Summary: Kawasaki disease (KD) is a childhood vasculitis that primarily affects medium vessels, particularly the coronary arteries. Coronary artery aneurysm occurs in about 25-30% of untreated cases, leading to significant morbidity. Intravenous immunoglobulin (IVIg) has been consistently shown to reduce the risk of coronary artery aneurysms to less than 5%, but the mechanism of immunomodulation is still unclear. Studies suggest that IVIg may modulate the innate immune system by affecting toll-like receptor pathways, autophagy, apoptosis, neutrophil extracellular trap, dendritic cell modulation, T-cell differentiation, cytokine release, and regulatory T-cell function. This review discusses the potential mechanisms underlying the immunomodulatory actions of IVIg in KD patients and summarizes the evidence regarding infusion protocols and dosages used in treatment.

Summary: This study aimed to determine the risk factors for resistance to intravenous immunoglobulin (IVIG) treatment and coronary artery aneurysm (CAA) development in Chinese pediatric patients with high-risk Kawasaki disease. The Xie Liping scoring system was found to be the most appropriate method for identifying high-risk patients. IVIG resistance could be predicted based on the bilirubin-to-albumin ratio. Serum albumin level and Z score of the left main coronary artery were independent predictors of CAA development.

Summary: This study examined the safety and efficacy of using anakinra in patients with IVIG-resistant Kawasaki disease. The results showed that the majority of patients became afebrile within 48 hours of the last dose escalation of anakinra, with a reduction in disease activity and normalization of CRP levels. Although some patients experienced adverse events, anakinra was generally well tolerated and showed some effectiveness in reducing fever and inflammation markers in IVIG-refractory patients with KD.
