Comparison of the Effect of 5 Different Treatment Options for Managing Patellar Tendinopathy: A Secondary Analysis

Objective: Currently, no treatments exist for patellar tendinopathy (PT) that guarantee quick and full recovery. Our objective was to assess which treatment option provides the best chance of clinical improvement and to assess the influence of patient and injury characteristics on the clinical effect of these treatments. Design: A secondary analysis was performed on the combined databases of 3 previously performed double-blind randomized controlled trials. Patients: In total, 138 patients with PT were included in the analysis. Interventions: Participants were divided into 5 groups, based on the treatment they received: Extracorporeal shockwave therapy (ESWT) (n = 31), ESWT plus eccentric training (n = 43), eccentric training (n = 17), topical glyceryl trinitrate patch plus eccentric training (n = 16), and placebo treatment (n = 31). Main Outcome Measures: Clinical improvement (increase of >= 13 points on the Victorian Institute of Sport Assessment-Patella score) after 3 months of treatment. Results: Fifty-two patients (37.7%) improved clinically after 3 months of treatment. Odds ratios (ORs) for clinical improvement were significantly higher in the eccentric training group (OR 6.68, P = 0.009) and the ESWT plus eccentric training group (OR 5.42, P = 0.015) compared with the other groups. We found evidence that a high training volume, a longer duration of symptoms, and older age negatively influence a treatment's clinical outcome (trend toward significance). Conclusions: Our study confirmed the importance of exercise, and eccentric training in particular, in the management of PT. The role of ESWT remains uncertain. Further research focusing on the identified prognostic factors is needed to be able to design patient-specific treatment protocols for the management of PT.

Summary: This study systematically reviewed health-economic evaluations of anterior cruciate ligament (ACL) injury and compared the cost-effectiveness of reconstruction surgery (ACLR) and non-operative treatment (NO). The results suggest that early ACLR may be the more cost-effective strategy for athletes and young populations with high-activity levels, while non-operative treatment with optional delayed ACLR may be more cost-effective. However, for middle-aged individuals with moderate activity levels, early ACLR may not be more cost-effective than delayed ACLR.

Clinical relevance of joint line obliquity after high tibial osteotomy for medial knee osteoarthritis remains controversial: a systematic review

Summary: This study aimed to review the literature on the association between knee joint line obliquity (KJLO) and clinical outcome after high tibial osteotomy (HTO) for medial knee osteoarthritis and summarize the KJLO cut-off value used in these studies. The results showed conflicting findings on the associations between postoperative KJLO and patient-reported outcome, medial knee cartilage regeneration, and 10-year surgical survival. The clinical relevance of KJLO after HTO remains controversial.

High-demand tasks show that ACL reconstruction is not the only factor in controlling range of tibial rotation: a preliminary investigation

Summary: The study aimed to determine whether the range of tibial rotation (rTR) increases after ACL injury and whether it returns to normal after ACL reconstruction (ACLR). The results showed that rTR increased during high-demand tasks compared to low-demand tasks. Before ACLR, rTR was smaller in the ACL-deficient knees compared to the contralateral knees. After ACLR, rTR increased compared to pre-operative but was still smaller than the contralateral knees.

Assessment of joint line obliquity and its related frontal deformity using long-standing radiographs

Summary: This study investigated the influence of radiographic techniques and osteoarthritis grade on measurements of knee joint line obliquity (KJLO) and KJLO-related frontal deformity. It was found that measurements of JLOAF, JLOAM, JLOAT, MJLA, JLCA, and HKA were influenced by single-leg/double-leg standing, while JLOAF, JLOAM, and JLOAT were also affected by bipedal distance in double-leg standing, and JLCA was affected by osteoarthritis grade. MPTA measurement of knee joint obliquity was not affected by standing position, bipedal distance, or osteoarthritis grade, and had excellent reliability. Thus, MPTA is proposed as the preferable KJLO measurement method for clinical practice and future research.

Joint line obliquity after lateral closing-wedge high tibial osteotomy does not adversely affect clinical and radiological outcome: a 5-year follow-up study

Summary: This study aimed to analyze the association between change in knee joint line obliquity (KJLO) and patient-reported outcome, radiological progression of osteoarthritis, and surgical survival after lateral closing-wedge high tibial osteotomy (HTO). The results showed that increased KJLO (postoperative MPTA ≥ 95.0°) or MPTA change ≥ 8.0° after HTO did not adversely affect patient-reported outcome, radiological progression of osteoarthritis, or surgical survival at an average 5-year follow-up.

Responsiveness of the Indonesian Versions of the Anterior Cruciate Ligament-Return to Sport After Injury Score, the International Knee Documentation Committee Subjective Knee Form, and the Lysholm Score in Patients With ACL Injury

Summary: The purpose of this study was to determine the responsiveness of ACL-RSI, IKDC, and Lysholm scores in an Indonesian-speaking population with ACL injury. The results showed that all three scores had good responsiveness and can be used for follow-up of patients after ACL injury. At the individual level, IKDC was found to be more efficient than ACL-RSI or Lysholm scores in detecting clinically important changes over time after ACL treatment.
