Summary: Intersphincteric resection (ISR) for low-lying rectal cancer aims to avoid abdominoperineal resection (APR). This study compared open and minimally invasive ISR, finding similar 5-year disease-free survival rates. Minimally invasive ISR showed advantages in postoperative outcomes like reduced surgical site infection and shorter hospital stay.

Summary: This study evaluated the heterogeneity in the safety and quality of ileoanal J-pouch surgery videos published online by peer-reviewed surgical journals. The blind assessment of these videos reported a high rate of unsafe or inappropriate technique for rectal mobilization and transection, with fair interobserver agreement among reviewers. Peer review of videos could facilitate training and accreditation in this complex procedure.

Summary: This study compares the outcomes of straight (end-to-end) anastomosis and colonic J-pouch anastomosis after anterior resection. The results show that J-pouch anastomosis is associated with lower rates of anastomotic leak and re-operation. Additionally, the J-pouch group has better functional outcomes, such as lower stool frequency.

Summary: Total abdominal proctocolectomy with ileal pouch-anal anastomosis (IPAA) is a common treatment for ulcerative colitis (UC), but it can lead to complications such as pouchitis and pouch disorders. Pouchitis can usually be treated with antibiotics, but chronic antibiotic refractory pouchitis (CARP) may require biologic therapies. Crohn's-like disease of the pouch (CLDP) is a less common complication that can be treated similarly to CARP. Standardized diagnostic criteria for inflammatory pouch disorders can help guide future therapeutic options. Structural pouch disorders are often related to surgical complications, and their management may involve endoscopic interventions and surgical procedures.

Summary: This study developed a new classification system for pouchitis based on the pattern of inflammation observed in pouches. It identified 7 different phenotypes and evaluated the contributing factors and prognosis of each phenotype. The study provides valuable insights for pouch management and future interventional studies.

Summary: Traditional end-to-end coloanal anastomosis often fails to achieve physiological control and defecation function, leading to functional issues like LARS; such as fecal incontinence, urge problems, and high stool frequency. Studies show that side-to-end anastomosis, coloplasty pouch, and colonic J-pouch are superior to end-to-end anastomosis in terms of functional outcomes.

Summary: Management of rectovaginal fistula (RVF) remains a challenge, especially in cases of postoperative RVF due to the surrounding inflammatory and fibrotic tissue. This study assessed the results of delayed coloanal anastomosis (DCAA) for RVF after rectal surgery, showing an 86% success rate at the end of follow-up. The use of temporary stoma with DCAA may slightly increase the success rate.

Summary: This study evaluated the incidence and risk factors for low anterior resection syndrome (LARS) following surgery for rectal cancer. The study found that the distance of the anastomosis from the anal verge, total proctectomy with total mesorectal excision (TME), and neoadjuvant radiotherapy were independent risk factors for LARS. The incidence and severity of LARS decreased with longer follow-up.

Summary: This study investigated the efficacy of reduced starting dose of first-line eribulin in patients aged >= 70 years with metastatic breast cancer. The results showed that the disease control rate was 40% with the reduced dose, and there was no cumulative neurotoxicity. Therefore, the study does not support the approach of a reduced starting dose in older patients.

Summary: This study aimed to assess the prognostic value of microscopically positive margins (R1) after gastrectomy on survival and tumor recurrence in diffuse-type gastric cancer (DTGC) patients. It showed that patients with R1 had poorer overall survival and disease-free survival, as well as higher early tumor recurrence rates. Therefore, achieving R0 resection should be pursued whenever possible.

Summary: Whole-organ pancreas and islets transplantations are therapeutic options for type 1 diabetic patients with severe complications. The choice of treatment depends on factors such as kidney function, comorbidities, and treatment goals. Simultaneous pancreas and kidney transplant is often recommended for patients with end-stage kidney disease, but careful selection is necessary. New strategies, including dynamic organ perfusion technologies and stem cell therapies, aim to improve outcomes and reduce immune reactions. This article reviews the indications, surgical techniques, outcomes, and future directions of these transplantations.

Summary: Despite recent therapeutical advances, gastric adenocarcinoma remains associated with a poor long-term survival. Diagnosis is often made at advanced stages, affecting prognosis. Factors such as tumor microenvironment, patient ethnicity, and therapeutic strategy play a role in patient outcome. Summary: In metastatic breast cancer, combining trastuzumab and pertuzumab with taxane-based chemotherapy is the standard of care. It is unclear if trastuzumab plus pertuzumab alone can be an optimal first-line strategy. This secondary analysis of a clinical trial suggests that the chemotherapy-free anti-ERBB2 strategy is feasible without compromising overall survival.

Summary: Pancreatic cancer is a deadly disease with unknown origins and limited treatment options. Surgical resection, the primary curative treatment, is associated with high morbidity and mortality rates and frequent recurrence. Patient-centered care and understanding patient perspectives are crucial for improving outcomes in pancreatic cancer surgery. Standardized patient-reported outcome and experience measures are needed to assess patient perspectives accurately.

Summary: This study evaluated the impact of neoadjuvant treatment on PD-L1 expression in patients with stage III non-small-cell lung cancer. The results showed that neoadjuvant treatment did not influence PD-L1 expression, regardless of the specific treatment protocol. Additionally, the dynamic change of PD-L1 expression did not correlate with event-free survival or overall survival.
