Hypoxic Burden in Obstructive Sleep Apnea: Present and Future

Summary: Conventional measures of OSA severity, such as AHI and ODI, have limitations in capturing the severity of OSA and its physiological consequences. The sleep apnea-specific hypoxic burden (HB), which measures the sum of individual areas under the oxygen desaturation curve, shows promise in identifying high-risk individuals with OSA. HB captures the depth and duration of OSA-related hypoxemia, providing important disease characterizing features not captured by conventional metrics.

Summary: Bronchiectasis is a complex and heterogeneous disease with various causes. It is characterized by neutrophilic inflammation but can also involve eosinophilic inflammation. Eosinophilic diseases, such as asthma and allergic bronchopulmonary aspergillosis, may coexist with bronchiectasis. Additionally, some bronchiectasis patients exhibit eosinophilia without identified concomitant diseases, and the role of eosinophils in this condition is not fully understood.

Summary: This study found a significant U-shaped relationship between BEC and severity and exacerbations in bronchiectasis, which was more prominent in the eosinopenic group. IC treatment only decreased the number and severity of exacerbations in the eosinophilic group.

Efficacy and Safety of Dry Powder Antibiotics: A Narrative Review

Summary: The use of inhaled antibiotics has expanded beyond cystic fibrosis patients to include non-CF bronchiectasis or chronic obstructive pulmonary disease patients. Inhaled antibiotics provide high concentrations at the site of infection, enhancing their effectiveness and allowing for long-term administration. New formulations of inhaled dry powder antibiotics offer advantages such as faster preparation and administration, as well as eliminating the need for cleaning nebulization equipment.
