Summary: This study evaluated the outcomes of heart retransplantation and found that the long-term survival rate after retransplantation was lower than after primary heart transplantation. Although the short-term outcomes were similar between the two groups, heart retransplantation should still be considered for select patients to optimize donor organ usage.

Summary: Despite the increase in living donor liver transplants, the outcomes of retransplantation after living donor liver transplant need further investigation. This study compared the waitlist outcomes and survival post-retransplant in recipients of initial living or deceased donor grafts. The results showed similar waitlist mortality and survival rates for both types of transplants.

Summary: Normothermic machine perfusion (NMP) is an effective tool for preserving liver allografts before transplantation. The University of Alberta's liver transplant program has successfully utilized NMP to extend preservation time in liver transplantation. NMP livers showed improved 30-day graft survival and no increase in complications compared with livers preserved using static cold storage (SCS).

Summary: This study assessed long-term weight outcomes after bariatric surgery in Saudi Arabia, finding significant weight reductions in patients at 1 and 3 years, with no substantial difference between sleeve gastrectomy (SG) and Roux-en Y gastric bypass (RYGB). However, weight loss rates decreased and weight regain increased beyond 3 years post-surgery, highlighting the challenge of maintaining weight loss in the long-term.

Summary: This study evaluated the surgical trends and techniques of robotic gastrectomy and analyzed the surgical outcomes of 2000 consecutive patients with gastric cancer who underwent robotic gastrectomy over 14 years. The results showed that robotic gastrectomy is safe and feasible with acceptable short- and long-term outcomes, and the proportion of robotic gastrectomies and technically demanding procedures have tended to increase over time.

Summary: This study analyzed the impact of pancreatic resection on the short- and long-term outcomes of patients with retroperitoneal sarcoma. The study found that patients who underwent pancreatic resection had higher morbidity and mortality, but minimal influence on survival. There were slight differences in safety and survival rates among patients who underwent different types of pancreatic resection.

Summary: This study retrospectively analyzed the effects and long-term survival rate of balloon atrial septostomy (BAS) in 12 patients with refractory right heart failure (RHF) from idiopathic pulmonary arterial hypertension (IPAH). The results showed that BAS improved hemodynamic variables and had clinical and cardiac functional benefits at 1-year follow-up. Although two patients died due to malignant arrhythmia and advanced heart failure, the overall survival rates at 1 and 3 years were 100% and 83.3% respectively.

Summary: This study assessed the survival experience of long-term peritoneal dialysis (PD) patients in Mexico and found that older age at PD initiation and the use of continuous ambulatory peritoneal dialysis were associated with an increased risk of mortality. This highlights the need to optimize treatment strategies to enhance long-term prognosis for patients with end-stage kidney disease.

Summary: Between 2015 and 2019, seven patients were diagnosed with M. chimaera infection following cardiac surgery. All patients underwent redo cardiac surgery and explantation of previously implanted prosthetic material. Three patients experienced infection relapse during follow-up, indicating the importance of addressing extracardiac infection and avoiding re-implantation of prosthetic material.

Summary: The Oncology Grand Rounds series aims to help doctors better apply clinical research to actual patient treatment. This particular round focused on the management and complications of hematopoietic stem-cell transplantation (HSCT) in acute myelogenous leukemia (AML). The study found that while transplantation therapy provides long-term survival opportunities, it also increases the risk of late complications.

Summary: This study retrospectively reviewed data from 112 patients with pulmonary sarcoidosis who underwent lung or heart-lung transplantation, finding that post-transplant survival rates were similar to those with other indications for lung transplantation. Older age and extensive pre-operative lung fibrosis were the main factors associated with worse survival.

Summary: This study aimed to evaluate the survival rate and prognostic factors in lung transplant recipients with interstitial lung disease complicating idiopathic inflammatory myopathy (IIM-ILD). The study found that the post-transplant survival rate in IIM-ILD was similar to that in international all-cause transplantation registries and worse in patients with classical IIM. Muscle involvement was identified as the main factor associated with worse survival.

Summary: This study evaluated the one-year incidence of fungal tracheobronchitis after lung transplantation and assessed the utility of diagnostic markers in serum and BALF to discriminate fungal tracheobronchitis from colonization. The results showed that the one-year cumulative incidence of Candida and Aspergillus tracheobronchitis after lung transplantation was 23% and 16% respectively. Diagnostic markers in BALF, such as neutrophils and β-glucan/galactomannan, were found to be useful in distinguishing between fungal colonization and tracheobronchitis.
