Summary: In left-sided minimally-invasive colectomy, intracorporeal anastomosis is associated with lower overall morbidity and s fewer rates of surgical site infection compared to extracorporeal anastomosis. It also results in a smaller incision length and faster postoperative gastrointestinal recovery, despite a longer operative time.

Summary: Benign anastomotic strictures following colorectal surgical resection are a common but under-reported complication, affecting up to 30% of patients and significantly impacting their quality of life. Endoscopic techniques, such as balloon dilatation and stenting, show varying success rates in avoiding the need for surgical reintervention, with challenges including the need for repeated treatments and stent migration. Further research is needed to explore novel therapies for improving quality of life in these patients.

Summary: This study presents the transanal end-to-end rectoanal anastomosis as a surgical technique for the treatment of rectal atresia. The results showed normal continence and regular bowel movements in the patients who underwent this procedure. It was concluded that transanal recto-anal anastomosis provides a safer and effective approach for anatomical reconstruction of the anorectum.

Summary: This study investigated the outcomes for patients with anal squamous cell carcinoma (SCC) who underwent pelvic exenteration in two high-volume, high-complexity units in the UK. The results showed that complete R0 resection is possible in the majority of patients with recurrent or persistent anal SCC, and it improves overall and disease-free survival compared to R1/R2 resection.

Summary: The ileo-anal pouch (IAP) has a 13% failure rate at 10 years post-procedure, leading to pouch excision (P.E.). A systematic review of 14 studies involving 1601 patients found that complications after P.E. are common, with mortality rates ranging from 0.58% to 1.4%. Quality of life is generally lower in P.E. patients compared to the normal population, often experiencing urinary and sexual dysfunction post-operatively. Further research is needed to compare QoL in patients undergoing P.E. and other treatment options to definitively assess post-procedure QoL.

Summary: This retrospective cohort study investigated the management and outcomes of pelvic sepsis after re-do IPAA. It suggests that early detection of pelvic sepsis and appropriate management strategies can improve pouch salvage rates, and there were no significant differences in functional outcomes and quality of life compared to the no sepsis control group.

Summary: Colitis-associated colo-rectal cancer is the main cause of death in inflammatory bowel diseases, and inflammation plays a key role in connecting the two pathologies. The NLRP3 inflammasome complex is important in innate immunity, but its dysregulation can lead to various pathologies like ulcerative colitis. Our review focuses on understanding the regulation of the NLRP3 complex and its role in current clinical settings. Promising results from 18 studies identified potential pathways for regulating the NLRP3 complex and its involvement in the metastatic process of colo-rectal cancer. However, more research is needed to validate these findings in a clinical setting.

Summary: Horizontal glottectomy (HG) is a safe surgical option for T1a-T1b glottic tumors, with comparable oncological outcomes to other treatments. It can be considered when there is poor laryngeal exposure, patient refusal of radiotherapy, or contraindication for radiation therapy. This systematic review included 14 articles and a total of 420 patients who underwent HG, with a laryngeal preservation rate of 93.6% and a mean post-operative hospitalization duration of 11.7 days.

Summary: This meta-analysis examined the feasibility and safety of robotic surgery for elderly patients (>70 years old) undergoing curative treatment for rectal cancer. The study found that elderly patients can undergo robotic resection with similar surgical quality and outcomes as young patients, without increasing postoperative mortality and morbidity. Therefore, robotic surgery is a viable option for elderly patients with rectal cancer.

Summary: This systematic review and meta-analysis examined the effectiveness of consumer-friendly contactless vital sign monitors compared to medical grade instruments. The results showed that consumer-grade monitors were accurate in measuring heart rate compared to medical devices, but there are limitations such as motion and poor lighting. More studies are needed to assess the accuracy of contactless blood pressure and respiratory rate monitors, and further validation in larger populations is required for clinical use.

Summary: During the COVID-19 pandemic, emergency rapid intubation teams (ERITs) in two Canadian hospitals demonstrated high success rates in airway management for critically ill patients with respiratory failure, along with high adherence to personal protective equipment (PPE) use. Common occurrences of hypoxemia and hemodynamic instability within the first 15 minutes following intubation should be anticipated.

Summary: Tracheal intubation is a common and high-risk procedure in critical care, with potential adverse events that increase the risk of death. Critically ill patients have a higher risk of peri-intubation complications due to their deranged physiology. Preoxy-genation strategies have been studied, but evidence on interventions to mitigate haemodynamic collapse after intubation is inconclusive. Airway management in COVID-19 patients is challenging due to the added risk of infection for healthcare workers. This review aims to update the evidence for intubation in critically ill patients, focusing on understanding peri-intubation risks and evaluating interventions to prevent or mitigate adverse events.

Summary: One of the most significant limitations of oximeters is their performance under poor perfusion conditions. This systematic review examines pulse oximeter model accuracy in adults under poor perfusion conditions. The review found that most oximeters are accurate in patients with poor perfusion, especially newer models and those placed on earlobes. Further studies are needed to examine multiple oximeter models used on a diverse selection of patients while following FDA recommendations to determine oximeter accuracy.

Summary: A study conducted in 2019 to 2020 investigated the added value of difficult airway scenarios in virtual reality fiberoptic intubation training. The study found that training on difficult airways did not offer an advantage over training on normal airways. This may be due to the learning curve required to reach proficiency in a specific airway scenario.

Summary: This study aimed to determine the accuracy of a deep learning approach in classifying segments of capnography waveforms as either 'breath' or 'no breath'. By using a convolutional neural network to classify 15 s capnography waveform segments from a group of 400 participants, false breath alarms were suppressed. In internal-external validation, the neural network achieved an accuracy of 0.97, precision of 0.97, and recall of 0.96. Overall, the neural network showed promising results in reducing false capnography alarms. Further research comparing its performance with the standard approach is needed.
