This review aims to determine the specific effects of PA on systemic levels of interleukins and inflammatory markers. A systematic literature search was conducted in three computerized bibliographic databases (Medline, Embase, CENTRAL) to identify randomized controlled trials and matched case studies. Applied key words were: RA and PA including the terms exercise, exercise therapy, gymnastics and exercise movement techniques. Inclusion criteria were data on all types of proinflammatory interleukins (IL), C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR). For data synthesis, the populations, interventions and outcomes were described according to the PRISMA statement. A total of 1289 publications were found. Fifteen papers, related to 14 different study populations, met the inclusion criteria. No study revealed a significant change regarding IL or CRP levels in response to the intervention (PA). In three study populations, a significant reduction of the ESR was identified, but the effect from PA was not discernible from effects of changes of the anti-rheumatic medication in these studies. The strong variability in study designs, cohort size and types of physical training programs remains an obstacle in the assessment of the measurable effects of PA on inflammatory markers in patients with RA. At present, there is no sufficient evidence to conclude that PA has a significant impact on systemic levels of inflammatory markers in RA.

Summary: This study evaluated 50 adults with secondary OA due to rheumatic diseases, finding distinct frequencies and risk factors of low BMD in THA and TKA patients. The findings suggest a potential value of DXA in the preoperative evaluation of bone status.

High prevalence and undertreatment of osteoporosis in elderly patients undergoing total hip arthroplasty

Summary: The study found a high prevalence of low bone mineral density in elderly patients with end-stage osteoarthritis scheduled for total hip arthroplasty. Routine DXA measurements should be considered in these patients to prevent undiagnosed and untreated osteoporosis, which could impact the success of osseointegration and other clinical outcomes.

Summary: Our study demonstrates that cytological evaluation of spinal lesions suspected of malignancy can provide a quick and reliable diagnosis of the underlying disease in patients with new-onset malignant spinal lesions, facilitating appropriate treatment. Implementing this process as a routine clinical diagnostic can shorten the time to diagnosis and improve treatment outcomes for this vulnerable patient group.

The Use of Intraoperative Cell Salvage in Two-Stage Revision of Septic Hip Arthroplasties: A Double-Center Retrospective Study

Lara Krueger, Andre Strahl, Leon-Gordian Koepke, Bernd Fink, Frank Timo Beil, Jan Hubert

Summary: This study evaluated the use of intraoperative cell salvage (ICS) during reimplantation of hip arthroplasties. Positive intraoperative cultures were found in 13 cases, but no significant correlation was found between the detection of positive cultures and the occurrence of complications. The use of ICS during reimplantation was considered to be indicated in terms of patient blood management, but further studies are needed for septic revision arthroplasty.

Summary: This study retrospectively analyzed data from 229 patients with distal femur fractures and found that lateral locking plate osteosynthesis was the main treatment method. The complication rate was 14.4% and was associated with increased BMI, increased fracture displacement, and specific fracture types.
