Occupational exposure during endovascular aneurysm repair (EVAR) and aortoiliac percutaneous transluminal angioplasty (PTA) procedures

ObjectivesThe purpose of this study was to determine the radiation exposure of primary interventionalist's different body parts during endovascular aneurysm repair (EVAR) procedures and aortoiliac percutaneous transluminal angioplasty (PTA) procedures and to evaluate the efficacy of a radioprotective drape.MethodsOccupational doses for 36 consecutive aortoiliac PTA procedures and 17 consecutive EVAR procedures were estimated using thermoluminescence dosimetry (TLD) chips (TLD-200, Hashaw, Solon, OH). Effective dose (ED) was calculated using the Niklason algorithm. For the evaluation of a 0.25mm Pb equivalent drape (Ecolab, Saint Paul, Minnesota, USA), experiments were performed using two physical anthropomorphic phantoms (Rando-Alderson Research Labs, CA, USA).ResultsMedian ED for a typical EVAR and PTA procedure was 4.71.4Sv and 4.4 +/- 3.6Sv, respectively. The highest radiation doses were measured for the operator's hands in both procedures. Moreover, considerable doses were measured to the operator's head, eye lenses and thyroid. Due to the use of the drape, radiation exposure of primary operator's abdominal area, genitals, thyroid and eye lenses was reduced by an average of 59%, 60%, 65% and 59%, respectively. However, dose area product (DAP) and peak skin dose(PSD) were increased by 20% when part of the drape was placed into the X-ray field.Conclusion

Management of Inadvertent Arterial Catheterization during Central Venous Catheter Placement: A Case Series

Summary: Percutaneous central venous catheterization is a commonly used technique, but it is not without complications, even with real-time ultrasound guidance. Arterial puncture is a well-known complication, and unintended subclavian or carotid artery cannulations during central venous catheterization attempts are rare but documented complications that can have serious consequences. Recently, endovascular balloon tamponade has become the preferred initial approach for repairing inadvertent arterial cannulations.

Automatic Radiobiological Comparison of Radiation Therapy Plans: An Application to Gastric Cancer

Summary: A new software tool was developed for quick and automatic calculation of tumor control probability (TCP) and normal tissue control probability (NTCP) in radiation therapy plans. This study compared different techniques for gastric cancer irradiation and found that intensity-modulated radiation therapy (IMRT) and volumetric modulated arc therapy (VMAT) provided higher TCPs and reduced the risk of late adverse effects in surrounding organs compared to three-dimensional conformal radiotherapy (3D-CRT).

Computational modelling to evaluate abdominal aortic aneurysm rupture risk: state-of-the-art, pitfalls and scientific impact

Summary: Abdominal aortic aneurysms (AAAs) are a potentially lethal health condition commonly found in the aging population of developed countries. Current management is based on the maximum diameter criterion, but this often leads to diagnostic inaccuracies. Individualized patient-specific rupture risk estimation is desirable, and computational modelling can provide insight into AAAs' progression and rupture.

Effect of Sac Asymmetry, Neck and Iliac Angle on the Hemodynamic Behavior of Idealized Abdominal Aortic Aneurysm Geometries

Summary: This study aims to evaluate the effects of aortic neck angulation, angle between the iliac arteries, and sac asymmetry on the hemodynamic variables of abdominal aortic aneurysms (AAAs). The results show that increasing neck and iliac angles result in more favorable hemodynamic conditions, while asymmetrical configurations are more advantageous. Therefore, these parameters should be taken into account when assessing the geometric characteristics of AAAs.

A neural network-enhanced methodology for the rapid establishment of local DRLs in interventional radiology: EVAR as a case example

Summary: The purpose of this study is to develop a neural network-enhanced workflow for the establishment/update of local diagnostic reference levels (DRLs) in interventional radiology using endovascular aneurysm repair (EVAR) procedures as a case study. The proposed algorithm can automatically extract dose parameters from dose reports and calculate relevant indicators. The accuracy and time efficiency of the algorithm were evaluated, showing promising results for the establishment of DRLs.
