau, Roberta Montisci

Summary: This study presents two cases of catecholamine-induced Takotsubo Syndrome (cat-TS), which emphasizes the importance of routine screening for myocardial impairment after catecholamine administration, and the role of prompt echocardiogram in early detection and suitable medical support.

Non-invasive coronary flow velocity reserve assessment predicts adverse outcome in women with unstable angina without obstructive coronary artery stenosis

Roberta Montisci, Maria Francesca Marchetti, Massimo Ruscazio, Mattia Biddau, Sara Secchi, Norma Zedda, Roberto Casula, Francesca Tuveri, Peter L. M. Kerkhof, Luigi Meloni, Francesco Tona

Summary: This study aimed to assess the role of CFVR in predicting long-term cardiovascular events in women with UA without obstructive coronary artery disease. The results showed that abnormal CFVR was associated with lower cardiac event-free survival. During the follow-up, 70% of women with reduced CFVR had cardiac events, compared to only 20% with normal CFVR.

Less air pollution did not explain the decline in admissions for AMI during the first wave of COVID-19 pandemic in Sardinia, Italy

Meloni Luigi, Ranieri Gaetano, Marchetti Maria Francesca, Francesca Aste, Piga Carlo, Casu Gavino, Pes Rosanna, Meloni Ivan, Loi Bruno, Montisci Roberta

Summary: During the COVID-19 pandemic, there was a decrease in hospitalizations for acute myocardial infarction (AMI) worldwide. A significant decline in AMI admissions was observed during the lockdown period compared to before the lockdown, and there was a significant increase after the lockdown. The density of air pollutants did not show a significant change. Fear of contagion is the most probable reason for the decrease in hospitalizations during the lockdown period.

Less air pollution did not explain the decline in admissions for AMI during the first wave of COVID-19 pandemic in Sardinia, Italy

Luigi Meloni, Gaetano Ranieri, Maria Francesca Marchetti, Francesca Aste, Carlo Piga, Gavino Casu, Rosanna Pes, Ivan Meloni, Bruno Loi, Roberta Montisci

Summary: During the COVID-19 pandemic, there was a decrease in hospital admissions for acute myocardial infarction (AMI) worldwide. A study comparing the admissions for AMI during the national lockdown showed a marked decline during the lockdown period compared to the 8 weeks before the lockdown. However, there was a significant increase in admissions for AMI in the 8 weeks after the lockdown. The study suggests that the fear of contagion may be the main reason for the drop in hospitalizations during the lockdown, as there was no substantial change in air pollution levels.
